AGENDA

• On May 5th 2020 the Members of CNYCC met and approved the recommendation of the Board of Directors to wind-down the DSRIP 1.0 activities of the Central New York Care Collaborative by December 31, 2020.

• We remain committed to our Partners and will continue to provide services and work to sustain the good work the PPS and its partners have accomplished over the past 5 years

• We thank you, our Partners for your hard work and dedication to improving the quality delivered to our community over the past 5 years.

CNYCC Wind Down

• As part of CNYCC’s Board-approved wind-down by 12/31/2020, CNYCC is prioritizing key activities including supporting the “approved funds flow methodology and distribution plan for allocating any remaining DSRIP payments” with a target of November for final partner payments

• One of CNYCC’s primary strategies to expedite the flow of funds to partners is to support partner organizations in earning their full Phase IV funds flow allocations, which also minimizes the amount of unearned funds that must be distributed later

• Additionally, CNYCC is evaluating the due dates for the final VBP readiness & implementation deliverables, and is seeking Board approval to accelerate the date from 10/31 to 9/30

Dear Valued CNYCC Partner Organization,

You are receiving this email as a designated contact person for your organizations' participation in the Central New York Care Collaborative through a

direct Phase IV Project Agreement or through a collaborative agreement with one of CNYCC's designated safety net hubs.

In your ShareFile folder, you will find your organization's Initial Phase IV 6/30/20 Performance Activity Scorecard containing your organization's

Performance Activity progress to date as well as initial activity scores for your organization's assigned Performance Activities based on CNYCC's initial

review of submissions due on or before 6/30/2020:

• A 'Green' activity score indicates that CNYCC's review of your submitted documentation uncovered no errors and payment has been earned for the

Performance Activity. No further partner action is required.

• A 'Yellow' activity score indicates that errors were uncovered during CNYCC's review, including no submission received for certain PAs. Partners must

remediate & resubmit documentation for the PA to be eligible for payment.

• NEW FOR PHASE IV: CNYCC has opened a Last Chance Opportunity for SELECTED Red Scores in Phase 4, offering a remediation period to

resubmit missed documentation. Along with your Q4 PA/VBP activities with a score of Yellow, you now have this ONE-TIME opportunity to resubmit

any previously Q1 – Q3 Red-scored activities. In your Scorecard, you will see a number of previously scored Red activities, now scored Yellow. These

Yellow scores are included in your Q4 Remediation Webform. This is the ONLY time you have to resubmit – there will be no other remediation period.

If you do not take this opportunity to resubmit these missed opportunities, the activities will receive a final score of Red.

If your organization received any 'Yellow' activity scores, later today your designated DSRIP coordinator will receive an email notification containing a

link to a webform to submit any documentation requiring remediation. The remediation window for ALL Yellow Performance Activities ends at 5:00

pm on Tuesday, July 28, 2020. CNYCC technical support will NOT be available after that time. Documentation will not be accepted if submitted after

that time or via any other mechanism than what is described in the CNYCC Partner Reporting Guide available on the CNYCC website (log-in required).

Do I need to participate?

• Only if you received a ‘Yellow’ on previous Activities and want to ensure you get a ‘Green’

When does Spirit Week start?

• Monday, July 20th!
