From the Nurse: A dental exam is required by the state of Illinois for students in grades Kindergarten, 2nd grade, and 6th grade. Please turn in a completed dental from to the school by May 15th, 2018 for the current school year. If your student had a recent exam, please request that the dentist fill out a dental form from that visit. A
waiver form from the school will be provided upon request. If you have any questions, feel free to contact us.
