Willem Kuyken, PhD, is Professor of Clinical Psychology at the University of Exeter, United Kingdom; Cofounder of the Mood Disorders Centre; and Fellow of the Academy of Cognitive Therapy. Robert Dudley, PhD, is a Consultant Clinical Psychologist for the Early Intervention in Psychosis Service at the Northumberland, Tyne, and Wear Mental Health NHS Trust, United Kingdom. He is currently serving as a Beck Institute Scholar at the Beck Institute for Cognitive Therapy and Research. Dr. Dudley’s primary clinical and research focus is the understanding and treatment of psychotic symptoms. Presenting an innovative framework for tailoring cognitive-behavioral interventions to each client’s needs, this accessible book is packed with practical pointers and sample dialogues.

Step by step, the authors show how to collaborate with clients to develop and test conceptualizations that illuminate personal strengths as well as problems, and that deepen in explanatory power as treatment progresses. An extended case illustration demonstrates the three-stage conceptualization process over the entire course of therapy with a multiproblem client. The approach emphasizes building resilience and coping while decreasing psychological distress. Special features include self-assessment checklists and learning exercises to help therapists build their conceptualization skills.

Maintenance Factors Just as many different factors can be triggers, there are many pathways to problem maintenance. When CBT therapists look for maintenance factors they usually first consider reinforcement factors...

Ironically, sometimes a person’s chosen coping methods can maintain the problem. This is because people often prefer coping approaches that are immediately reinforcing; those that lead to immediate rewards or reductions in distress. Solving the root causes of distress can involve a temporary increase in discomfort. Therefore, CBT therapists identify the long-term costs of coping methods as well as the short-term benefits.

Mark’s therapist wonders whether Mark’s preferred methods for dealing with his low mood may inadvertently maintain his difficulties. The following dialogue illustrates how Mark’s therapist helps him link his avoidance with maintenance of low mood.

THERAPIST: OK, let’s see what we can figure out together. Looking at these examples (pointing to Figure 6.3), what do you think may keep the problem going?

MARK: (Looks at Figure 6.3 for a minute silently.) Well, it doesn’t help when I just give up on things.

THERAPIST: What do you mean?

MARK: Well, when I was in the shop and my mood crashed I just went home to bed. That didn’t help, did it? It is not like I went and got ready for the meeting, or put time aside on Sunday to do it.

THERAPIST: That’s a really good observation, Mark. (Smiles encouragingly.) What do you think led to you to go home to bed like that?

MARK: Well, at the time it was just better to be away from the situation. I just wanted to be home and not think about it. It helped me a little at the time.

THERAPIST: So, at the time it helps a little to avoid your problems. Thinking about it now, how much do you think it helped in the long run?

MARK: In the end it made it worse, because the pressure just built up.

THERAPIST: So what if we draw an arrow between going home to bed and not being ready for the work meeting to remind us that avoidance might be something that keeps your problems going?

MARK: Yeah, I can see that.

Mark’s therapist then suggests they review the other examples discussed previously to help establish whether this is a common pattern.
