COMMON GUIDELINES FOR ONLINE REGISTRATION AND APPLICATION FOR SELECTION IN TO DIPLOMA IN PHARMACY (D-Pharma) COURSES IN ODISHA PRE REQUISITES FOR ONLINE REGISTRATION & ONLINE APPLICATION : The Candidate should have the following documents / information ready before applying online for D- Pharma Courses. 1. COMMON GUIDELINES FOR ONLINE REGISTRATION AND APPLICATION FOR SELECTION IN

TO DIPLOMA IN PHARMACY (D-Pharma) COURSES IN ODISHA

PRE REQUISITES FOR ONLINE REGISTRATION & ONLINE APPLICATION : The Candidate should have the following documents / information ready before applying online for D-

Pharma Courses.

1. Scan copy of the Passport Size Color Photograph, preferably the weight should be less than

100Kb, and should be saved in .jpeg/.jpg format of Size-within 4 ½

cm and 3 1/2

cm. (length x

breadth) or in Pixel 140 x 175 Pixel

2. Scan copy of the Candidate’s Full Signature, preferably the weight should be less than 20Kb,

and should be saved in .jpeg/.jpg format of Size-within 6 cm and 2 cm (length x breadth) or in

Pixel 300 x 51 Pixel

3. Having a valid e-mail address, if not, should open one email address.

4. Should have copy of the Matriculation Certificate (for Date of Birth) and Copy of the

Qualifying Examination (10+2 Science).

10 COME FOR ADMISSION WITH FEES TO THE CONVENOR OFFICE STEP-1

1. The Candidates are advised to visit the DMET Web site www.dmetorissa.gov.in

2. In the Right panel of the website, find the Link in Right Side Pharmacy Admissions & click on that.

3. Download the prospectus for this year and thoroughly go through this.

4. Make all the above documents ready and then click – CLICK HERE FOR ONLINE

REGISTRATION, APPLICATION , CHOICE LOCKING, ALLOTMENT LETTER ETC

5. You will be redirected to the following page.

6. Download the Instruction Manuals given under Notifications if you need it

7. Click on the Blue colour Button where D. Pharmacy is written.

8. You will be redirected to the following page, where you can get the details PROGRAMME FOR

ONLINE COUNSELING PROCESS- D PHARM

8. Below of this page, you can Find a few red / pink cloloured Buttons and Click on the Button

ONLINE REGISTRATION AND APPLICATION.

9. The following Page will be redirected to you where you have to Preliminary Register your e-

mail id (valid) , Name (as mentioned in Matriculation Certificate) etc.

10. Your e-mail ID will be your Registration ID for all future Logins.

11. You have to enter your Password (basically the password should be Minimum 6 characters,

may be alpha / numeric, combination of alpha numeric etc and within 8 characters long.

12. You have to put a secret question and Secret answer for your password, in case you forgot

your password, then you will be asked for your secret question and answer for intimation of

your password.

13. After filling up all the basic information under Preliminary Registration page, You Click CLICK

HERE TO PROCEED FOR ONLINE APPLICATION.

STEP-2

1. You will be redirected to the following page which is meant for Online Application.

2. Your Name will be automatically Shown here as you have entered during the Preliminary

Registration , (Step-1)

3. Please fill up the information like Father’s Name / Guardian’s Name , Date of Birth (Pick the

date details from the Calender control), Select Sex from the drop down menu, Select Yes or

No in case you have passed 7th

up to ME Standard etc.

4. Select the Religion you have like Hindu, Christian or Others

5. Please select your Category from the menu given. If you belong to more than one Category,

like if you belongs to SC but you are also having a Green Card, an Ex-Ser Certificate of your

Father, and a Physically Challenged / Handicapped, then You have to Select all these

Categories if you want to claim these. For this, you have to select all these SC, GCH, EX-S,

PH by pressing the Ctrl Key.

6. Please select from the drop down menu, whether you belong to PH (Physically Challenged) /

Dibyang or NOT and whether you reside in the State Odisha or No as per the Clause 5 of the

Prospectus.

6. Please enter your Permanent address and if your permanent address is same as your present

address, then please select the Chk Box (Tick mark) present in the middle. If not, you can fill

your both the address.

7. Please do not try to fill up the e-mail address as the e-mail id which is registered during Step-1,

will be automatically shown here. 8. Under the Section Academic Details, please select the Qualifying Examination from the Drop

Down menu. Name of the Board / Council from which you have passed, The Year of your

Registration (means year of Admission) and the year of Passing. You have to mention the No of

attempts that you have taken to pass the qualifying examination. Means, if you have failed in

Annual Exam, and passed in Supplementary exam, you have to mention No of attempts as

2, also if you have dropped the Annual exam and appeared the exam in the next year, then

also you have to mention the number of attempts as 2. 9. Then please fill up the marks secured from the Full marks in the qualifying Examinations. In this

section, you have to enter the Full marks as well as Marks secured in the qualifying examination

individually.

In Case of the Subject, where Practical Marks are there like Subjects of Physics, Chemistry,

Biology as individual or as a Combination of Botany and zoology, you have to mention the

Full Marks including Theory and Practical. Like also, in the Secured Marks Column, you

have to Put your combined Theory and Practical Marks.

Also If your Theory and Practical Marks are separately mentioned in the Mark Sheet, you

can fill it up separately. And in case your mark sheet reflects cobinely your Theory and

Practical Marks , you can write the combined marks against Theory only and Put Zero

against Practical Marks section.

If you have passed your Qualifying Exam from a Board / Council, where you have obtained

Grades, then You look after the back side of your Certificates where the Conversion

calculation is given. In general and most of the cases, the Grade should be multiplied with

9.5 and the percentage /marks will be calculated.

10. You have to Calculate your marks combined in Physics, Chemistry, Biology and Physics,

Chemistry, Mathematics separately including your practical marks also . The Total marks

secured in these three subjects will be calculated separately and entered the Marks which is

higher. That means if your combined marks in case of (Physics + Chemistry + Biology including

practical marks) is higher than the combined marks in case of (Physics + Chemistry +

Mathematics including practical marks), you have to enter your combined Marks of (Physics +

Chemistry + Biology), and if it is less, then the combined marks of (Physics + Chemistry +

Mathematics) Always add Practical Marks while calculating. 11. As, lIke if you have secured 50 in Phy, 45 in Che, 55 in Math, 60 in Bio out of 100 in each subject,

then the combined marks in Phy+Che+ Math= 150, and the combined marks in Phy+Che+ Bio =

155, then you have to mention Full Marks 300 and Secured Marks = 155 against point no-8 of 19

12. Enter your Grand Total Marks with Extra Optional (Ful Marks as well as Marks Secured in

respective columns / Text Boxes). If you are old student and in your Mark Sheet Marks clearly

marked as Grand Total Marks without Extra Optional and With Extra Optionals, then please

mention according to the mark sheet. If you are a recent pass out and nothing Optional like

mentioned in the mark sheet, then you have to calculate the total no of subjects marks appeared

in the examination and out of which all total subjects appeared how much you have secured. In

*** Do Not enter your Application Fee Details Section as this will be filled later after giving the payment online.

13. Then Upload the scan copy of the Pass port Photo and Signature as mentioned and Click the SUBMIT Button.

STEP-3

1. After filling up all the information successfully (Except the Application Fee Section) , you will

be redirected to the Print Application Page which will look like below.

In this Page , you will find all the information that you have entered in previous Step. Here just

write down the Application Number in a plain paper which will be required to give

Application Fee through Online Mode. But it is advised not to take the Application Print

Out in this stage as the Application process is incomplete as no payment of Application

Fee is entered.

If you find any information is wrongly entered and reflecting in this page, you can click the Option

BACK TO HOME to EDIT the DATA / INFORMATION ENTERED.

STEP-4

1) Now log out from the System (You can see the Right Corner of the website application form) and follow the instructions how to give the payment online / Application Fees Online.

1) Open www.onlinesbi.com in any browser like Google Chrome / Fire Fox / Internet explorer 2) Then Click on SB Collect (As shown below link)

3) Then Click the Box / Select the Box (Tick Mark will appear)

4) Then Click on Proceed 5) Then Select Odisha 6) Then Select Educational Institutions

7) Then Select SCB Medical College Cuttack and Click on Submit button

8) Then Select D Pharm Counseling 2019-20

9) Then Fill up the desired Information and Write the Application Number allotted to You and then click Submit

10) Then Check all the Information entered by You and Click Confirm

11) Then you can select to give the payment either Debit Card / ATM card / Net Banking of any Bank. If you do not have any ATM / Debit Card / Credit Card / Net Banking facility you can select the last option Other Payment Mode and take the print out of the Challan and can visit any State Bank of India branch to deposit Cash against the challan (Within 2 days preferably).

12) After the payment is successful you can chk , you will be given a bank reference Number DUB XXXXXX which you can enter in the Application Form.

13) After the payment is successfully done, please log in to the Online Application again, Click on Edit option and Enter your Bank Reference DUB XXXXXX and Update the Application Form and Take the Print out of the Application Form to Submit during the document verification process.

14) Pl remember that, the Bank Challan should be in the Name of the Candidate it self. 15) Once the payment against the application is successfully done, no way the Application fee will be

returned back to the Candidate. 16) If you are facing any problem related to your transactions, you may contact your Bank and Convener, D

Pharm Selection Committee will held no responsibility for this.

STEP-5

1. Now , after the payment is done successfully, again open the Online Application Link by

Clicking LOGIN

2. To enter the Application Fee details, Please Click the Link EDIT.

3. You enter the Online SBI Reference Number, Date of Payment, Bank State Bank of India.

4. After updating the Application Fee / Bank payment details, if you want to change any

information that seems to be wrong and needs any correction you can edit the same.

5. After making necessary corrections, you can find a Button below of the Page UPDATE and

you can click on that.

6. Then, You can take the Print out of the updated / Changed / Corrected information.

STEP-6

1. After taking the Printout of the Online Application, Keep one copy of the Application with You

for your future reference and write your user id (e-mail) and Password on that and Do not

disclose your user id (e-mail id) and Password to others. If your password is mis-used by

others, then you will be sole responsible for this.

2. Then after taking the Print out, in the other copy, Please Sign (Full Signature) in the place

Undertaking by the Candidate Section and mention the Date.

3. Then keep all the Xerox copies of the Certificates ready as indicated in the instructions

(Instructions for document verification will be uploaded in due course of time)

4. You need not to send the Application and Documents to the Authority through any

means. In case you send the application, your application will be out rightly rejected.

5. For Document Verification, the venue will be intimated in the website prior to the Verification

Schedule and You have to bring the Application and the Copy of the Certificates along with the

Original Copies for Document Verification. If you failed to verify your documents, then your

Application will be Rejected.

6. If you are not able to come physically for document verification because of some reasons, You

may authorize some body with an Application and can send the Application form, Xerox copy

of documents, Original documents and valid ID Proof of yours and the Authorized Person’s for

Document verification.

7. After the Document Verification, the Valid Applications along with Common merit Rank will be

published in the web site.

8. If you will get a valid Rank / Merit, then during the Schedule , you will be asked to Keep

Choices where you are interested for admission and the details instructions of Choice Filing

will be available in the website.

9. You are advised to visit the website before any Schedule in regular intervals for any New

Notifications / Changes. As no communication in any other means will be carried on in this

regard.

STEP-7

1. If you have lost your Print out of the Application, or Not taken the Print out at the time of Online

Application, then nothing to be worried. You can Click the LOG IN button, enter your user id

(valid e-mail that you have registered and password you have given during registration) and

can Edit the Information, Take the Print out of the Application any time before document

Verification.

GENERAL INFORMATIONS

1. In case you forget your password given, you have to mail from your e-mail address to the e-

mail- [email protected] for your password.

2. If you have registered one e-mail ID once, then the same e-mail Id will not be allowed for

Registration Next Time.

3. If the e-mail ID and the Name that you have entered during Registration process, will be same

, if registering for second time, it will not be accepted.

4. If Photo and Signature scan copy having other formats than the formats mentioned as well as

more in weight than the weight mentioned, it will show error.
