COMMISSION STAFF'S SECOND REQUEST FOR INFORMATION TO KENTUCKY POWER COMPANY

Kentucky Power Company ("Kentucky Power"), pursuant to 807 KAR 5:001 , is to

file with the Commission the original and five copies of the following information , with a

copy to all parties of record . The information requested herein is due on or before

September 1, 2015. Responses to requests for information shall be appropriately

bound , tabbed and indexed. Each response shall include the name of the witness

responsible for responding to the questions related to the information provided.

Each response shall be answered under oath or, for representatives of a public

or private corporation or a partnership or association or a governmental agency, be

accompanied by a signed certification of the preparer or person supervising the

preparation of the response on behalf of the entity that the response is true and

accurate to the best of that person 's knowledge, information , and belief formed after a

reasonable inquiry.

Kentucky Power shall make timely amendment to any prior response if it obtains

information which indicates that the response was incorrect when made or, though

correct when made, is now incorrect in any material respect. For any request to which

Kentucky Power fails or refuses to furnish all or part of the requested information ,

Kentucky Power shall provide a written explanation of the specific grounds for its failure

to completely and precisely'respond .

Careful attention should be given to copied material to ensure that it is legible.

When the requested information has been previously provided in this proceeding in the

requested format , reference may be made to the specific location of that information in

responding to this request. When applicable, the requested information shall be

separately provided for total company operations and jurisdictional operations. When

filing a document containing personal information , Kentucky Power shall , in accordance

with 807 KAR 5:001 , Section 4(1 0) , encrypt or redact the document so that personal

information cannot be read.

1. Refer to Kentucky Power's Response to Staff's First Request for

Information, Item 6, Attachment 1, which provides the accounts receivable history

broken down by tariff code for Leodia Easterling's account for electric service at 440

31st Street, Ashland , Kentucky. Provide the accounts receivable history broken down

by tariff code for Douglas Morrison's account for electric service at 440 31st Street,

Ashland, Kentucky, from April 1996 to May 2015.

2. Refer to Appendices A, B, and C, which are attached hereto . Appendix A

is Kentucky Power's Tariff O.L. (Outdoor Lighting) with an effective date of April 1, 1991,

which was in effect when the outdoor light was installed on Kentucky Power Pole No.

2184 on April 6, 1992. Appendix B is Kentucky Power's Tariff O.L. (Outdoor Lighting)

with an effective date of July 1, 1992, which was in effect when Leodia Easterling

established electric service at 440 31st Street, Ashland , Kentucky, in December 1994.

Appendix C is Kentucky Power's Tariff O.L. (Outdoor Lighting) with an effective date of

August 2, 1995, which was in effect when the Morrisons established electric service at

440 31st Street, Ashland , Kentucky, in April 1996.

a. Confirm that, pursuant to Kentucky Power's Tariff O.L. in effect on

April 6, 1992, mercury vapor outdoor lighting lamps were not available for new

installations.

b. Confirm that, pursuant to Kentucky Power's Tariff O.L. in effect in

December 1994, mercury vapor outdoor lighting lamps were not available for new

installations.

c. Confirm that, pursuant to Kentucky Power's Tariff O.L. in effect in

April 1996, mercury vapor outdoor light lamps were not available for new installations.

3. Refer to Kentucky Power's Answer, numbered paragraphs 5(e) and (g) .

Explain why Kentucky Power states that the lamp installed on Complainant's outbuilding

appears to be a mercury-vapor lamp.

4. Refer to Mr. and Mrs. Morrison's Response to Staff's First Request for

Information, page 20, which is represented to be a picture of the bulb from the lamp

over the Complainant's outbuilding. According to the Morrisons, the base of the bulb is

engraved with the following : JFMAMJJASOND-1234567890-R. Confirm that the bulb

appears to be a high-pressure sodium retrofit bulb for mercury vapor lamps.

Bills computed according to the rates set forth herein will be increased or decreased by a System Sales Factor per KWH calculated in compliance with the System Sales Clause contained in Sheet Nos. 19-1 and 19-2 of this Tariff Schedule.

DELAYED PAYMENT CHARGE.

A delayed payment charge on residential customer accounts will be applied pursuant to the delayed payment charge on Tariff R.S. On all accounts other than residential not paid in full within 15 days of date of bill an additional charge of 5% of the unpaid portion will be made.

HOURS OF LIGHTING.

ALL Lamps shall burn from one-half hour after sunset until one-half hour before sunrise every night and all night, burning approximately 4,000 hours per annum.

OWNERSHIP OF FACILITIES.

AL'L facilities necessary for service including fixtures, controls, poles, transformers, secondaries, Lamps and other appurtenances shall be owned and maintained by the Company. ALL service and necessary maintenance will be performed only during the regular scheduled working hours of the Company.

Bills computed according to the rates set forth herein will be increased or decreased by a Fuel Adjustment Factor per K\JH calculated in compliance with the Fuel Adjustment Clause contained in Sheet Nos. 5·1 and 5·2 of this Tariff Schedule. The monthly kilowatt-hours for Fuel Adjustment Clause and the System Sales Clause computations are as follows:

SYSTEM SALES CLAUSE.

Bills computed according to the rates set forth herein will be increased or decreased by a System Sales Factor per K\JH calculated in compliance with the System Sales Clause contained in Sheet Nos. 19·1 and 19·2 of this Tariff Schedule.

DELAYED PAYMENT CHARGE.

A delayed payment charge on residential customer accounts will be applied pursuant to the delayed payment charge on Tar i ff R.S. On all accounts other than residential not paid in full within 15 days of date of bill an additional charge of 5X of the unpaid portion will be made.

HOURS OF LIG HTING.

All lamps shall burn from one·half hour after sunset unti l one-half hour before sunrise every night and all night, burning approximately 4,000 hours per annum.

0\JNERSHIP OF FACILITIES.

All facilities necessary for service including fixtures, controls, poles, transformers, secondaries, lamps and other appurtenances shall be owned and maintained by the Company. All service and .necessary maintenance will be performed only during the regular scheduled working hours of the Company.

The Company shall be allowed 3 working days after notification by the customer lamps.
