The Application Packets must contain the following: Three (3) identical copies of the duly accomplished official Application Form (only one copy for each packet). All copies must be originally signed in ink. The designated official representative shall sign the Application Form. Group Picture: Size 4” or 6" colored photograph, with the name of the group printed legibly in ink at the back of each photograph. The group picture must include only the following: performers, accompanist for choir (if any), andconductor. Application Packets sent through the Philippine Postal Corporation should be postmarked on or before 18 September 2015. (Note: new deadline) The Competition Secretariat shall not be responsible for any circumstances which will prevent the arrival of the Application Packet at the designated address on the designated deadline. Participants must show proof of enrollment (for students,most recent From 5) and employment (for faculty and staff). The CUs will provide accommodations, meal allowances, and transportation to and from the competition venue during the final round. Established UP choirs are not eligible to join the competition. (UP Madrigal Singers, UP Singing Ambassadors, UP Concert Chorus, UP Manila Chorale, UPLB Choral Ensemble, and other touring groups.) The decision of the board of judges is final.
