SERIES TOPIC: COMPLEX CARE PEDS
Series begins April 27, 2016 and meets once per month, every 4th Wednesday through October. Sessions are conducted via live video from your home or office during lunch hours, 12:00 -1:00 p.m. MST.
• Better assess and manage the care for children with medical complexity • Create a shared care plan • Engage with other types of providers involved in the care of a child with medical complexity • Identify and share resources
WHAT TO EXPECT
Live web-based, video-conference sessions. Use your smart phone, tablet or laptop with a camera and click on the link we send you.
During each 60-minute session, a subject matter expert will lead a brief presentation. Participants then present a case for feedback, recommendations and discussion.
