Harmony for Aging and Adult Services™ SAMS Case ManagementAt the core of Harmony’s modular solution for adult and aging services is SAMS Case Management—a comprehensive integrated case management system that enables agencies to share and manage all consumers in a single, secure database across the state, including local agencies and providers.
SAMS Case Management includes a wealth of features designed specifically for organizations to plan, track, manage, and pay for services delivered to their consumers. Care managers are automatically alerted to new referrals and consumers through a real-time dashboard, ensuring that important activities, tasks, and events are never missed. The care manager can then begin working with assigned consumers to conduct in-depth assessments using the customizable Harmony for Aging and Adult Services Assessments. These assessments provide a historic view of all previous answers given by the consumer and also provide powerful real-time calculations to assist in eligibility determinations, scoring, and more. These calculations will help ensure that consumers are appropriately placed and receive adequate care, all the time.
Once enrolled in a program, consumers can work with one or many care managers to collaborate on the care they receive. Service plans are created and, upon review and confirmation, converted into service orders, giving service providers automatic acknowledgment of scheduled service deliveries. Service orders are immediately displayed to providers via the Dashboard.
