Program Benefits Loan Repayment Program • Offers 60 percent of total outstanding qualifying educational loan balance incurred while

pursuing an education in nursing for a 2-year service commitment • Qualifying participants may receive an additional 25 percent of their original loan balance

for a third year of service

Scholarship Program • Cover nursing school tuition and fees; An annual payment for cover expenses for books,

clinical supplies, and instruments; and Monthly stipends to cover living expenses• 2-year minimum service commitment in an NHSC-approved site in a Health Professional

Shortage Area (HPSA) of greatest need• 1-year additional service commitment per scholarship year, not to exceed 4 school years.

Practice Site Eligibility Requirements• Must provide culturally competent, comprehensive primary care

services (medical, dental, or behavioral) which correspond to the designated HPSA type.

• Must function as a part of the system of care that either offers or assures access to ancillary, inpatient, or specialty referrals.

• Must not discriminate in the provisions of services to a patient because of the individual’s inability to pay, race, color, sex, national origin, disability, religion, age, sexual orientation.

• Must accept patients covered by Medicare and have an appropriate agreement with the applicable State agency for Medicaid and CHIP.

Practice Site Eligibility Requirements• An entity must have a financial assistance policy with a discounted/

sliding fee schedule for patients who qualified to ensure that no one who is unable to pay will be denied access to services. **Free clinics, Correctional facilities, and most Indian Health Service, Tribal, and Urban Indian sites are exempt

The sliding fee scale discount program must include the following elements: 1. Be applicable to all individuals and families with incomes at or below 200

percent of the most current federal poverty guidelines/level. 2. Provide full discount for individuals and families with annual incomes at or

below 100 percent of the federal poverty guidelines/level, or allowance for an nominal charge only, consistent with the site’s policy

3. Adjust fees based on family size and income for individuals and families with incomes above 100 and at or below 200 percent of the federal poverty guidelines/level.

Health Professional Shortage Designations

Health Professional Shortage Area (HPSA) designations identify areas facing a critical shortage of health care professionals.

State and federal agencies use HPSAs to prioritize and direct limited resources to areas with unmet primary, dental, and mental health care needs.

Generally, a “MATURE” network means you have defined leadership, mission/vision, a clear path for network membership/recruitment and retention. A network organization is also considered formal if the network has a signed Memorandum of Agreement or Memorandum of Understanding, or other formal collaborative agreement, as well as signed and dated bylaws.

Statutory Charge: Strengthen the rural health care system as whole Topic: Networks will improve population health by implementing promising practice, evidence-informed and/or evidence-based approaches to address health disparities and enhance population health in their communities.

Statutory Charge: Expand access to, coordinate and improve the quality of essential health care services Topic: Integrated health networks will collaborate to expand access to and improve the quality of essential health care services by focusing on projects and/or network activities directly related to the evolving health care environment

Statutory Charge: Achieve Efficiencies Topic: Integrated health networks will focus on integrating health care services and/or health care delivery of services to achieve efficiencies and improve rural health care services.

Rural Health Network Development PLANNING Grant The purpose of the Network Planning program is to assist in the development of an integrated healthcare network, if the network participants do not have a history of formal collaborative efforts.The Network Planning program promotes the planning and development of healthcare networks in order to: (i) achieve efficiencies; (ii) expand access to, coordinate, and improve the quality of essential health care services; and (iii) strengthen the rural health care system as a whole.The goals of the Network Planning program are to help rural providers better serve their communities given changes taking place in health care, as The program will support one year of planning with the primary goal of helping networks create a foundation for their infrastructure and focusing member efforts to address important regional or local community health needs.
