administered by MetLife. The amount of the policy depends on the health plan that covers you. Call Member Services at 1-800-551-3225 if you are not sure of your plan and your insurance benefit.

SIgN uP FoR EASy oNlINE AccESSVanguard makes it easy for you to manage your 401(k). Just go to vanguard.com/register to register for secure access to your account online. When prompted, enter the 32BJ SRSP Plan number, 094163. (Vanguard is available in Spanish, too. Just go to vanguard.com/espanol.)
Once you have registered, you can go to vanguard.com/retirementplans any time and sign in to check your account balance, view your statements, learn about SRSP investment options, and more. The site includes tools, podcasts, and articles to help you with financial planning.

uPdATE youR SRSP BENEFIcIARyIt’s important to designate a beneficiary (or beneficiaries), and to keep this information current, in case something happens to you. You can designate your beneficiary online or by phone. To name or change a beneficiary online, log into your account and go to My Profile and select Beneficiaries. To do the same by phone, call 1-800-523-1188 (1-800-828-4487 for Spanish). If things change, you can go back to the site and update your beneficiary information any time.
youR lIFE INSuRANcE BENEFIT BE SuRE you hAVE dESIgNATEd youR BENEFIcIARy(IES) death. So be sure you have completed
your beneficiary designation form and have submitted it to MetLife. You can name a single beneficiary or many. If you elect more than one beneficiary, you must allocate the percentage each will receive so that the total allocated to all beneficiaries equals 100%. If you have not designated a beneficiary, you can obtain a form from Member Services at 1-800-551-3225 or at www.32bjfunds.org (select the Health tab and click on “Forms”). Your beneficiary can be anyone you choose and you can change your beneficiary at any time by completing a new form.
AddITIoNAl coVERAgEYou also have an Accidental Death and Dismemberment benefit (AD&D), which provides a financial benefit to you if you lose a limb due to an accident and to your beneficiary if you die in an accident.

If you’re at risk of developing diabetes, the YMCA’s Diabetes Prevention Program might be right for you. Studies show that lifestyle changes and modest weight reduction can prevent or delay the onset of diabetes.
That’s why the program focuses on healthy eating, increased physical activity and maintaining a healthy weight. Participants in the Diabetes Prevention Program meet in a classroom setting as a group with a trained YMCA lifestyle coach for 16 sessions to learn and practice lifestyle skills. You learn how to make
healthier food choices, how to build more physical activity into your daily routine and how to manage a healthy weight.
Right now the program is only available for Union Health Center patients with a diagnosis of pre-diabetes, but it will be available at other 5 Star Centers soon.
