In accordance with the documents of the Paradise Towers, Inc., I request your consent to make the following changes, alterations, renovations, removals/and/or additions to my unit:

This application must be submitted filled out by the unit owner and their licensed and insured contractor. Both signatures are required for this document to be accepted at the management office. If the unit owner is not available, it may be faxed and signed by the owner.

This package is intended for any owner and their licensed and insured contractor who wishes to repair, modify or remodel any part of the owner’s unit. Please read carefully below and follow the Contractor’s and Owners Rules and Regulations.

1. Residents having ANY work done MUST submit a $500.00 cashier’s checkor money order deposit to the management office. This insures both:

a. Any damage to the building can be covered.

b. And allows both Management and the ‘Association’ to insure therules are strictly adhered to.

At the completion of work, the ‘Association’ will refund the deposit if no damage to the building has been committed and if all applicable permits, if any, are closed by the City of Hallandale Beach. Note: deposits may take up to 30 days to be refunded. Your cooperation is required. Should you have any questions or concerns, please call the office @ 305-831-3604

2. ALL OWNERS MUST ENSURE THAT CONTRACTORS AND SUB- CONTRACTORS SUBMIT TO THE MGMT. OFFICE PRIOR TOCOMMENCING WORK, A COPY OF THE CERTIFICATE OFINSURANCE FOR:

a) GENERAL LIABILITY in the amount of $300,000.00b) WORKERS COMPENSATION OR IF APPLICABLE

CERTIFICATE OF EXEMPTION FOR WORKERSCOMPENSATION INSURANCE.

c) OCCUPATIONAL LICENSE.d) APPLICABLE PERMITS IF ANY.

3. Workers are only permitted on the property Monday through Friday from8:30am to 4:00pm. No weekends or holidays.

The owner and contractor assume ALL liability and will be responsible for all damages and/or injury as well as any damage to any of the common- areas and p r o p e r t y o f other owners, which may result f r o m t h e performance of this work.

4. The owner and contractor will be responsible for the conduct of ALLpersons, agents, contractors and employees who are connected with thiswork.

5. Unit owners and/or Contractors working for unit owners foundcontravening Contractor’s and Owner’s Rules and Regulations will beremoved from the property and the $500 security deposit will beforfeited. A new $500 security deposit will be required for work toresume, regardless of what contractor resumes the work.

6. Contractors are responsible for the daily cleaning of the catwalk.

7. All damage to walls, doors, floors etc. are to be repaired immediately(owner will be billed if Paradise Towers, Inc. is forced to make repairs).

8. No supplies, tools or materials are to be stored, disposed of or cleaned onunit balconies or in common areas including laundry rooms.

All water shut down requests must be submitted with a two-day minimum notice to the management office. A $100-dollar fee is mandatory for all water shut downs.

9. Contractors are directly responsible for the removal of all constructiondebris and materials from the Association property. The building’s trashrooms, trash chutes and trash containers may NOT be used for thesematerials. Progress and type of work inside units will be inspected byManagement. Management of Paradise Towers, Inc. has the authority tostop any work or workmen in violation of these rules. Repeat violators willbe removed from the property and will no longer be permitted on thepremises.

10. Any damage to plumbing caused by cement, paint or grout being poureddown the drains or toilets will be the responsibility of the contractor andowner.

11. Management must approve all work and advise if any permits are requiredaccording to City of Hallandale Beach, Federal, State and other Localbuilding codes and regulations. The Broward County and City of HallandaleBeach will be notified of any violation and non-compliance.

12. Specifications (SPECS) must be submitted with original Contractor’s &Owners Rules and Regulations. Packages with missing or incompleteSPECS will not be accepted at the management office. SPECS andcompleted package must be submitted and approved prior to managementissuing approval letter for the City of Hallandale Beach.

13. No plastic toilet or sink supply lines are permitted to be installed in any unit.Only welded ALL METAL supply lines and connectors are permitted. AllAngle Stop Valves must be welded NO COMPRESSION or any other typeof valve will be permitted.

AIR CONDITIONING ONLY

The following are the mandatory requirements when replacing or installing a new air conditioner.

1. A building permit for A/C exchange must be pulled from the City of HallandaleBeach.

2. Water shut downs must be scheduled with the management office and a fee of$100 must be paid.

3. The condenser water supply along with the return ball valves must beremoved and replaced with new ones.

4. The high pressure condenser water hoses must be removed and replaced withnew ones.

5. The condensate drain and P-Trap must be checked.

Permit Requirements

Pursuant to Section 104.1.1 of Florida Building Code: Permits

A permit ensures the consumer that the work is being done properly and with the appropriate materials. A licensed contractor is required to obtain permits in the following situations:

General maintenance and repairs exceeding $500 in labor and materials for projects modifying a building's existing structure in any way (including tile, wood flooring and marble); Any electrical work, including installation of ceiling fans; Replacement of an air conditioner unit; Installation of a water heater; Changing a toilet or a sink fixture.

Please contact the Building Division at 954-457-1386 if you should have any questions as to whether a permit is required.

Please be advised that the Association will require a copy of any required permit before work is to begin in the unit.

A copy of a final inspection and closed permit must be submitted before the security deposit can be refunded.

CONTRACTOR’S & OWNERS ACKNOWLEDGEMENT

We agree for ourselves, employees and agents to abide by the Contractor’s and Owner’s Rules and Regulations at all times; and to cause our subcontractors, their employees and agents likewise to obey such rules and regulations. We agree as well to release the Association from any liability or damage caused as a result of the work performed.

I, as the owner, agree that if damage occurs to the common areas during this work process, the Association will bill me accordingly and/or withhold the $500.00 security deposit. In addition, I, as the owner, agree that if the $500.00 security deposit is withheld, then another $500.00 security deposit is required in order to resume work in my unit.

I will be responsible for complying with and will comply with all applicable federal, state and local laws, code, regulations, and requirements in connection with this work and I will obtain any necessary governmental permits and approvals for the work.

I understand that the Association through its staff has the right to inspect the work perform in my unit to ensure compliance with all applicable federal, state and local laws, code, regulations, and requirements in connection with this work.

I understand that if a violation of compliance with any applicable federal, state and local laws, code, regulations, and requirements in connection with this work is found, the Association through its staff has the right to stop all work until the above mentioned is complied with.

I understand and agree that the Paradise Towers, Inc. and its Board of Directors, agents and staff have no responsibility with respect to such compliance and that the Board of Directors and it’s designees approval of this request shall not be understood as the making of any representation or warranty that the plans, specifications or work comply with any law, code or regulation or governmental requirement.
