1 CONSULAR SERVICES PASSPORTS What documents should I submit if I wish to renew my passport? Filipino citizens are entitled to a Philippine passport. Filipino citizens who have acquired Canadian citizenship are eligible to apply for a Philippine passport after re-acquisition of their Philippine citizenship (oath of allegiance under RA 9225). A passport may be renewed 6 months prior to its expiration date. Personal Appearance & a confirmed appointment schedule are REQUIRED for APPLICATION OF A NEW PASSPORT (PLEASE COME IN PROPER BUSINESS ATTIRE WITH COLLAR AND SLEEVES). I. PASSPORT RENEWAL A. Latest passport with photocopy of the data page (page showing photo & complete name) B. Duly accomplished passport application form C. Original and photocopy of one of the following: Applicant’s valid Permanent Resident Card/Work Permit, Dual Citizenship Certificate (Order of Approval/Identification Certificate), Status Verification from CIC, Record of Landing (if within 3 years from date of arrival), or latest Tourist Visa to Canada, whichever is applicable D. If the latest passport currently held is a Green Passport or a Machine Readable Passport (MRP) or was issued on or before 01 May 1995, submit the following: PSA / NSO issued Birth Certificate (original & photocopy); At least one (1) valid identification document (ID); Other supporting documents (i.e. PSA/NSO marriage certificate if married and using married surname, etc.); and Fee: C$81.00 for new passport. Additional Requirements: For applicants below 18 years of age: Original NSO Birth Certificate, plus photocopy; (If accompanied by mother) Photocopy of mother’s passport; (If accompanied by father) Original marriage contract, plus photocopy; Photocopy of mother’s and father’s passports.

*For passport applicants who are permanent residents of Canada but cannot present a valid permanent resident (PR) card or whose PR cards have expired, they will have to present a "No Record Letter" issued by Citizenship and Immigration Canada (CIC). Kindly follow this link (http://www.cic.gc.ca/english/information/applications/guides/CIT0058ETOC.asp) to find out how to apply for this document. **For passport applicants who are in Canada on work permits but whose permits have expired, they will have to present proof that they have applied for a renewal of their work permits. This proof may be in the form of a receipt from Citizenship and Immigration Canada (CIC) showing the name of the applicant proving that a renewal of the work permit has been applied for. Change of Name (for female applicants) from Maiden (Single) to Married Requirements A, B, C, D plus E. If married in the Philippines - Original & photocopy of NSO Marriage Certificate and Birth Certificate; F. If married in Alberta and Saskatchewan - Original & photocopy of NSO Marriage Certificate and Birth Certificate. Fees:C$81.00 for new passport; C$33.75 for Report of marriage. G. If married outside of Alberta and Saskatchewan - Original & photocopy of NSO Marriage Certificate and Birth Certificate. Fee: C$81.00 for new passport. Note: For those who were married outside the Philippines, a Report of Marriage (ROM) must have been filed with the Philippine Embassy/Consulate with proper jurisdiction for the Philippine Statistics Authority (former NSO) to be able to issue an authenticated Marriage Certificate. Change of Name (for female applicants) from Married to Maiden (Single) Due to Death of Spouse Requirements A, B, C, D, E plus H. Death certificate from NSO of the deceased husband; or Court Order for presumptive death. Fee: C$81.00 for new passport Due to Annulment/Divorce: (those recognized under Philippine Laws)

Requirements A, B, C, D, E plus I. Duly authenticated copy of annulment/divorce decree/order and NSO-issued MC with the annotation on the annulment decree; *Divorce should be duly recognized by a Philippine Court. Fee: C$81.00 for new passport Married applicants who opt not to use husband's surname (allowed if husband's surname has not been used before). Requirements A, B, C, E, if latest passport is green ; A,B,C,D,E or F, if latest passport is brown, plus J. Affidavit that applicant has opted to use maiden surname. Fee: C$81.00 for new passport C$33.75 for Report of Marriage (if no report has yet been made). II. REPLACEMENT OF LOST PASSPORT (Please note that there is a 15-day verification prior to the processing of application for replacement of lost valid passport) Requirements B, C, D, plus K. Police Report (if lost passport is still valid) L. Affidavit of Loss (may be accomplished at Philippine Consulate General for separate Fee) M. Duly authenticated birth certificate N. Duly authenticated marriage certificate (for married women applicant) O. Photocopy of latest/lost passport, if available Additional Requirements: E or F, H, I, J (whichever is applicable)

Fees: C$202.50 for lost e-passport/C$121.50 for lost MRP or green/brown passport. III. PASSPORT APPLICATION FOR MINORS (under 18 years old) First Time Applicants Requirements B, C plus P. Birth certificate issued by the Vital Statistics of Canada Q. Marriage certificate of parents and proper ID (if minor is legitimate status); Personal appearance of Mother and mother’s ID (if minor is of illegitimate status) R. Passports or any government issued IDs of applicant’s parents and photocopies of such ID S. Letter of consent signed by either parent in the presence of the Consular Officer. If neither parent can appear in person, the signature/signatures must be acknowledged by a notary public T. Report of Birth Fees: C$81.00 for new passport; plus C$33.75 for report of birth Renewal Requirements A, B, C, Q, R Fee: C$81.00 for new passport PASSPORT RELEASE Applicant MUST provide a self-addressed prepaid return envelope per applicant from CanadaPost (secure a signature on delivery sticker if possible) or a private courier service of applicant’s choice. The Consulate however will NOT assume responsibility for passports/documents that may be lost in the mail. What documents should I submit if I wish to renew my green or MRP (passport number starting with “XX”) booklet? Effective 01 July 2016, all application for passport renewal of holders of green or MRP (Machine Readable Passport; Passport Number starting with “XX”) shall be required to submit the original and photocopy of the following documents, in addition to the usual requirements for passport renewal: 1. Actual MRP (passport number starting with "XX") / green booklet; 2. PSA-Authenticated Birth Certificate (PSA-formerly NSO); 3. At least one (1) valid identification document, and 4. Other supporting documents as may be required by the processor. (i.e. PSA-MC if married and using married surname, etc.) MODE OF PAYMENT Calgary applicants may pay in exact amount in cash or by money order/bank draft payable to the Philippine Consulate General Calgary. What documents should I submit as a first time adult passport applicant with delayed registered birth? 14 July 2016 – The Department of Foreign Affairs has announced that first time adult passport applicants with delayed registered birth are required to submit the following additional supporting documents, in original format: a. Sequential or “growing-up” photos; b. Additional identification cards, e.g. old drivers license, old employment id, student id, old social club membership, etc.; c. Documentary evidence to show use of full name, e.g. payslips, bank statements, college registration, etc.; d. Evidence of registry of the applicant’s address in the passport application form, e.g. utility bills, official correspondence from public or private sector, etc. Applicants may also be required to be interviewed by a Consular Officer and provide further information to establish identity.

What should I do if I wish to change from my married name to my maiden name in my passport? Female applicants that wish to change their names from married to maiden in their new passports must submit the following requirements with their passport application: Latest passport with photocopy of the data page (page showing photo & complete name) Duly accomplished passport application form Original and photocopy of one of the following: Applicant’s valid Permanent Resident Card/Work Permit, Dual Citizenship Certificate (Order of Approval/Identification Certificate), or latest Tourist Visa to Canada, whichever is applicable If the latest passport currently held is either a Machine Readable Passport (MRP) or was issued on or before 01 May 1995, submit an NSO Birth Certificate for proofs of place of birth & complete middle name (original & photocopy) If change of name is due to death or presumptive death of spouse Death certificate from NSO of the deceased husband; or Court Order for presumptive death. What should I do if I wish to change from my maiden name to my married name in my passport? Female applicants who wish to change their surnames from maiden to married in their new passports must submit the following requirements with their passport application: Latest passport with photocopy of the data page (page showing photo & complete name) Duly accomplished passport application form Original and photocopy of one of the following: Applicant’s valid Permanent Resident Card/Work Permit, Dual Citizenship Certificate (Order of Approval/Identification Certificate), or latest Tourist Visa to Canada, whichever is applicable If the latest passport currently held is either a Machine Readable Passport (MRP) or one issued on or before 01 May 1995, an NSO Birth Certificate for proofs of place of birth and complete middle name (original and photocopy) Original and photocopy of Birth Certificate Original and photocopy of NSO Marriage Certificate (For those married in the Philippines and abroad) Payment of C$81.00 for new passport NOTE: For those who were married in Canada or abroad, a Marriage Certificate may only be secured from the NSO if the marriage was reported to the Philippine Consulate/Embassy with proper jurisdiction (Report of Marriage). For this purpose, please refer to ROM form and requirements. Can I choose NOT to use my husband’s surname in my passport? A female applicant may choose NOT to use her husband's surname in her new passport provided that she has not yet used her husband's surname in her passport before. If the applicant wishes to proceed, the following requirements must be submitted in applying for a new passport: Latest passport with photocopy of the data page (page showing photo & complete name) Duly accomplished passport application form Original and photocopy of one of the following: Applicant’s valid Permanent Resident Card/Work Permit, Dual Citizenship Certificate, (Order of Approval/Identification Certificate) or latest Tourist Visa to Canada, whichever is applicable If the latest passport currently held is either a Machine Readable Passport (MRP) or was issued on or before 01 May 1995, submit an NSO Birth Certificate for proofs of place of birth & complete middle name (original & photocopy). Plus: Affidavit that applicant has opted to use maiden surname. Fee: C$81.00 for new passport. What documents does a dual citizen submit in order to apply for a new passport? Applicants who have recently become dual citizens (by virtue of Republic Act 9225) and are applying for a new Philippine passport should submit the requirements listed below when applying for a Philippine passport:

Latest Philippine passport with photocopy of the data page (page showing photo & complete name)

Duly accomplished passport application form

Original and photocopy of one of Dual Citizenship Certificate - Order of Approval/Identification Certificate

If the latest passport currently held is either a Machine Readable Passport (MRP) or was issued on or before 01 May 1995, submit an NSO Birth Certificate for proofs of place of birth & complete middle name (original & photocopy).

Marriage Certificate/Divorce Decree or Annulment Order/Death Certificate of Spouse with photocopy, whichever is applicable

What documents should a minor applicant submit to obtain a passport? A. Minors (under 18 years) applying for their FIRST passport should submit the following requirements with their passport application: Duly accomplished passport application form Original and photocopy of Dual Citizenship Certificate (Order of Approval/Identification Certificate) or Report of Birth, whichever is applicable Birth certificate issued by the Vital Statistics of Canada Marriage certificate of parents and proper ID (if minor is legitimate status); Personal appearance of Mother and mother’s ID (if minor is of illegitimate status) Passports or any government issued IDs of applicant’s parents and photocopies of such ID Letter of consent signed by either parent in the presence of the Consular Officer. What documents should I submit if I wish to apply for a replacement of a lost passport? Please take note that there will be a 15-day verification prior to the processing of application for replacement of lost valid passport. Applicants in an Outreach Program MUST pay by CASH, MONEY ORDER / BANK DRAFT, payable to the Philippine Consulate General Calgary. Can I apply for a passport by mail? Passport application by mail is NOT allowed anymore. Personal appearance at the Philippine Consulate before a Consular Officer is required for digital capture of passport applicant's photo, fingerprint & signature. MODE OF PAYMENT Calgary applicants may pay in exact amount in cash or by money order/bank draft payable to the Philippine Consulate General. Applicants in an Outreach Program MUST pay by CASH, MONEY ORDER / BANK DRAFT, payable to the Philippine Consulate General Calgary. How do I pay for my passport fees? The Consulate General only accepts payments in cash, postal/bank money order or bank certified checks made payable to the Philippine Consulate General Calgary. Personal cheque, credit or debit card payment is not acceptable. When will I receive my new passport? To avoid any inconvenience, the Philippine Consulate General in Calgary reminds all Philippine passport holders to RENEW their Philippine passport BEFORE the validity of their passport falls below six (6) months. Under the e-passport system, all passport applications are sent to Manila for final processing and printing. It may take 8 to 12 weeks from the date of application for new passports (regular passport renewal/first time applications) to arrive in Canada. Lost passport applications require a 15-day verification period in addition to the 10 to 12 weeks estimated waiting time.

Due to the long waiting period which is beyond the control of the Consulate General, Filipinos in Canada are strongly urged to renew their passports long before they need to renew their work permits and visas or adjust their immigration/citizenship status. Passport applicants are also advised not to buy plane tickets until they have received their new passports. Each applicant must provide a self-addressed prepaid "Expresspost" return envelope from Canada Post (secure a signature on delivery sticker if possible) or hire a private courier service of the applicant's choice. Please note that the Consulate will not be responsible for passports/documents that may be lost or damaged in the mail. Passport follow-ups will be entertained only after 10 weeks has lapsed from the date of application.
