1. The following Ventilation system Management documentation has not been presented:Workplace Air Monitoring/SamplingVentilation System User ManualCommissioning/Initial Appraisal ReportThe above should be provided to evidence compliance with the Regulations.

2. The client has made attempts to source the Commissioning Report and User Manual but has been unable to do so.

3. Air Cleaner - No record of filter efficiency and concentration of the contaminant in the return air testing has been seenwhere the LEV system includes arrangements for re-circulating into workplace areas. Relevant testing should be conductedand records made available. The client has made a written statement to Allianz indicating that the process is only used 1 to 2hours a week.

4. Air Mover - Technical Specification of the Air Mover and associated Motor Manufacturers details have not been fullyverified, these should be provided to assist in completing section 3 of this report.

5. Capture Hood - Fixed Capture x 3

6. Capture Hood - No airflow indicator installed at the Capture Hoods. Further guidance for the use of airflow indicators canbe found in HS(G) 258, Controlling Airborne Contaminants at Work. Consideration should be given to the provision of asuitable airflow indicator, which would indicate to the operator that the system is working effectively.

Comments 1. Qualitative testing using method as detailed in HS(G)258, including observations of the process andcontaminant sources, effective containment of the hazardous substance, and the way in which theoperators use (or simulation of use) of the LEV system found the visual appraisal of the level of control tobe satisfactory.

Quantitative Testing Results PASS

Comments 1. Quantitative testing carried out on the LEV System was found to satisfy the guidance found in HS(G)258,Table 9 and Table 12 for this type of Hazardous Substance.

2. The comparison of the ventilation system layout/process from the last thorough examination & testwas observed to be unchanged with system performance found to be at approximately the same levelsas recorded at the previous or benchmark examination and test with specific capture/transport velocityfound to be approximately the same.

Air Cleaner - No record of filter efficiency and concentration of the contaminant in the return air testing has been seenwhere the LEV system includes arrangements for re-circulating into workplace areas. Relevant testing should be conductedand records made available. The client has made a written statement to Allianz indicating that the process is only used 1 to 2hours a week.

4. Air Mover - The fan and motor technical details could not be fully captured and should be provided.

5. Engineering Control - Contaminant dusts are present on the new knife serrator. It should be ensured that the extractionsystem is used during all grinding operations.

Carrs HS Plant Inspection

Comments. Have you �xed any issues? if so action taken. Is further action needed, if so whatHSG-258 p64 notes re-circulated usually ok for contaminants that are; low toxicity, that are particles. This LEV is used approx. 1-2 hours per week and even then not all at the same time. With this in mind trying to measure the TWA of the exhaust fume would result in no �ndings. We will start and inspect the �lters on a regular basis, taking photo’s before and after cleaning (as required). Results from this will indicate of further action is required.
