CHAPTER 1GENERAL

1-1. Purpose. This regulation prescribes pro-cedures governing procurement, issuance, record-ing, and changing several items of personalinformation normally used as personnel identifiers.1-2. Applicability. This regulation applies to theActive Army, Army National Guard, Army Re-serve, and to individuals entering into such mili-tary status.1-3. Explanation of terms. As used in this regu-lation, the following explanations apply:

a. Baltimore SSA Office. Headquarters of SocialSecurity Administration, Baltimore, MD 21235.

b. Local SSA Office. SSA district or branchoffice servicing the area or installation, or otherplace of duty assignment for personnel stationedin the United States, Puerto Rico, the VirginIslands, and other oversea areas.

c. Military status. The condition of an officer,warrant officer, or enlisted person who has beenenlisted, inducted, or appointed in the Regular

Army, Army of the United States without com-ponent, Army National Guard, or Army Reserve.1-4. Policy, a. The Social Security Number(SSN) or Temporary Identification Number(TIN), as appropriate, has been used as the per-sonal identifier number of US Army personnel inthe.active, retired, and Reserve components since1 July 1969.

ft. No prefix or suffix to the SSN or TIN willbe used.

c. Immediately following entrance into militaryservice, the individual will inform relatives andfriends of his or her SSN or TIN and will advisethem to use this number in personal correspond-ence. This requirement also applies when the in-dividual's TIN is replaced by his or her SSN.Official requests for information concerning per-sons in the military service, as well as the formswhich, in certain instances have to be completedby relatives or friends, will bear the SSN or TIN.

CHAPTER 2NAME AND BIRTH DATA

Section I. GENERAL2-1. Scope. This chapter prescribes proceduresfor recording and changing record of name and/Or birth data.2-2. How to record names, a. Complete lastname, full first name, and full middle name, ifany, normally will be recorded in that order in allmilitary records. When "Jr.," "II," etc., is used,it will appear after the middle name. In the ab-sence of a middle name, it will be entered immedi-ately following the first name. The abbreviationsNMI (no middle initial) or IO (initial only) will.not be used.

Examples:Swift, Benjamin FrankMartin, Elizabeth MaryJones, Edward Karl, Jr.Does, William, Jr.Dew, John William, II

&. This style will not apply to existing recordsthat would be impracticable to change—for ex-ample, correspondence or signatures.

c. The name which appears in item 1, DA Form66 (Officer's Qualification Record) and item 1,DA Form 20 (Enlisted Qualification Record)will be presumed to be the correct official name.

Section. II. CHANGE OF RECORD OF NAME AND/OR BIRTH DATA2-3, Authority, a. The individual must requesta change of record, when required. The authorityto approve change of record is delegated to theimmediate commander or his designec.

&. Change of record will be authorized onlywith the evidence required for each type set forthherein.: . .. !

2-4!', When made. Action to change a record willbe initiated only when there is a reasonable assur-ance that it can be completed during the indi-vidual's stay at an installation. Action normallywill not be undertaken during assignments of briefduration, since delay is frequently encountered inobtaining documentation.2-5. How to change record of name. The indi-vidual must submit a signed request to his com-manding officer, containing current name, newname,;and reason (s) for the desired change. Hewill attach the evidence as outlined below.

*• Addition, deletion^ or correction. The addi-*?on+ or .'deletion of "Junior," "Senior," "III," or

name, and corrections involving transposedr slight change in spelling may be accom-'p'n. the individual's signed application.

' J^*™^<^M>W- The law forbids reproduction9i certificates of naturalization, and the individualmus^' retain the original. Therefore, his com-mander or designee will indorse the application

and state that he has seen the original certificateand that the name thereon agrees with that shownin the application. Certificate number will be in-cluded in the indorsement.

e. Marriage or divorce. Application will be ac-companied by an authenticated copy of certificateof marriage or divorce decree (final decree in stateswhere required).

d. Court order. Application, must be accom-panied by an authenticated copy of the court orderor decree,

e. Oilier thorn, court order. The following evi-dence is required: .

(1) An authenticated copy of public recordof birth filed at or near time of birth.

(2) If no public record as described in (1)above exists, a statement to that effect by theproper public official accompanied by the followingevidence:

(a) Authenticated copy of baptismal cer-tificate, or

(b) Affidavits from both parents, if livingor

(c) Affidavits from two or more persons(preferably disinterested) who will fully identifythemselves, state their ages, and give name, date,and place of birth of claimant together with thesource(s) of their information.

2-6. How to change record of birth data. Eachindividual must submit a signed request to hiscommanding- officer, stating his correct birth date/place, and reason(s) for the erroneous.data shownon Army records. The following evidence must beattached:

a. A birth certificate or authenticated copythereof which agrees in all respects with the datacontained in the request. If the name on the birthcertificate does not agree with that shown in therequest for a change, the reason for the discrep-

. ancy will be included in the request.6. If no public record of birth exists, a statement

to that effect by the proper public official will berequired, accompanied by as much of the follow-ing evidence as is reasonably available:

(1) Authenticated copy of baptismal cer-tificate.

(2) Affidavit from physician or midwife inattendance at birth.

(3) Affidavits from both'parents, if living.(4) A statement by the individual's com-

manding officer that he has seen the passport whichauthorized entry into the United States, and thatthe birth date/place on the passport is the same asshown in the request for change.

(5) Affidavits from two or more persons(preferably disinterested) who will fully identifythemselves, state their ages, and give name; date,and place of birth of claimant together with the'source(s) of their information. The affiants shouldalso state the full name of the father and maidenname of the mother of the claimant and the ad-dress (or last known address) of the parent orparents of the claimant if living.

(6) Authenticated copy of school record,from the first school attended, showing date ofbirth or age on admittance.

(7) Authenticated extract from the familyBible or other family record.

c. If evidence in & above is unobtainable OP isconsidered insufficient to change the birth data,Bureau of Census Form 10-611 will be prepared,requested report of first census taken after birth.Blank forms may be obtained upon request fromHQDA (DAPC-PAR-S), 200 Stovall Street,Alexandria, VA 22332, or the Bureau of Census.This report will be submitted as additionalevidence.

; 2-7. How to amend records, a. Records to bechanged. Only records pertaining to the current^period of service will be amended. For officers andwarrant officers, this record is the DA Form 66 andDA Form 41 (Record of Emergency Data) ; forenlisted personnel, the DD Form (EnlistmentContract) 4.. DD Form 47 (.Record of Induction),DA Form 20, and DA Form 41.

b. Procedure. The individual should draw asingle line through the entry and enter the newdata above, preferably by typewriter or in blue-black ink. Old entry will not be obliterated,2-8. CHAPTER 3SOCIAL SECURITY NUMBER

Section I. GENERAL3-L SSN Central Registry System. Com-manders at all echelons will insure that the cor-rect number is recorded on personnel records andthat only one SSN" is assigned to an individual.To assist commanders in fulfilling this responsi-bility, DejmrtmentjjJ the ArmTjoporatcs an SSNCentral Registry System. This system enables dieArmy to verify numbers with the SSA. Com-manders will be advised of the results of theseverification procedures and of cori-ectivc actionto resolve unverified cases.3-2. Safeguarding SSN card. The SSN cardissued an individual is evidence of his account withthe SSA. It will not under any circumstances beretained with military records. Each person willbe cautioned to safeguard his SSN card to preventloss or mutilation.

3-3. Forms, a. The following forms, required inthe administration of the SSN program, will berequisitioned through normal publications supplychannels:

(1) Form SS-5 (Treasury Department, In-ternal Revenue Service (Application for SocialSecurity Number)).

(2) Form OAAN-7003 (Department ofHealth, Education, and Welfare, Social SecurityAdministration (Request for change in SocialSecurity Records)).

1. FormOA-702 (Department of Health, Edu-cation, and Welfare, Social Security Administra-tion (SSN Card)) is issued to individuals only bythe SSA. This form will not be requisitioned.

Section II. PROCUREMENT OF NEW OR DUPLICATE SSN3-4. Prior to entering a military status, a. Apei-son contemplating entering military. servicewho does not have an SSN card should obtain, oneprior to entry.

b. Each individual planning to enter militaryservice will.be advised—

(1) To have card in his possession when re-porting for entry; or

(2) If his card has been lost or mutilated, toapply through his local SSA office for a duplicate,if time permits; or

(3) If an SSN card has never been issued, toapply for one through his local SSA office, iftime permits;

(4) That the normal processing time to obtaina new or duplicate SSN card is 3 weeks;

(5) That in the event he cannot locate hisSSN card when entering on active service, heshould have available other documentary evidence(old tax return, payroll slip, wage statement, Statemotor vehicle driver's license, etc.) of his assignedSSN.

c. Although every effort will be made to obtainan SSN card prior to entry into the Active Army,

Army National Guard, or Army Reserve, the lackof a card, will not under any .circumstancesdelay induction, enlistment, rcenlistmcnt, orappointment,

d. Potential personnel accessions (enlistees/inductees) will be screened for possession ofan SSN card at the time of pre-inductionprocessing and/or application for enlistment.An individual who has not been assigned or whohas lost a previously issued SSN card willbe assisted in applying for an SSN or replacementcard. When the 3-week normal processing timeto obtain a new card is available prior toentry on active duty, Form SS-5 will beprepared and forwarded in accordance withinstructions on the reverse. The special "NotifyEmployer" procedures described in paragraph 3-6will not be used in this case. The original of theform will be forwarded to the local SSA office forprocessing. When an individual does not have anSSN card, but possesses evidence of his assignedSSN, application for a duplicate will be forwardedto the local SSA office. The individual having documentedevidence .of an assigned SSN will enter in item11 (mailing address) of the Form SS-5 the ad-dress of a parent, relative, or guardian who willbe able to forward the SSN card to the individualshould he enter military service prior to receipt.3-5. Upon entry into military service, a. AnSSN will be obtained as soon as possible for theindividual entering active service who has neverbeen issued an SSN card, or whose card has be-come lost or mutilated and who does not have evi-dence of an assigned (SSN. This individual willbe required to complete an application (Form SS-5). Form OAAN-7003 will be used when applica-tion, is for a duplicate SSN card and a correctionof name is also involved (para 3-8).

b. For officers or warrant officers entering activeduty, the application will be prepared at the ini-tial duty station (temporary or permanent). Pro-cedures prescribed in paragraph 3-6 will govern.The original copy of the Form SS-5 will be for-warded to the Commander, USAPDSC, ATTN:Central Registry Unit, Edgewood Arsenal, MD21010, for processing. Item 34 of the individual'squalification record (DA'Form 66) will be anno-tated in pencil with the words "SS-5 processed."3-6. AFEES and training activity procedures.a. Action to be taken by AFEES. AFEES com-manders will take the following action in process-ing Army personnel for immediate enlistment orinduction who have lost SSN cards and who haveno evidence of their SSN, or who have not beenissued an SSN:

(1) Prepare Form SS-5 in duplicate. Entrieswill be in accordance with instructions on the re-verse of the form and the,following:

(a) Print or stamp boldly in the top leftmargin on both copies of the form the words "USArmy." (Use rubber stamps.)

(b) Item 1. Enter name exactly as shown onpersonnel records.

(c) Item 10. If an application has beenprocessed previously, so indicate,

,(d) Item 11. Do not enter the individual'smailing address. Instead, enter on both copies ofthe form the following data: beneath the word"city," print the words "MAIL TO D. Or," andbeneath the word "ZIP code" print the number"21204." These markings will insure proper SSAhandling.

. (e) On the reverse of the form in the upperspace provided for SSA district office use, enterthe TIN assigned the individual.

(/) On the reverse of the form in themiddle space show on both copies: "Commander,US Army Personnel Data Support Center,ATTN:. Central Registry Unit, EdgewoodArsenal, MD 21010." (Use rubber stamp.)

(2) Distribute Form SS-5 as follows. Attachthe original form to the lower left corner of theappropriate copy of the member's DD .Form 4 or47 (AFEES only) and transmit to the Com-mander, USAPDSC., USAPDSC will process theapplication with a designated SSA district office.

. The duplicate copy of the Form SS-5, marked"DUPLICATE," is forwarded to the custodianof the Personal Financial Record (PFR).

(3) Make, a pencil notation'showing "SS-5processed" in item 1 or 2, as appropriate, of theMPRJ file copy of the DD Form 4 or 47.

b. Verification of records. Training activity per-sonnel will verify that the SSN has been recordedon the individual's records. When a TIN has beenassigned by an AFEES, commanders of trainingactivities will verify that a Form SS-5 has beenprocessed.by checking for a; notation on;the DDForm 4 or;47 and the existence of ,a duplicateForm SS-5 in the PFR, If this check reveals thatan application has not been initiated, the com-mander will have Form SS-5 prepared and theoriginal copy forwarded to the Commander,USAPDSC, ATTN: Central Registry Unit,Edgewood Arsenal, MD 21010, for processing.

c. Liaison with managers of SSA district orbranch offices. Recruiting Station, AFEES, andtraining activity commanders will establish liai-son with managers of district or branch SSA offi-ces to develop mutually agreeable procedures forprocessing Forms SS-5. Managers of SSA districtoffices listed in table 3-1 'have been instructed tocooperate with commanders. Recruiting Station,AFEES, and training activity commanders shouldbo aware of and direct members and potentialmembers to use SSA procedures for electronicverification of alleged numbers. The proceduresused by SSA district or branch offices offer fast(24-hour), efficient screening service for individ-uals who have lost or mutilated their SSN cardand know their SSN. If it is verified, the localoffice will issue a duplicate card.

since submission of the application and notifica-tion of the assigned SSN has not been received,either through • the supporting personnel dataprocessing activity or by the individual's receiptof the SSN card, the training activity will con-tact the Commander, USAPDSC or the local SSAoffice, as appropriate, to determine the cause ofdelay and anticipated delivery date of the card.3-7. Procurement of duplicate SSN cards on acontinuing basis, a. When a member's SSN cardhas become lost or mutilated subsequent to hisentry into a military service, he should apply fora duplicate (Form SS-5). When submittedthrough the member's organization, the person-nel officer will verify the SSN entered in item 10,

Form SS-5, from personnel and finance records(DA Forms 20, 66, 41, 1996, and 3716) and thestandard personnel plate. On the original card, thenumber is printed in-bold red figures, whereas on aduplicate card it is typed.

. b. Duplicate SSN cards may be obtained bymembers on an individual basis, except as indi-cated in paragraph 3-6, from the local SSA officeprovided the name is reported exactly as shown onmilitary personnel records. A member who has losthis card but still has the stub may obtain a dupli-cate card without delay by presenting the stub toany local SSA office. The special "Notify Em-ployer" procedures described in paragraph 3-6will not be used in these cases.

Section III. CHANGES IN PERSONAL DATA IN SOCIAL SECURITY RECORD

3-8. Reporting corrected data. When personaldata reported in a previous application '(FormSS-5) changes or is found to be incorrect, FormOAAN-7003 (Request for Change in Social Secur-ity Records) will be used to correct this data. Aduplicate SSN card bearing the same number asthe old card is issued when an individual's namehas been officially changed. Duplicate cards are notissued for other corrections, nor is receipt of thecorrected data acknowledged.

a. Submission of Form OAAN-7003 ismandatory—

(1) Upon entry into a military service whenthe name shown on the SSN card does not sub-stantially agree with the name as recorded in mili-tary personnel records; e.g., a Social Security cardhas been issued bearing the name "Bill Alden" butthe personnel records of the individual show hisname as "William John Alden." It is not necessaryto submit Form OAAN-7003 when only the middleinitial is shown on the SSN card and the personnelrecords show a full middle name or two middlenames or initials, or to add or delete a suffix suchas Jr., or Sr.

(2) When the name of the member is changedbecause of marriage, divorce, or other reasons,Form OAAN-7003 will be prepared and for-warded simultaneously with the official change ofname in personnel records, but not before suchchange is officially authorized and effected. .

b. Form OAAN-7003 may be used to notifySSA of corrections in information other than

name, such as date of birth, or correction ofmother's or father's name. Submission = of FormOAAN-7003 is an individual responsibility; how-ever, personnel will be encouraged to submit thesecorrections in a timely manner. Erroneous infor-mation in the social security record can delay pay-ment of benefits.

c. The submission of Form OAAN-7003 is notrequired for—

(1) Changes in employment or militarystatus;

(2) Changes in civilian or military mailingaddresses; or

(3) Translation into English of social secu-rity cards issued to Puerto Ricans in Spanishlanguage, if otherwise correct.3-9. How to prepare Form OAAN-7003. a. Ifthe form is prepared at time of entry into militaryservice for the purpose of acquiring a duplicateSSN card or changing SSA records, the proce-dures in paragraph 3-6 for submission of FormSS-5 will be followed.

b. If Form OAAN-7003 is prepared at otherthan the time of entry into military service, it willbo prepared in single copy, typed or printed inblue-black ink. Entries will be in accordance withinstructions on the reverse of the form with modifi-cation as follows:

(1) Item 1. Enter name exactly as shown onpersonnel records.

(2) Item 11. Enter military mailing addressof the organization having custody of the member's

personnel records. If a change in name is involved,the duplicate SSN card will be returned addressedto the commander of the organization.

(3) Item 14> The member will sign his namein ink as normally used for official purposes.

c. For a correction in name, attach the lastissued SSN card or stub, if available.

d. If the form1 is prepared at other than thetime of entry into military service, forward it tothe local SSA district office'.

Section IV. RECORDING AND REPORTING SSN3-10. Recording in military personnel records.The SSN will be recorded as soon as possible afterfirst point of contact. Memory will not be acceptedas a basis for recording. When the individualenters on active duty without an SSN card, butpossesses documented evidence of his assignedSSN, this evidence will be accepted as a basis forrecording. The SSN will be, recorded on the DAForm 66 or DA Form 20 from the actual SSNcard or stub, except that the documented evidencecited above may be used only at time of initialentry on military status. The SSN which is re-corded on the above records will be considered thecorrect SSN if discrepancies occur in various mili-tary records. It may subsequently be transcribedfrom one record to another without. verificationfrom the card or stub. If the SSN card is not avail-able, action will be taken in accordance with para-graph. 3-7.3-11. Reporting of SSN. When an individual has

entered on active duty without an SSN, personnelofficers will advise agencies listed in paragraph2-8, by letter, of the change from TIN to SSN.The letter will include the TIN and SSN to insurecorrect identification of the individual.3-12. Delivery of SSN cards to individuals.Upon receipt of a new or duplicate card, the com-mander will forward the card with stub to the cus-todian of the individual's records for recording onappropriate documents. When recorded, the SSNcard -will be given to the individual forsafekeeping.3-13. Duplicate SSN. If it is discovered that anindividual has two SSN or two individuals havebeen assigned the same SSN, they must contactthe local SSA office for assistance in. initiatingcorrective' action. If the corrective action resultsin a need for a change to personnel records, actionssimilar to those specified in paragraph 3̂ 11 willbe taken. ,

CHAPTER 4TEMPORARY IDENTIFICATION NUMBER

4-1. Issuing authority. The commanders of or-ganizations listed in table 4-1 are responsible forcontrol and issuance of TIN. Commanders notlisted in table 4r-i who are.required to process anindividual for entrance into a military service willcontact the nearest listed agency for assignmentof a TIN.4-2. When issued, a. The TIN will be issued toindividuals who do not have an SSN during thepre-induction, pre-enlistment, or pre-appointmcntprocessing for entrance into the Active Army.

b. The TIN for members of the Army NationalGuard will be issued by the State adjutants gen-eral in accordance with separate instructions is-sued by the Chief, National Guard Bureau.

c. US Army TIN are to be used only once. Thesame TIN will never be assigned to differentindividuals.4-3. Composition of TIN. The TIN for theActive Army and the US Army Reserve is a 9-digit number constructed as follows:
