Assessment of Disease-Specific Functional Class in Pulmonary Arterial Hypertension
I Patients with pulmonary hypertension in whom there is no limitation of usual physical activity; ordinary physical activity does not cause increased dyspnea, fatigue, chest pain, or presyncope
II Patients with pulmonary hypertension who have mild limitation of physical activity; there is no discomfort at rest, but normal physical activity causes increased dyspnea, fatigue, chest pain, or presyncope
III Patients with pulmonary hypertension who have a marked limitation of physical activity: there is no discomfort at rest, but less than ordinary activity causes increased dyspnea, fatigue, chest pain, or presyncope
IV Patients with pulmonary hypertension who are unable to perform any physical activity at rest and who may have signs of right ventricular failure: dyspnea or fatigue or both may be present at rest and symptoms are increased by almost any physical activity
Definizione di “cor pulmonale”
Alterazione della struttura o della funzione del ventricolo destro dovuta ad ipertensione polmonare causata da malattie che colpiscono il polmone o la sua vascolarizzazione.
L’interessamento del cuore destro conseguente a malattie che interessino primitivamente quello sinistro o conseguente a cardiopatie congenite non rientra nella definizione di “cor pulmonale”
Cuore polmonare acuto e cronico
La maggior parte delle condizioni che causano il cuore polmonare sono croniche e lentamente progressive
Cuore polmonare cronico
Talvolta i pazienti possono presentare una sintomatologia acuta e pericolosa per la vita. Tale scompenso acuto si realizza quando il ventricolo destro non è in grado di compensare un aumento improvviso della domanda, risultante o dalla progressione della malattia di base o dalla superimposizione di un processo acuto

La causa più frequente di cor pulomonale è la BPCO
Nei pazienti affetti da BPCO il coinvolgimento del cuore destro può essere correlato con la maggiore gravità della disfunzione respiratoria
L’ipertrofia ventricolare destra è presente nel 40% dei pazienti con un FEV1 <1,0 L e nel 70% di quelli con un FEV1 <0,6 L
Ma anche la presenza di ipossiemia, ipercapnia e policitemia sono variabili indipendenti che predicono lo sviluppo di ipertrofia ventricolare destra nella BPCO, anche se la correlazione non è così forte come per le altre alterazioni funzionali

Questi cambiamenti sono per molti versi simili a quelli visibili in altre forme di ipertensione polmonare

Lo sviluppo di ipertensione polmonare è un fattore prognostico negativo nei pazienti affetti da BPCO, sia dal punto di vista della QoL che della mortalità
Sebbene tale problema sia stato tradizionalmente considerato la conseguenza della distruzione del letto vascolare e dell’ipossia, studi recenti indicano che tali fattori non correlano molto bene con la pressione nell’arteria polmonare
Nuovi dati iniziano a mostrare che l’ipertensione polmonare in tale situazione è probabilmente la conseguenza del danno diretto del fumo di tabacco sui vasi polmonari, con anormale produzione di mediatori che controllano la vasocostrizione, la vasodilatazione e la proliferazione delle cellule vascolari, portando ad un rimodellamento vascolare con conseguenti alterazioni fisiologiche

Pulmonary hypertension in idiopathic pulmonary fibrosis
The prevalence of PH complicating the course of patients with IPF has been reported as occurring in 32 to 85% of patients
Data from the UNOS showed that 45% of patients with IPF listed for transplant have right-heart catheterization evidence of PH, with 9% of patients having a mean pulmonary arterial pressure (mPAP) of greater than 40 mm Hg

Serial progression of PH in IPF
The concept of serial progression has been underscored in a retrospective review of 39 patients with IPF in whom serial right-heart catheterization measures of PAP were available.
The first measure showed a prevalence of PH of 33%, whereas the second measurement in the same group performed immediately before transplant demonstrated a prevalence of 85%.

Lung volumes, exercise capacity and association with PH and IPF
Intuitively, the severity of fibrosis and degree of restrictive physiology should correlate with the prevalence and degree of PH
However, it appears that PH may not correlate with lung volumes in patients with IPF
This finding is intriguing and supports the notion that factors aside from progressive fibrosis are responsible for PH in IPF
As opposed to lung volumes, exercise capacity does appear to have a significant association with PH in IPF.

Pathophysiology of obstructive sleep
apnoea/hypopnoea syndrome The primary defect in OSAHS is an anatomically small or
collapsible pharyngeal airway.
During wakefulness, neuromuscular compensatory systems function to increase the activity of the pharyngeal dilator muscles, thus preserving airway patency.
However, this reflex driven augmented muscle activity is lost at sleep onset, and collapse of the pharyngeal airway occurs.
The associated hypoxaemia and hypercapnia drive increasing respiratory effort, and ultimately arousal from sleep occurs, thus re-establishing airway patency and ventilation. Once the patient returns to sleep the cycle begins again.
The patient thus suffers the consequences of repeated sleep disruption as well as recurrent hypoxaemia and hypercapnia.
