POOLCORP is excited to welcome you to the Diving Into Wellness Program for 2016. We care about your health and well-being. Our fun and educational program will enable you to make permanent lifestyle changes that help you achieve your full health potential. The goal of this program is to help you reach your optimal health by creating a supportive culture that provides activities to help maintain good health and to assist those at risk to better manage their conditions.

Download the HealthyNow app to log activities and check points, along with other helpful uses, on your iPhone or Android device. 1. Go to www.DivingIntoWellness.com.2. Log into your online Wellness Center.3. Click the MOBILE COMPANION button.4. Register your phone by following the instructions on your screen.5. Depending on your mobile device, go to either the iTunes App Store or Google Play to download the HEALTHYNOW app. 6. Enter while registering your phone in the Wellness Center.

INCENTIVES FOR 2016Receive a substantial discount on your 2017 POOLCORP Medical Plan contributions (the discount for 2016 ranged from $728 to $1,456 depending on your plan and coverage level).

Drawing for a $100 gift card for those participants who earn at least 50 points before May 31st.

$50 gift card for those employees who complete all the requirements of this year’s program and (1) participated in the program last year, but did not qualify for the Medical Plan Contribution Discount, or (2) were hired prior toAugust 1, 2015 and did not participate in the program last year. If your spouse is covered under a POOLCORP Medical Plan, they too must complete all the requirements of this year’s program for you to be eligible for the $50 gift card. The $50 gift card will be processed in the �rst paycheck in 2017.

By participating in the program’s Wellness Screening and earning the program goal of 200 points, you will meet the program requirements for 2016 and will be well on your way to achieving greater health. You will have access to a variety of wellness tools and resources to reach the program goal of 200 points. The good news is, the Health Assessment is not a requirement for 2016, but it is available and you can earn points for completing it. All full-time and part-time employees working at least 30 hours per week are invited to participate, as are spouses that are covered underany of POOLCORP’s medical plans. If your spouse is currently enrolled in one of POOLCORP’s medical plans, you both must complete all requirements of the program to earn the incentives below.

The Wellness Center is your online portal full of helpful resources and tools, including nutritional and exercise trackers,healthy recipes, educational webinars, and much more. Additionally, this is a secure repository for all of your health information. Access your Wellness Center by going to www.DivingIntoWellness.com. Full instructions are available online.

REQUIRED WELLNESS ACTIVITIES

Wellness Screening

REQUIREDYour wellness screening measures total cholesterol (LDL, HDL, triglycerides), glucose and cotinine (tobacco). It also includes a biometric screening to measure blood pressure and obtain height and weight measurements (Body Mass Index).

Scheduling Your ScreeningPlease visit www.DivingIntoWellness.com for the hyperlink. Then select “Register Now” on the rightside of the screen. Registration Key: POOLCORP2016, Unique ID: (your employee number) or(employee ID + S) for spouse. Beginning July 1st, you and your spouse, if on a POOLCORP medicalplan, can schedule an appointment for a screening at your sales center/location, but you must do so within 14 days before the event date or you will not be guaranteed an appointment.

There will be an onsite wellness screening for your convenience taking place during the month of August at your sales center/location. The onsite screening is a fasting test administered through a blood draw. Each screening will take between 10 to 20 minutes per person. Results will be mailed within 10 business days after your screening date.

Day of ScreeningFasting is required for 8-12 hours prior to your screening. Please drink at least two glasses of water the morning of the screening and take all prescription medications normally.

WELLNESS SCREENING POINTS

Category RangesYour results will fall into one of three categories (Healthy, Moderate, or High Risk), and you will receive points for your range in each of the four areas. You will also earn 40 points for testing tobacco free.

HEALTHY RANGE: 40 POINTSMODERATE RANGE: 20 POINTS

Year-Over-Year Category Improvements 20 POINTS PER CATEGORYEarn 20 points for improving in each category range from 2015 to 2016.

Year-Over-Year Tobacco Improvement ADDITIONAL 50 POINTSIf you tested positive for tobacco in 2015 and test negative for tobacco in 2016, you will earn 50 points.

HIGH RISK RANGE: 5 POINTSTOBACCO FREE: 40 POINTS

Personal Health Coaching 50 POINTS • 100 POINTS MAXTelephonic coaching connects you with one of MedCom’s health coaches to discuss weight management, stressmanagement, or tobacco cessation. You may enroll at any time by calling (866) 284-5268, but are limited to oneprogram at a time. Program length varies by health need.

Online Workshops 25 POINTS • 100 POINTS MAXOnline workshops are self-paced programs designed to educate and empower you to take control of your lifestylechoices. The duration of these programs range from 5-12 weeks based on topic. You may enroll in one workshopat a time.

Wellness Webinars 15 POINTS • 60 POINTS MAX Wellness experts have prepared 15-20 minute sessions available in the Wellness Center on a variety of interestingtopics. After watching, simply take the corresponding quiz to receive credit.

Spring/Fall Wellness Challenge 30 POINTS A fun and engaging 4-week wellness challenge will be available in the fall. The challenge will consist of a combination of activites and Wellness Center tasks. The challenge begins September 7th. Detailed information will be provided prior to the start of the challenge.

Sporting Events 30 POINTS PER EVENT • 90 POINTS MAXEarn points for participation in a run/walk/bike race or an organized sporting event. Upload proof using theveri�cation tool. For more information about how to upload veri�cation, go to DivingIntoWellness.com.

PROGRAM ACTIVITIES - Earn points now through November 30.

Health Assessment 10 POINTSThe Health Assessment is an online survey used to pro�le your overall health. It will help you gain a better understanding of your overall healthy state, identify any health risks you may have and provide a platform for dialogue with your healthcare professionals. These results will aid in customizing your wellness experience. Log into your Wellness Center through www.DivingIntoWellness.com and click on HEALTH ASSESSMENT on the navigation bar.
