The agenda was approved as presented. 3.0 APPROVAL OF MINUTES The minutes from the March 21, 2017 meeting were approved as presented.

4.0 AFC ADVISORY COMMITTEE TERMS OF REFERENCE

AFC Advisory Committee TOR.pdf Jane reviewed the attached Terms of Reference (TOR) for the newly formed Age-Friendly Communities Advisory Committee. Revisions from the previous TOR are highlighted within.

5.0 WHAT’S HAPPENING (local community initiatives) - ROUNDTABLE A roundtable discussion took place regarding local community initiatives. A summary of the discussions are below:

a) The County of Simcoe has received approval to move forward with an in-depth design of 100 affordable housing units in Wasaga Beach (mix of 1, 2 and 3 bedroom units).

b) The County of Simcoe Social Housing Department has finalized the annual report card for the County’s 10 year Affordable Housing and Homelessness Prevention Strategy. An information report will be provided to County Council in the near future.

c) The Town of Wasaga Beach will be releasing their Age-Friendly Community Plan in the near future.

d) The Simcoe County Library Cooperative is supporting the Local Integration Partnership’s public awareness campaign addressing racism and discrimination in Simcoe County. The campaign will run from June 1st to 30th and everyone living/working/visiting Simcoe County can pledge. Pledge forms are available at local municipal libraries.

e) The County of Simcoe Paramedic Services Community Paramedicine has received base funding of $294,900 from the NSM LHIN.

f) A discussion took place on how difficult it can be for rural isolated seniors to receive the supports they need.

g) OCAN (Orillia Community Action Network) Georgian College students partnered with the OPP and NSM LHIN to do an analysis of emergency calls to LTC Homes in the Orillia area to better understand the nature of the

calls. Analysis shows that some calls should be directed to 2-1-1 than 9-1-1. Rosslyn will forward a copy of the report to this committee once received.

h) A “Food Security Forum” will be hosted at the Simcoe County Museum on June 14th. See attached for details.

JUNE 14 FOOD SECURITY FORUM - I

i) The government has partnered with the Ontario Trillium Foundation (OTF) on the Local Poverty Reduction Fund. The Fund is providing funds to support innovative, local, community-driven solutions that improve the lives of those most affected by poverty. https://www.ontario.ca/page/local-poverty-reduction-fund?_ga=2.158084154.1427078329.1496089451-1594571858.1496089451

j) The Town of Innisfil has partnered with Uber to subsidize the cost of rides for its residents by offering flat rates to travel to certain public hubs and discounts for travel anywhere else in the community.

k) County Council has approved the budget to begin implementation of an inter-municipal transit program (linkages between municipalities). The program will be phased in over the coming years.

l) A new home for homeless youth has been constructed in Collingwood – The Barbara Weider House for Young People.

m) Collingwood will be implementing a program where seniors can attach portable lights to their clothing to improve their visibility at nighttime.

n) The Cyber Seniors project led by SMIFS is underway. The first sessions has been postponed to September due to technical issues.

o) National Access Awareness Week takes place from May 29 to June 4, 2017.

p) June is Seniors’ Month in Ontario. 2017 marks the 33rd annual Seniors’ Month. This year’s theme is “Living Your Best Life.”

q) Independent Living Services provides support and services to people with

physical disabilities or limitations in Simcoe County. They can also connect people to other relevant organizations.

r) The Township of Tay will be seeking provincial funding to begin work on a Local Aging Plan for their community.

s) The Ontario government has announced their commitment to invest $100M over three (3) years towards the implementation of a dementia strategy.

t) The Alzheimer’s Society has received funding from the Ontario Trillium grant to bring the “Minds and Motion” program to Simcoe Muskoka for people in the early stages of dementia and their care partners.

6.0 HOSPICE PALLIATIVE CARE NETWORK PRESENTATION

Sandra Dunham, Executive Director, North Simcoe Muskoka Hospice Palliative Care Network (NSMHPCN) provided the attached presentation. THE NSMHPCN is an alliance of health care providers, volunteers, and consumers who facilitate the planning and development of palliative care, to the residents in our communities through the provision of:

A broad system for palliative care coordination and integration of palliative care services at a systems level monitoring and assessment of community needs promotion of service innovations palliative care service, education and research

It was discussed that the NSMHPCN has a 24-hour support line for healthcare providers. A discussion took place regarding the addition of a fourth tier entitled “being a friend” to the Levels of Palliative Care pyramid found on slides 9 and 10. A discussion took place regarding the Tyze Network. Tyze is an online personal networking tool that brings people together around someone receiving care. Tyze is a simple way to get organized and keep everyone informed. It allows a person to:

Save time: Send updates to everyone at once from anywhere. Get organized: Keep track of everything in one place. Home care

appointments, doctor visits, medication lists and documents can be easily stored and accessed.

Reduce stress: Share the care – easily let family and friends know how to help.

Stay connected: Share news, stories and photos.

7.0 DRAFT PLANNING FRAMEWORKS

7.1 Civic Participation and Employment

and stakeholders should be identified at a high level (e.g. use department names not names of individuals). AGE-FRIENDLY COMMUNITIES ADVISORY COMMITTEE

Terms of Reference

Age-Friendly Community Dimensions: The World Health Organization framework for Age-Friendly Communities can be used to assess and enhance the age-friendliness of communities. The framework identifies eight aspects of community life that overlap and interact to directly affect older adults, as such:

Goal: To develop and seek Council endorsement of Simcoe County’s Positive Aging Strategy: Older Adults Strengthening our Communities including a planning framework.

Objectives: To engage community members to assess how age-friendly Simcoe County is,

within the above eight domains and to determine age-friendly priorities. To develop a coordinated approach by engaging stakeholders to help define the

key principles for aging well within Simcoe County. To increase awareness in communities to help support healthy aging. To incorporate age-friendliness in our communities into the short and long-range

strategic planning process for the County of Simcoe. To provide advice and support in the ongoing activities of Age-Friendly

Community Planning.

Mandated Membership: The Age-Friendly Communities Advisory Committee will be comprised of 20-30 members. Membership will include:

County of Simcoe Staff. 2 County of Simcoe Councillors appointed by Simcoe County Council. Representatives from senior-serving organizations. Representatives from disability-related organizations. Representatives from Seniors Advisory Committees. Representatives from the cities of Barrie and Orillia.

Representatives from the LHINs and Health Links Representatives from Francophone and Aboriginal organizations. Representatives from the local business community or Chamber of Commerce. Individuals involved with one (1) or more of the eight (8) age-friendly domains. Members of the public.

Term The implementation of this committee will begin May 2017. The terms of the committee will be re-evaluated Fall 2018 or at the end of the current County Council term.

Mandate The committee is mandated to:

Provide strategic direction, planning and guidance in the undertaking of Simcoe County’s Positive Aging Strategy, including identification of:

- ways in which the communities are currently accessible, inclusive and age friendly;

- issues and barriers to age-friendliness that are present in the communities and their services;

- opportunities to improve the accessibility, inclusivity and age-friendliness of the communities;

- gaps and ways in which current age-friendly assets and initiatives can be expanded.

Engage a diverse range of community residents/stakeholders to inform the development of the age-friendly assessment.

Disseminate the results of the assessment. Based on the results of the assessment develop a planning framework that acts

as a guide for municipalities in future development. Confirm project charter and monitor progress throughout the term. Anticipate and review project constraints and challenges and provide

suggestions for improvements and resolutions. Act as a forum and catalyst for community collaboration around the objective(s)

of making the communities more age-friendly. Promote the concept of age-friendly across the communities, including the

private, non-profit and public sectors. Engage members of the business community as key partners in making work

places and private sector services in the communities more age-friendly.

Working Groups The committee has the authority to establish working groups as it deems necessary to fulfill designated functions consistent with the committee’s mandate.

Meetings Meetings will be held bi-monthly (every 2 months), with additional meetings held on an as needed basis, as determined by the Chair.

Reporting The committee shall report through the Chair to the County Council.

“Maybe for volunteer hours a student could be partnered with a senior that would teach them how to knit, sew or some other skill that the senior has. Maybe a student could do a project about genealogy where they are partnered with a senior who tells them about what their life was about and the history of the area. Start a club for anything where the senior is encouraged to come to a group meeting once a week to read, sew, knit, or work on a project that is relevant to that person. Let the senior LIVE”
