India is the world capital for chronic lung diseases. The largest number of deaths due to chronic respiratory
diseases in the world occur in India, even after controlling for the total population. India leads the world in 1,2
asthma mortality and has the second largest number of deaths in Chronic Obstructive Pulmonary 1,2
Disease (COPD) . Chronic respiratory diseases are the second leading cause of death in India. According
to crude estimates, about 100 million people in India suffer with chronic respiratory diseases,
predominantly affecting the airways. One of the largest studies conducted to understand what ails 3
India reports that 50% of the patients, who visit a primary care physician in India, do so for a respiratory
cause, with over half of them visiting a doctor for a chronic respiratory problem. An estimated 3.5 million
patients visit a doctor every day in India for asthma or COPD alone. The prevalence of Asthma among 4,5
children in certain cities like Pune and Bangalore has doubled over the past 5-10 years . The prevalence of 6
COPD has also been shown to be much higher than previously believed . A significantly large proportion is
attributable to causes other than tobacco smoke like exposure to biomass fuel and indoor and outdoor air 7
pollution .
Despite knowledge of the increasing prevalence of chronic respiratory diseases and the emerging newer
risk factors that are responsible for this exponential growth, the fact remains that the quality of care that
we offer to our patients with chronic respiratory diseases remains poor. Obstructive Airways diseases are 8
often under diagnosed and inappropriately managed . A study from a tertiary undergraduate and post
graduate teaching hospital in Pune showed that up to 65% cases of obstructive airways diseases remained 9
undiagnosed and therefore untreated . Despite the fact that only clinical diagnosis without a spirometry 10,11,
can lead to missing more than 50% of cases of OADs spirometry remains poorly used by general 12,13
physicians across India . This under diagnosis coupled with improper management of Asthma by 14
primary care physicians is leading to poorly controlled disease conditions, leading not only to physical
distress to the patient and their families, but also loss of work days, school absenteeism, night time
awakenings, higher rates of acute exacerbations and the growing need for hospitalization, and sometime 15,16,17,18
even intensive care . Not only this, it also imposes a huge and unnecessary economic burden on our 19,20,21
country . One of the main reasons for the under diagnosis and poor management of chronic
respiratory diseases, is inadequate emphasis on respiratory diseases in medical schools, including lack of
training in spirometry during undergraduate and postgraduate curriculum. A study from a tertiary care
teaching hospital in Pune identified that less than 1% of the doctors and nurses being trained or already
trained in modern medicine knew how to use a pressurized metered dose inhaler though most of them 22
believed otherwise . More worryingly, over 50% of them believed that inhalers were addictive. This
highlights the very poor level of knowledge that both undergraduate and postgraduate doctors have
before they start their practice.
The quality of services provided by the PCPs are weak as compared to specialists. Hence, there is a need for
strengthening their competencies to effectively manage COPD and Asthma at primary and secondary
level. “Certificate Course in Management of COPD & Asthma" (CCCA)” is an effort in this direction.

Prof. Ghoshal is a Medical Director at National Allergy Asthma Bronchitis Institute,
Kolkata. He was the President of Indian Chest Society & Governor to American College of
Chest Physicians. He has more than thirty years of clinical and teaching experience as
Professor and Head of Respiratory Medicine. He nurtured the stream from its infancy to
its present super specialty status in Bengal and in India. A revered teacher, academician
and researcher, he has illustrious students across the globe. But the most he is known as a
master clinician.
He specializes in Obstructive Airway Diseases (Asthma / COPD), Diffuse Parenchymal

Dr. B.V. Murali Mohan (Bengaluru)
Dr. Mohan is currently working as Senior Consultant Pulmonologist and Head of the
department of Internal Medicine and Pulmonology at Mazumdar Shaw Medical Center
(MSMC) in Narayana Health City, Bengaluru.
He was earlier Professor and Head of the Department of Medicine at Dr. BR Ambedkar
Medical College, Bangalore. After completing his MBBS and MD (General Medicine) at
Bangalore Medical College, he took his MRCP (UK) from the Royal College of Physicians
of Edinburgh, and post-MRCP training in Respiratory Medicine at Ninewells and Kings
Cross Hospitals, Dundee, and at the Newcastle General Hospital, Newcastle upon Tyne.
He subsequently took the Speciality Certificate Examination in Respiratory Medicine
from the UK and was awarded the Fellowship of the Royal College of Physicians and Surgeons of Glasgow, UK.
Dr. Mohan is especially interested in the areas of Asthma, COPD, Sleep Medicine and Cardio-Pulmonary
Resuscitation (CPR). He has over 30 papers presented at various national and international conferences, and
several papers in international journals. As a clinician, his experience in treating interstitial lung disease and
pulmonary hypertension patients makes the Pulmonology department of NH-MSMC a referral center for
patients with these disorders. His research at MSCTR involves investigating the pathobiology and potential
molecular targets of pulmonary fibrosis, and is now embarking on the genetics of pulmonary hypertension. He is
the joint author of a WHO manual (under print) on Tobacco cessation for Medical and Dental Professionals. He
also heads the Nightingales Lifesaving Services, which has, to date, trained over 20000 persons in CPR in
Bangalore.

Dr. Harjeet Dumra (Ahmedabad)
Dr. Dumra is a Director, Chest Disease Center at Sparsh Hospital in Navrangpura,
Ahmedabad and Consultant Interventional Pulmonologist, Sleep Medicine and Critical

Dr. George Albert D'Souza (Bengaluru)
Dr. D’Souza is Dean of St. John's Research Institute; Professor and Head Division of Chest
medicine at Bengaluru. He did MD in Internal Medicine & DNB in Respiratory Diseases
from PGIMER, Chandigarh. He initiated the pulmonary services of SJMCH in 1989 and as
part of these services also established the TB services. SJMCH was the first private sector
hospital to be part of the National Tuberculosis Program under the RNTCP. He had also
worked as a Research fellow (FACT) in the department of thoracic surgery at Freeman
Hospital, Newcastle upon Tyne, U.K. for 3 years (1991 to 1993).
He holds a position of examiner in various boards and universities. Also, Dr. D’Souza is an
intramural member of executive committee and governing board of St. John’s National
Academy of Health Sciences and has been associated extramurally in Board of super specialties of Rajiv Gandhi
University of Health Sciences and Research Advisory Board of Manipal University.

Dr. Deepak Talwar (Noida)
Dr. Talwar is a Director and Chair, Department of Pulmonary, Allergy, Sleep & Critical Care
Medicine at Metro Group of Hospital and Heart Institutes, Noida. He has completed his
diploma and MD post graduation while working in V. P. Chest Institute, Delhi and later did
post doctoral DM in Pulmonary & Critical Care Medicine from PGIMER, Chandigarh.
Dr. Talwar is a member of many national & international medical associations and has

listed in the Limca Book of Records 2000. Dr. Talwar’s expertise and research is in COPD,

Dr. J.C. Suri (Delhi)
Dr. Suri is a Consultant, Professor and Head in Department of Pulmonary, Critical Care
and Sleep Medicine at Vardhman Mahavir Medical College & Safdarjung Hospital,
New Delhi. He has introduced the field of Clinical Sleep Medicine in India and
established the first state-of-the-art sleep lab of India, at Safdarjung Hospital in 1990.
He is a Founder President & Chairman of Indian Sleep Disorder Association and Ex-
President of National College of Chest Physicians.
Dr. Suri is an associate editor of Lung India, Indian Journal of Chest Diseases, Indian
Journal of Tuberculosis and Editor-in-Chief of Indian Journal of Sleep Medicine. He is
also on the Editorial Board of several other national and international journals.

Dr. Nitin Yashawant Abhyankar (Pune)
Dr. Abhyankar is working as a Consultant Pulmonologist and Head of Department of
Pulmonary Medicine at Poona Hospital and Research Centre, Pune. After pursing his
MBBS and MD (Internal Medicine) from Mumbai University, he did European Diploma in
Adult Respiratory Medicine from Berlin in 2008. Dr. Abhyankar was the first
Maharashtrian doctor to pass this prestigious examination. He is one of the 300
HERMES diplomates around the world.
He is also working as a Course Co-coordinator - Respiratory therapy at Poona Hospital
and Research Centre, Pune. He is a Founder and Director of School of Interventional
Pulmonology since year 2012 and has trained 7 students in flexible Bronchoscopy and 6
students in medical thoracoscopy till date. Dr. Abhyankar holds an experience of 10 years as teaching faculty in
DNB Medicine Program at Poona Hospital & Research Center.

Dr. Parthasarathi Bhattacharya (Kolkata)
Dr. Bhattacharya pursed his MD (Internal Medicine) and DM (Pulmonary Medicine) from
Punjab University & PGIMER, Chandigarh. He has worked as a Chief Pulmonologist of the
EKO-BRONCHOSCOPY center, Calcutta for over 5 years. Dr. Bhattacharya is currently a
Secretary and founder of non-profit society, Institute of Pulmocare and Research with
the motto of research, education & patient care & has been associated with it fully from
2007 onwards. With his great interest in teaching Dr. Bhattacharya has been involved
formally or informally in many institutes from the beginning of his career. He has served
SMST, IIT Kharagpur for over two years as an adjunct professor & is presently involved in
IIEST, Shibpur. He has completed several projects & has been investigator or co-
investigator in over 15 ongoing projects at the IPCR, IIT Kharagpur etc.

Dr. Dhar is a renowned Consultant Pulmonologist, Intensivist and Coordinator at
Department of Pulmonology and Critical Care Specialist in Fortis Hospital, Kolkata with
over 22 years of professional experience. Dr. Dhar is proficient in all disciplines of
respiratory medicine including airways disease, pulmonary fibrosis, pulmonary

Prof. Parvaiz A Koul (Srinagar)
Prof. Koul is working as a Professor and Head, Medicine in Sher-i-Kashmir Institute of
Medical Sciences (SKIMS), Srinagar. He is also working as a Nodal Officer for AIDS and
HIV, NACO India; Nodal Officer & Head, Regional Geriatric center, SKIMS; Chairman, Core
Committee on RNTCP, J&K etc. After completing his MD in Internal Medicine, Prof. Koul
has pursed fellowship in Bronchoscopy, Pulmonary and Sleep Medicine from LRS
Institute of TB & Lung Disease, Delhi and North Shore LIJ Health System, Long Islands,
New York respectively. He is a member of many national and international professional
bodies like Indian Chest Society, Indian Society of Bronchology, World Association of
Medical Editors, American College of Physicians, etc. Prof. Koul has recently been
awarded with Bill & Mellinda Gates Awards in International Congress on Infectious Diseases 2016, Best Referee
Award Indian Chest Society, 2011 for Lung India and many more. He has more than 140 publications to his credit,

Medicine, Lancet, and Lung India etc. He has delivered more than 400 presentations, lectures, talks at various
conferences, symposia and CME programs at the state, national and international level.
Prof. Koul has set up an independent state-of-art Influenza Lab. His current interests include Influenza,
Pulmonary and Sleep Medicine. Owing to his interest in serving the community, Prof. Parvaiz Koul has organized

He went to UK after completing his MBBS and an MD in Respiratory Medicine in Calcutta, India. He did his PLAB
exams and then trained initially as a Clinical Fellow/Senior House Officer and then as a Research and Training
Fellow. During this period he did MRCP and also did Certificate of Clinical Education. He had a major declared
commitment to teaching during the time and also carried out Clinical Research projects which he presented at
National and International meetings. He then secured a LAT post followed by a NTN in Respiratory and General
Medicine. During the period after getting into the NTN post he worked in District General Hospitals and tertiary
referral hospitals with distinction. He completed Research projects and presented and published these during
the course of his work. He then worked for more than 1 year as a Respiratory Consultant with special interest in
Lung Cancer and Interventional bronchoscopy and thoracoscopy etc at a tertiary care centre in St James’
University Hospital in Leeds. He came back to Kolkata, India in Dec 2008 to serve his motherland and joined

Dr. Guleria, is a Professor and Head, Department of Pulmonary Medicine and Sleep
Disorders at AIIMS, Delhi. He has obtained degree of MD in Medicine and DM in
Pulmonary Medicine from PGIMER, Chandigarh. He has acquired many awards and
honors including prestigious “Padma Sri” award, the fourth highest Indian civilian award
in 2015 in the field of Medicine, "Raj Nanda Pulmonary Disease Fellowship" awarded
jointly by the Raj Nanda Trust and the Royal College of Physicians, UK in 1997 for training
and research and many more. He has published 342 research articles and written 48
chapters in national and international books. He has delivered more than 200 invited
lectures at different national and international conferences, CME and updates.
He is a member of the many medical associations of national and international bodies. He is also on the Advisory
board of WHO for the SAGE working group on Influenza vaccines and immunization, consultant on the technical

Dr. Salvi is the Director of the Chest Research Foundation, a state-of-the art research
institute dedicated for research and education in the field of respiratory diseases, with a
major focus on Obstructive Airways Diseases. He is an Honorary Senior Lecturer at
Imperial College, London, a Visiting Faculty at the Johns Hopkins University, and a Ph.D
guide at the University of Pune and the Symbiosis International University. After
completing his specialization in Respiratory Medicine at the University of Pune, he spent
8 years in the UK researching the effects of air pollution on the human lung. He earned a
Ph.D from the University of Southampton, UK, and returned to India in 2002 to set up the
Chest Research Foundation. He has contributed significantly to research in Obstructive
Airways Diseases in India and has pioneered research in non-smoking COPD. He was invited by Lancet to write a
review article on this topic which now has become a major research focus for academicians all over the world. He

conducted by the Chest Research Foundation for chest physicians, physicians,
pediatricians, general practitioners as well as paramedical staff. After completing her
graduation in general medicine at the Maharashtra University of Health Sciences, she has
worked as a casualty medical officer across hospitals in Pune, Maharashtra, India. She
completed her post graduate diploma from Bilcare Private Limited in association with

Foundation for the last 6 years. She has coordinated a landmark nationwide point prevalence study on the

Dr. Barne is the Head of the Training Programmes Division at Chest Research Foundation,
Pune, a state-of-the art research institute dedicated for research and education in the field
of Obstructive Airways Diseases (OADs), primarily Asthma and COPD. Dr. Barne has been
instrumental in creating an identity for Chest Research Foundation as a leading institute
providing continuous medical education in Asthma and COPD to practicing Physicians,
Chest Physicians, Pediatricians, General Practitioners and Post Graduate Students of
Medicine, Pulmonology and Pediatrics. Under her leadership, CRF has trained more than
10000 doctors across India as well as internationally. She has contributed immensely
towards capacity building amongst paramedical staff by initiating several training
programmes for them to learn diagnostic and management tools for Asthma and COPD. She has been the lead in
uniting Heads of the Pulmonology, Medicine and Pediatric Departments of Medical Colleges across the country

Dr. Rahul Kodgule
Dr. Kodgule is the Head of Academic Research and Director of Advanced Pulmonary
Function Testing Program at the Chest Research Foundation, Pune. After serving the
Indian Army as a Short Service Commissioned Officer, Dr. Kodgule joined Chest Research
Foundation in 2008. His key areas of interest are research in asthma and COPD and lung
function testing.
Dr. Kodgule has published several original research and review articles in national and
international journals. He received several awards from national and international
respiratory societies for his contribution towards research. Dr. Kodgule is a reviewer for
several journals. He has also functioned as principal investigator for clinical trials at Chest
Research Foundation.
Dr. Kodgule played a key role in the development of a stethoscope that can be connected to a smart-phone and is
currently working on exploiting the rich data in lung sounds for understanding respiratory diseases better. He also
derived the reference values of Peak Expiratory Flow Rate for Indian adult population. Dr. Kodgule has developed
expertise in lung function tests like spirometry, impulse oscillometry, body plethysmography, diffusion lung

Dr. Sneha Limaye
Dr. Limaye is the Head of Department - Clinical Trials at Chest Research Foundation.
Under her leadership, CRF runs a state-of the art Clinical Trials Facility and has won the
Best Clinical Trials Award in 2012 by Indian Society for Clinical Research. After her medical
under graduation from Indira Gandhi Govt. Medical College, Nagpur, Dr. Limaye joined
Chest Research Foundation in 2007. She underwent Lab training in Sputum induction and
processing methods from University of Winsconsis in 2010 and was awarded a Fulbright
fellowship from Johns Hopkins University, Maryland, USA on conducting multicenter
studies.
Post her training at Johns Hopkins, Dr. Limaye set up the RRNI - Respiratory Research Network of India in Aug 2012,
an initiative by Chest Research Foundation. She is the National Co-ordinator for this network which has 52
registered Pulmonologists and Physicians across India, engaged in multicenter research activities. She is an

He was served for more than 5 years as Lead Consultant in Maternal Health Division of the Ministry of Health &
Family Welfare and his job responsibilities included implementation of the maternal health programme, framing
of checklists, undertaking regular supportive supervision visits, planning capacity building of various
stakeholders and framing of operational and technical guidelines in consultation with the experts.
Prior to this, in his stint of more than 3 years at NACO as Sr. Technical Officer, Blood Safety and Programme Officer

The certificate course begins with making the practicing physician realize
the need to focus on respiratory diseases and the fact that our country is
losing precious man days and money because we as practitioners are still
following the age old methods for diagnosis and management of Asthma
and COPD. Understanding investigations like Spirometry and Peak
Expiratory Flow Meter which are standard of care investigations for
obstructive airways diseases is made extremely simplified. Management
aspects are covered in detail with emphasis on patient counselling which
forms the back bone of management of all chronic diseases. Correct
technique of using the Inhaler devices is also important for proper
management of Asthma and COPD. COPD is a systemic disease and the
systemic manifestations are a major cause of morbidity and poor quality
of life. These are discussed as is long term oxygen therapy. Not all patients
of Asthma have similar presentations. The course discusses the various
avatars of Asthma and also the common co-morbids like Allergic Rhinitis
and Gastro Esophageal Reflux Disease associated with it. Smoking
Cessation and Pulmonary Rehabilitation are essential yet neglected
aspects of COPD management. The course emphasizes on the need and
ways to address these issues to gain good control of symptoms. However,
no matter how well we manage asthma and COPD, exacerbations may still
happen and the course cover how to manage these exacerbations. It also
tackles the difficult issue of end of life communication as it is best to
discuss with the patient and family of patients with end stage COPD what
their preferences and resources are like.
The course follows a continuous Monitoring and Evaluation system
throughout the cycle to ensure quality assurance and standardized course
delivery pattern on all India level.

Course Design and Duration
Certificate Course in Management of COPD & Asthma is a
unique, on the job training course with a primary objective
to enhance knowledge, skills and core competencies of
PCPs in management of chronic lung disease (COPD &
Asthma).
The total duration of the course is eight months (November
2016 to June 2017), with once a month contact session,
scheduled on designated weekend at Regional Centers,
across India.
CCCA is a joint certificate program designed and delivered
by Public Health Foundation of India (PHFI) and its academic
partner; Chest Research Foundation (CRF) and Narayana
Health (NH).

Participant assessment will be through a continuous internal
evaluation, course work and performance in written
examination. The criteria for successful completion of the
course shall be as follows:
• Participation in at least 7 contact sessions (Including the
pre-test and post-test of each module)
• Submission of two interim assignments at the end of
module III and module VI to the respective Regional Faculty
• Appearance and clearance of final written examination in
the form of 50 MCQs with min. 50% score along with

The candidate completing the certificate course successfully as
per the above mentioned criteria shall be awarded certificate of
completion jointly issued by PHFI, CRF, NH and respective
Regional Faculty.
