1. This document is compiled from notes and a diary made from collected records in the early 1970s withlater additions.

2, Sources are indicated by the references shown below, mainly individuals who have communicated

information from documents they have studied or created, in many cases providing copies. Many ofthe individuals are no longer with us but my indebtedness to them and to the officers of the

several organisations mentioned is acknowledged with gratitude and apologies for taking so long

to do so.

3. Where conflicting dates have been found and remain unresolved these are shown and are subjectto clarification.
