Payroll Newsletter February 17th, 2017 CRP Newsletter Article – January/February 2017 Reminders about Disclosing PHI A few situations have occurred recently regarding the disclosure of Protected Health Information (PHI) to the wrong patients, and the disclosure of names and diagnosis to other patients and staff. Per HIPAA regulations we are required to protect our patient’s PHI. CHI’s Privacy Standard No. 2 states: CHI Entities may not Use or Disclose Protected Health Information (PHI), except as permitted or required by law. A CHI Entity needs to obtain a valid Authorization from Individuals to Use or Disclose their PHI when the Use or Disclosure is not related to Treatment, Payment or Health Care Operations (TPO), or as required by HIPAA privacy regulations. Disclosure of PHI to the wrong patient: One of the leading causes for breaches within CHI is providing documents to patients that contain another patient’s information. Three common examples include sending discharge paperwork home with the wrong patient; sending patients to appointments within our facility with another patient’s admission information; and prescriptions being sent with the wrong patient. Each of these situations could be avoided by reviewing each page to confirm the correct patient’s information prior to releasing the document to the patient. Verbal Disclosures of PHI: With the uptick in the spread of Influenza A there have been multiple situations in which staff have disclosed patient names and the diagnosis of Influenza A to other patients. The intention has been to notify the patients who may have been exposed; however, we are still required to protect our patients PHI. The best way to avoid this situation is to rephrase the message to de-identify the information to be disclosed. For example, when contacting patients who may have been exposed by a staff member who has tested positive for influenza A, it would be best to say: Good afternoon Mr. Jones. I am contacting you to notify you that during your recent visit to our facility, you may have been exposed to the influenza A virus. If you notice any symptoms, please reach out to your regular provider or our clinic for assessment and further treatment. Thank you. Reminders about Disclosing PHI

A few situations have occurred recently regarding the disclosure of Protected Health Information (PHI) to the wrong patients, and the disclosure of names and diagnosis to other patients and staff. Per HIPAA regulations we are required to protect our patient’s PHI. CHI’s Privacy Standard No. 2 states:

The beau ful quilt above was made and donated by Amy Noeske and Cindy Fitzner.

This quilt will be used to cover pa ents at the hospital a er they have passed away and for family

members to use when viewing their loved one. It will be a beau ful way for families to say

good bye and be with their loved one. On February 10th Sister Dorothy

blessed the quilt during the prayer service.

When you go on the website, please check the $15 paid membership (not the FREE) and fill out the form, when you come to the question, ‘Is your place of employment paying for part or all your registration’ check ‘yes’ and a drop down box will pop up for you to ‘click’ on CHI Mercy Health. Your membership fee will zero out at this point (remember CHI Mercy Health is paying your membership fee), if you choose to join as a family, CHI Mercy Health will pay for your membership fee and the confirmation page will tell you what you still owe for your family membership and where you should send your payment.

The Young People’s Healthy Heart Program at CHI Mercy Health worked with the Washington School grade 4 students for 7 weeks focusing on keeping a good blood pressure. The unit culminated in a Family Fun and Fitness Night on Thursday, Feb. 9

where students taught their parents and grandparents and served healthy snacks. Over 80 people learned how to keep a healthy blood pressure that night!
