Early Bird registration applications must bepostmarked by September 19, 2014 (Washington, D.C.) and October 10, 2014 (Las Vegas). The Early Bird tuition is $795 for physicians, $595 for retired physicians/sonographers/vascular technologists and $595 for residents/fellows/ full-time military (letter required).

The full course tuition for applicants postmarked after September 19, 2014 (Washington, D.C.) and October 10, 2014 (Las Vegas) is $835 for physicians, $635 for retired physicians/sonographers/vascular technologists and $635 for residents/fellows/full-time military (letter required).

Cancellations received in writing up to twoweeks prior to the course will receive arefund minus a $50 processing fee. Therewill be no refunds for cancellations receivedafter that date. Refunds will not be given for“no-shows”.
