RESEARCH FOCUS: Over the course of my career, I have led a research group that has focused on the discovery and validation of cancer biomarkers with the underlying goal of enhancing the diagnostic accuracy for cancer in Surgical Pathology and Cytopathology specimens. Board Certified in Anatomic and Clinical Pathology (1991), with subspecialty certification in Cytopathology (1995), I served as the Director for Cytopathology and Surgical Pathology and as the vice chair for Anatomic Pathology at the University of Colorado Health Sciences Center. Following my recruitment to Stony Brook University Medical Center as Chair of the Department of Pathology in 2007, I remained active in the practice of Surgical Pathology and Cytopathology, took on additional roles as the Director of the Stony Brook BioBank and Research Histology Core Laboratories, and continue to lead a translational research program that trains graduate students, postdoctoral research fellows, medical students, and residents in advanced histologic and molecular approaches for tissue-based cancer research. At Colorado, I invented the method of DNP labeling of nucleic acid probes, was a pioneer in the development of methods for in situ hybridization of mRNAS in the early 1980s, and was the first to report the molecular analysis of x-chromosome inactivation in archival tissues as a marker of clonality. I also participated in the early validation of tyramide-based signal amplification technology for in situ hybridization and high sensitivity immunohistochemistry.

Diagnostic Cancer Biomarkers: A major focus of my translational research at the University of Colorado from 1991-2007 was in the identification and validation of objective molecular approaches to improve diagnostic accuracy in surgical pathology and cytopathology. The ultimate aim of this work was to provide pathologists with molecular markers that can be integrated and interpreted in the context of tissue histopathology and cytopathology. My lab was the first to report the analysis of telomerase expression as a biomarker of cervical cancer, provided the first comprehensive evaluation of p16 expression in cervical neoplasia, and pioneered the development of molecular diagnostic adjuncts to improve diagnostic accuracy for HSIL and cervical squamous cell carcinoma. Under my direction, the Surgical Pathology service at the University of Colorado became the first lab in the world to deploy p16 testing in pathology diagnostic laboratories to improve diagnostic accuracy. I was also the first to report that survivin, an inhibitor of apoptosis, is a prognostic biomarker in breast cancer and my lab also was the first to identify NQO1 as a potential biomarker of pancreatic ductal adenocarcinoma. Finally, based on in silico, in vitro, and immunohistochemical studies, my lab was the first to report that B7-H4, a regulator of T cell activation, is highly overexpressed in cancer and is a sensitive biomarker of ovarian, uterine, breast, and pancreatic adenocarcinoma. Prognostic Cancer Biomarkers (2007-present): Using cervical cancer as a model system to identify prognostic markers, my group discovered by mass spectrometry and validated by immunohistochemistry that keratin 17 (K17) is more accurate than clinicopathologic assessment to predict patient survival. Although K17 was previously known to provide mechanical support in normal epithelial cells, we found that K17 also has oncogenic functions, not only in cervical cancer, but also a wide range of cancer types, including pancreatic ductal adenocarcinoma (PDAC). To identify the molecular mechanisms through which K17 controls fundamental aspects of tumor cell biology and thus, is a rational target for therapeutic intervention in PDAC, I assembled an inter-institutional team of investigators that have tremendous experience with orthotopic and transgenic mouse models of pancreatic cancer, the development of PDAC organoid cultures and patient-derived xenografts, and RNA-Seq data analysis and predictive modeling. Our studies, currently funded by the Pancreatic Cancer Action Network (PanCAN) are the culmination of our collaborative efforts to further understand the mechanisms through which K17 promotes tumor aggression and to provide a rationale for targeting K17 in PDAC.
