2005-2006 Magnetic Resonance Encephalography – Direct MR Imaging of Epileptic Activity NYU Radiology Department Pilot Project $10,000 Grant PI The major goal of this project was to assess the feasibility of using high-speed gradient echo MR imaging to visualize epileptic foci in patients with intractable epilepsy.

2006-2009 Brigham and Women’s Radiology Startup Research Fund $150,000 PI These funds have been directed towards the encephalographic functional Magnetic Resonance Imaging project described below.

2008-2009 Encephalographic Functional Magnetic Resonance Imaging – Direct MR Imaging of Cerebral Electromagnetic Activity Brigham and Women’s Radiology Foundation Award $25,000 PI The major goal of this project is to develop a technique to directly image electromagnetic activity in the brain using simultaneous EEG and high-speed MR acquisition. Patient cohorts being studied include patients with intractable epilepsy as well as normal subjects performing visuomotor tasks and sleeping subjects.

Coinvestigator The major goal of this study is to investigate the safety and feasibility of using MR-guided ultrasound to treat inoperable brain lesions. My role is to provide neuroradiological guidance in screening potential subjects, performing treatment sessions, and assessing treatment response.

2011-2012 Fast Volumetric MRI for Localizing Electromagnetic Discharges in Pediatric Epilepsy $47,000 Boston Children’s Hospital-MIT Collaborative Grant Program Co-PI (with Elfar Adalsteinsson at MIT) The major goal of this study is to extend eMRI beyond planar acquisition by developing a volumetric mode of acquisition for localizing epileptic discharges in pediatric patients

2011-2014 Thrombolysis in Pediatric Stroke (TIPS) $26,000 (Children’s Hospital subcontract) NIH Coinvestigator An NIH-funded US-Canadian consortium of 20 pediatric facilities to examine the safety and efficacy of intravenous thrombolysis for acute childhood stroke. Boston Children’s Hospital has been selected as the imaging core lab for this study, and I will serve as one of two readers of the associated imaging studies.

Description/Purpose of the project 2016-Present Vascular-Neural Interfaces

MIT (Ed Boyden, PI) / Columbia University Co-investigator, Institutional PI This project has the goal of developing novel miniature and nano endovascular devices designed to directly record and stimulate electrical neuronal activity. First-generation devices are modifications of cerebral stents, with incorporated chip boards that are wirelessly powered and wirelessly transmit, and are capable of multi-unit recording and stimulation; these are being trialled in rabbit and pig animal models for deployment in the distal internal carotid and vertebrobasilar system. Next-generation devices are designed for single-unit recording and stimulation, for deployment in the microvasculature. Such systems may prove to be powerful basic neuroscience tools for measuring brain activity, to have high clinical utility as novel diagnostic and therapeutic tools for epilepsy and movement disorders, and to be instrumental in the development of brain-machine interfaces.

Andrea Rosi, MD University of Florence Careggi Hospital, Florence, Italy (Neurointerventional Radiology) As the penultimate step towards completion of his neurointerventional radiology training at the Careggi Hosital of the University of Florence, Dr. Rosi was awarded a research fellowship by the Meyer Fonazione Pediatrico in Florence, to spend spent 3 months full time at Children’s Hospital, in an effort to broaden his exposure to the spectrum and management of pediatric neurovascular disease. In addition to observing all neurointerventional procedures during that time, Dr. Rosi initiated and brought close to completion a major project analyzing the clinical and radiographic characteristics of moyamoya in our Children’s cohort, as well as an analysis of currently used grading scales and risk factor assessment. This culminated in his thesis project, submitted to Careggi, and is expected to lead to several publications that will represent major contributions to the field of moyamoya.

Clinical Innovations • Working closely with colleagues from Neurosurgery and Neurology to build a nationally and

internationally recognized multidisciplinary group for the diagnosis and treatment of pediatric neurovascular disease, offering the full gamut of treatment for pediatric stroke and vascular malformations of the brain and spinal cord. Consultations are received and patients are treated from throughout North America, South America, Europe, and Asia. Seamlessly tight integration of all aspects of clinical care, follow-up, and outcomes research in this patient cohort are now a reality, with the formal establishment of the Cerebrovascular Surgery & Interventions Center at Children’s in late 2013, co-directed by Dr. Edward Smith of Neurosurgery and me.

• Pioneering the use of novel embolic agents and creative use of access devices developed for adults (e.g. Onyx, intermediate catheters, novel vascular occlusion devices), in children with intracranial and extracranial vascular anomalies. Close work in assessing treatment response is ongoing with the Vascular Anomalies Center and colleagues from the departments of Plastic Surgery, Otolaryngology, and Neurosurgery.

• Development of a program for intra-arterial chemotherapy for retinoblastoma, in coordination with colleagues in Oncology and Ophthalmology. In this context, we serve on the steering committee for the Children’s Oncology Group formal study of intra-arterial retinoblastoma.

• As Division Chief of Interventional Radiology, fostering the development of Interventional Radiology-based treatment approaches to solid tumors at Boston Children’s Hospital and the Dana Farber Institute, in coordination with colleagues in Oncology, General Surgery, and Head and Neck Surgery, and Orthopedics. Approaches range from participation in international, multicenter randomized prospective trials for primary therapy, to adapting techniques used by interventionalists in adults (e.g. RF ablation, cryotherapy), for children with recurrent or resistant tumors, to palliative IR-based treatment for pain and functional limitations.

• Overseeing the development and implementation of an electronic Interventional Radiology case protocol system, allowing for iterative input from attending physicians, nurse practitioners, technologists, and schedulers. The system replaced the paper-based protocolling that had been in place and resulted in a significant gain in efficiency and drop in error rate.

• Overseeing the rollout of the first known Radiologists’ Assistant program nationwide, in Interventional Radiology in a pediatric setting. Our RA placed vascular access lines and enteric tubes and performs biopsies and drainages under the supervision of IR attending physicians. Her work dramatically increased our Division’s clinical efficiency, while she consistently earned praise from patients and families. Her complication rate and average procedural fluoroscopic dose were equal to or lower than the attending physicians’. Implementing this program required close coordination with the Hospital and Department administration, as well as with numerous intra-divisional caregivers. The program was stopped due to absence of a national billing infrastructure for RA work.

• Developing a novel fetal intervention for vein of Galen malformations, along with colleagues from Children’s and BWH in high-risk maternal-fetal medicine, pediatric radiology, pediatric anesthesia, and neonatal intensive care. Ultrasound-guided direct transabdominal needle access to the vein of Galen malformation via the fetus’ posterior fontanelle will be attempted on a compassionate care basis in three cases where, based on current treatment options, the fetus faces a poor prognosis. The technique is being developed using a fetal brain model custom-built by the Children’s simulation group. If successful in the three early cases, a formal IRB-approved clinical trial assessing the safety and efficacy of fetal treatment of vein of Galen malformations will be run.
