Program Evaluation QuestionsWhat changes are made in school environments

Collection of activity logs and pedometer readings in 3 separate classrooms in grades 4 through 11

ID# Determination/CoordinationThe Fitnessgram generated ID needs to be

communicated with the classroom teacher(s)It is important that the classroom teacher(s)

and PE teachers coordinate to insure that the correct information is logged for the same student.

The same ID# will be used for the same student for both submissions

Data SubmissionData will be submitted in the Fall and SpringClassroom information to be mailed to

DPI will be entered in one (1) booklet per student

When submitted to DPI, the booklet will not contain names.

Should only have ID# that will be used for both the fall and spring data submissions.

FitnessGram testing data will be submitted electronically to UW
