This year’s stimulating, full-day, Pre-Convention session features international dementia specialist David Sheard, who will demonstrate how emotional memory can be worked with as a reality using the Quality of Interactions Schedule (QUIS), a validated observational assessment tool. The session will include one OANHSS member home who recently used a QUIS audit to inform staff learning, and to begin to look at care planning and possible culture change for the benefit of residents. Consultation Sessions: Strategy for Community Policing

OANHSS recently shared with members an opportunity to participate in province-wide consultations about community-based policing approaches. These consultation sessions, hosted by the Ministry of Community Safety and Correctional Services, give members the opportunity to discuss important issues that could benefit from a community-policing response, including the need to support LTC homes in effectively dealing with challenging families and/or residents, including predatory residents and those with responsive behaviours.

Proposed Amendments to the Excellent Care for All Act

In response to members’ questions about the proposed regulatory changes to the Excellent Care Act for All (ECFAA), OANHSS shared that the changes being proposed are two-fold: to define the meaning of the term “caregiver” and to clarify references to personal health information (PHI). More detail, as made available on Ontario’s Regulatory Registry, is as follows: a) Defining the term “caregiver” in the regulation:

The Ministry is proposing to provide a meaning for the term “caregiver” that would apply to all references in ECFAA and its regulations.

The intent of this is to more clearly define who is able to bring forward a complaint to the Patient Ombudsperson and to ensure consistency of meaning in all of the ECFAA’s legislative and regulatory provisions. Consequently, the proposed definition would apply to all provisions pertaining to caregivers under the ECFAA (e.g. provisions related to surveying caregivers concerning their satisfaction with services provided by a health care organization; engaging caregivers in the development of Quality Improvement Plans for health care organizations;

and requirements related to the patient relations process in health care organizations).

In order to have one consistent definition of caregiver, the Ministry is proposing to remove an existing definition of caregiver that applies only to the patient relations process in hospitals.

b) Clarify references to Personal Health Information (PHI):

The Ministry is seeking to revise the regulation under the ECFAA for the purposes of making clear the specific permissions that Health Quality Ontario has with respect to collecting PHI (essentially, redundant provisions will be removed).

Members will be aware that in 2014, some prior amendments were made to the ECFAA under the Public Sector and MPP Accountability and Transparency Act, 2014. These amendments, which have not yet come into force, were done to establish a Patient Ombudsperson in Ontario. Once these come into force, the Patient Ombudsperson (Christine Elliott) will be able to investigate, facilitate the resolution of, and report on complaints made by patients, former patients and their caregivers that relate to the care experience of the patient or former patient at a hospital, long term care home, or community care access centre. Christine Elliott begins her appointment as the Patient Ombudsperson on July 1, 2016. Individuals with questions may contact Kathryn Pilkington, Director of Professional Services and Health Policy.

I n last month’s CLRI update, we outlined the

CLRI program’s accomplishments over the first

four years of its mandate. As shown in the map

below, we have reached more than 50% of long term

care (LTC) homes across Ontario.

A proposal for the renewal of the program, entitled

CLRI 2.0 The Future of Long-Term Care in Ontario:

An Integrated Program, was submitted earlier this

year to the Ministry of Health and Long-Term Care

for consideration. We anticipate reaching even more

LTC homes in our continued efforts to enhance the

quality of care throughout the sector.

CLRI 2.0 – What’s Next? Proposed Initiatives to Foster and Spread Innovations in Learning, Research and Care

In 2011, the Ministry of Health and Long-Term Care developed the Ontario Centres for Learning, Research

and Innovation (CLRI) in Long-Term Care located at Bruyère,

The CLRI 2.0 proposal was developed in

consultation with a range of long term care

stakeholders including family and resident council

associations, professional associations, academia and

varied knowledge translation partners. The aim was

for the proposal to align with, give voice and address

the needs of the LTC sector.

An environmental scan and stakeholder input

highlighted several challenges facing the sector:

Dementia and responsive behaviours

Mental health issues

Increased complexity of LTC population

These challenges need to be addressed through

enhanced staff knowledge, skills, and training;

recruitment and retention strategies; and further

investments in care, research, education, and

innovation. These will allow the sector to improve

capacity to provide high quality care, to meet

provincial regulations and to improve performance

on key quality indicators.

In CLRI 2.0, we will continue to collaborate with

LTC homes in the adoption and continuous

improvement of evidence-based best practices to

improve the quality of care for all residents.

Below are some examples of initiatives we are

considering pursuing in CLRI 2.0:

A Learning Innovation Network would enable

educators and practitioners to keep up-to-date

with the latest research, to include geriatric

content in curricula, and to spread best

practices to the LTC community.

An enhanced Knowledge Transfer Strategy

would expand existing resources, create a

repository of resources and best practices, and

increase the reach of education programs.

A Research Mobilization Team would support

LTC homes interested in participating in

research (e.g., defining research questions,

protocol development, ethics, data analysis).

Long-Term Care Rapid Reviews would

provide reviews and synthesis of research and

evidence that respond to ministry and

stakeholder inquiries in a timely manner.

A Culture Change Coaching Resource would

support interested homes in shifting toward

social models of living. Specialized units,

polypharmacy guidelines and new approaches

to advanced care planning as well as arts-based

approaches to care will help keep practices

current. Leadership development will continue

to be a priority.

Upcoming Workplace Health & Safety Training Sessions

For Workplace Health & Safety training sessions between March and June, see the Public Services Health & Safety Association website. Training includes Certification Parts One and Two, and Health and Safety for Supervisors and Managers.

Nutrition Screening in Older Adults Should Be a Priority

A systematic review of studies found that community-dwelling older adults who were provided with timely nutrition screening followed up with appropriate intervention and monitoring improved their nutritional status. Older adults are often at risk for malnourishment, and nutrition screening is important to determine risk. Nutrition interventions following screening included dietetics care, nutrition education, and referral to community services such as Meals on Wheels. See the PubMed Abstract for more details.

Yoga May be Beneficial for Seniors

A recent systematic review of studies found that yoga programs resulted in small improvements in balance and medium improvements in physical mobility in people aged 60 years and older. Further research is needed to determine whether yoga-related improvements in balance and mobility lead to prevention of falls. See the PubMed Abstract for more details.

Improving Continuity of Care Reduces ED Visits by LTC Residents

A study in Halifax looked at how a model of coordinated, team-based, primary care in LTC facilities impacted continuity of care and transfers to emergency departments. A “Care by Design” (CBD) model was implemented across 10 LTC homes. This model includes the following key components:

Interdisciplinary team approach Regular team meetings of family physicians, LTC

nurses, care assistants, paramedics, pharmacists, and other staff (e.g., social works, OT, PT, recreational therapists)

A dedicated family physician per floor (about 30 residents/physician)

Weekly onsite visits to see residents identified by nurses as needing primary care

Coordinated 24/7 on-call physician coverage for urgent or emergent issues by a roster of participating CBD physicians

30 minute response time by on-call physicians to calls from LTC homes

On-call sign-out communications, telephone support, and standing orders/protocols

CBD physicians are self-selected participants demonstrating a special interest in LTC and additional training in care of the elderly. Study findings showed statistically significant reductions in ED use: a 34% reduction in 911 calls per month from LTC homes and a 36% reduction in monthly ambulance transports from LTC to ED. In addition, there was greater continuity of care as indicated by the increase in primary care visits and an increase in informational notes between providers after implementation of CBD.
