2. Introduction

Introduction:One of the most exciting innovations in healthcare is in the field of medical simulation. Employing medical simulation techniques can help move medicine from the old “see one, do one, teach one” method to a “see one, practice many, do one” model for success. Simulation is arguably the most prominent innovation in medical education over the past 15 years. The proliferations of simulation-based literature and research have all heralded the promise of simulation as a keystone of health profession education. It is commonly asserted, albeit with limited substantive data, that every hospital and other health professional training institution have either a simulation center, simulation equipment, or a simulation-based educational program.

Due to the importance of simulation in education and training, the Saudi Commission for health specialities(SCHS) represented by the Department of Medical Education and Postgraduate Studies Trainee Division (ME-PGS) and its partner the King Fahad Medical City-Simulation Center (KFMC-SC) have designed a set of simulation courses for essential skills/competencies in selected training programs.

General Objective:The intended general objectives of such courses are to:

• Introduce the concept of simulation to the trainee

• Effectively utilize simulation in training

• Establish a longterm partnership with local simulation centers

• Serve as a baseline database before the ultimate goal of program specific simulation courses cur-riculum which is based on the skills/competencies required for the trainee in his/her program in each training year

Simulation Courses Delivered:1. Adult Airway Management Course for Internal

& Emergency Medicine, Surgery and Anesthesia Residents. It was conducted twice 23-24 January 2013 and 29-30 May 2013

2. Pediatric Airway Management Course for Pediatric Residents, 20-21 March 2013

3. Central Line Insertion Under USS Guidance for Internal & Emergency Medicine, Surgery and Anesthesia Residents. It was conducted twice 20 February 2013 and 22 May 2013.

4. Basic Laparoscopy Course for General Surgery Residents 11-12 May 2013

5. Basic Bronchoscopy Course for Pulmonary Fellows 23 May 2013

Courses fees:The SCHS and Gulf Medical Co. have equally shared the financial support (for three workshops only)* to subsidize the registration fee for Saudi Board Resi-dents to a competitive and affordable amount that can be paid by the resident.

I. Course Design & Strategies:

All courses started with an introductory session, followed by dividing the residents into groups which enables team work. It contains core knowledge and technical skills which was delivered using the following teaching strategies:

• Simulation / clinical scenarios

• Didactic lectures

• Interactive lectures

• Small group discussion

• Feedback sessions

• Debriefing sessions

II. Simulators & Equipments (Hardware)

The simulators used and equipment were updated and properly installed. The clinical scenarios were appropriate and data on the simulators were in line with the clinical scenarios. There was no complaint for equipment malfunction from the residents.

III. Personnel

The instructors were Consultants in their specialties. Those instructors were supported by competent technicians and companies representatives to ensure proper simulators function

IV. Residents’ Overall Courses Evaluation:

The process of the evaluation was based on three techniques:

• Group discussion (face-to-face)

• Pre-test / Post-test

• Completing an evaluation form (closed and open ended questions)

The overall residents’ evaluation of the courses was marked very good. Which is reflected on their marked improvement as determined by the post-test compared to the pre-test. In the first Adult Airway course, most of the residents showed improvement on the second day of the course, and this was partly because the course organizers detected that the weakness in the residents was in their knowledge of basic sciences, therefore they changed the strategy of the course on the second day to address this weakness. This responsive technique by the course organizers to the residents’ knowledge and skills level is definitely one of the strong points of the course providers.

The residents conclude at their evaluation by thanking the SCHS for their support in providing such courses. They showed enthusiasm and motivation to attend future courses and workshops.

Introduction

Anticipating and recognizing respiratory decompensation/failure is only the first step in emergency airway management. Practitioners must be familiar with the indications and techniques for airway intervention and how to anticipate a difficult airway.

The Saudi Commission for Health Specialties (SCHS), in collaboration with King Fahad Medical City (KFMC) are offering an airway management simulation course to give the trainees a chance to learn, practice and demonstrate their ability in applying many airway skills used for resuscitation.

During the course the participants will be provided with a state-of-art systematic overview and hands-on experience with preparation, team work and tools needed to manage common and potentially complex airway situations. The course is endorsed by the American College of Chest Physicians (ACCP).

Learning objectives

Upon the completion of this course the learner will be able to:

Identify upper airway anatomy. Demonstrate proper assessment of airways. Identify all required tools needed for safe

intubation. List the drugs used for pre-intubation with

their proper dosage and side effects. Demonstrate single and double person bag

valve mask ventilation (BVM) techniques. Demonstrate endotrachial intubation

utilizing direct laryngoscope and laryngeal mask airway (LMA).

Demonstrate proper use of LMA.

Introduction

This course teaches fundamental knowledge and technical skills to perform laparoscopic surgery. It includes didactic sessions and hands-on skills training. Cognitive topics range from safe peritoneal access and the physiology of the pneumoperitoneum, to the proper use of electrosurgical energy and troubleshooting laparoscopic equipment. Technical skills taught include two-handed manipulation of laparoscopic instruments, managing endo-loops, and intra/extra corporeal suturing and knot tying.

Learning objectives

Upon completion of this course the learner will be able to:

Describe the instruments and equipment used in laparoscopic surgery

Identify important intraoperative considerations such as anesthesia and patient positioning

Discuss the physiology of the pneumoperitoneum

The course content was adopted from The Methodist Hospital System. Dr. Brian Dunkin, Head, Section of Endoscopic Surgery, Medical Director, Methodist Institute for Technology, Innovation and Education (MITIE), was present during the course. During the course an on-line Fundamentals of laparoscopic Surgery (FLS) exam was offered for the senior and chief residents.

Course objectives:

The objective of this project was to train Pulmonary Fellows in specialized skills using high fidelity medical simulation

Knowledge content:

This was a full day workshop where the Fellow will be introduced to the basics of bronchoscopy including: indication, anatomy, prepara-tion, complications and how to drive the scope. The fellow will have the opportunity to work on a virtual simulator and acquire specific milestones with automatic feedback system and reporting. The ratio of instructor to trainees was 1:4.
