• File active importante de patients africains…..à Paris !
• Réseau de questions/réponses via internet : qualité d’un générique, doses d’EFV en association à RFP, interaction entre ARV et argile, doses pour un patient de 19 ans et 25kg, durée de stabilité d’un flacon pédiatrique ouvert de Viramune, prolongation de la durée de péremption d’un stock de 20000 boites d’un ARV qui périme dans 2 mois….

• Seven batches of study treatment were dispensed. compared with expected doses, median (range) concentrations of the three components were NVP 96% (89–100), d4T 89% (84–96), and 3TC 99% (91–103).• Plasma drug concentrations were available at W2 (n=56), W4 (n=56), and W24 (n=50).• Only 2 patients presented (at both W2 and W4) with NVP, d4T, and 3TC plasma concentrations less than the LOQ.• Adequate NVP plasma concentration (3,400 ng/mL) was reached by 79% (95% CI 66–88) of patients at W2, 93% (83–98) at W4, and 84% (71–93) at W24.• Overall median plasma concentration of NVP was 5,008 ng/mL (range <50–19,122) and those measured 12 h after last drug intake were 16 ng/mL (<10–107) for d4T and 172 ng/mL (12–890) for 3TC.

ng/mL),• Parmi les 77 C24H d’ATV mesurées :
– 35 inexploitables pour intervalle de temps < Tmax (phase d’absorption) nécessitant une modélisation PK plus complexe et Nb d’échantillons plus important pour exploitation PK,
– 42 C24H d’ATV compatibles avec une interprétation,
• Parmi les 42 C24H d’ATV mesurées et interprétables :– 8 C24H d’ATV < 30 ng/mL (7 patients), suggérant :
• soit des difficultés d’observance sur la dernière prise,• soit des problèmes d’absorption intestinale (interaction avec
les modificateurs de pH gastriques type argiles, IPP etc.).
• Parmi les 34 C24H d’ATV mesurées, détectables et interprétables chez 24 patients :
– Médiane (IQR) = 185 ng/mL (96-407),– CV(%) interindividuel = 181%,– 10 patients ont 2 mesures,– Extrêmes : < 30-3999 ng/mL – 16 C24H < 150 ng/mL (47%) – Au total, 57% C24H d’ATV (<150 ng/mL et <30
ng/mL) sont considérées comme inefficaces à S4 et/ou S24 (sans TDF associé)
– C24H Efficace de 150 ng/mL (BMS)– Mentions légales REYATAZ (Etude BMS089)
• C24H = 120 ng/mL • CV 109% (n= 13 patients infectés par VIH)

Predictive factors associated with NVP-R mutations.In univariate analysis, two factors were associated with the identification of NVP-R mutations: the maternal plasma viral load and the plasma NVP concentration. Indeed, the risk for presenting NVP-R mutations was three times higher for each 1 log increase of plasma viral load (odds ratio [OR], 3.1; 95%CI, 1.00–13.28; P 0.02) and for each 100-ng/ml increase of plasma NVP concentration (OR, 1.24; 95% CI, 1.05–1.50; P 0.01In the final multivariate model, these two factors remained associated with the selection of NVP-R mutations: adjusted OR of 3.4 (95% CI, 1.0–12.1; P 0.05) for the plasma viral load at inclusion and adjusted OR of 1.3 (95% CI, 1.1–1.6; P 0.006) for the plasma NVP concentration at day 2.

• A reduction of 25% of d4T doses led to a decrease in AUC and Cmax by 31% and 44%, respectively,
