1 Document History2 ContentDateD o c u m e n t3 Version4 First draft2014/12/15015 Expand description of housing and handling; add anesthesia model derived from relevant6 parts of AQI schema.

2015/01/0402

7 Revised description based on 2015/01/16 t/con discussion and other feedback; improve8 anesthesia medications model and include mixtures; remove biosafety (BSL) information9 and Specific Pathogen Free (SPF) status monitoring since they does not affect interpretation;10 removed animal positioning during acquisition; remove unchanged PS3.3 Annex C sections

2015/03/1403

11 Reorganize by procedure-related phase, factor feeding out of housing, add more phases12 (including induction, preparation), add start/end dates and times for housing (esp. for13 interventions), fill out heating conditions, assign codes, restore BSL information, add detailed14 example.

2015/03/2404

15 Update after WG6 first read, and assigned supplement number 187.2015/03/250516 Add context groups for premed and local anesthetics, more inhalational and injectable17 anesthetics, restore open issue about single value for housing unit air changes etc., add18 depth for bedding, enrichment manufacturer.

19 Draft for WG 6 Review before Public Comment2015/06/030720 Public Comment.2015/06/170821 Post public Comment - add exogenous substances2015/08/020922 Post public Comment - update exogenous substances (include stereotactic coordinates,23 laterality, distinguish role of therapy from model, generalize concept for tumor cell graft to24 just tumor graft), update relationship constraints, add missing PS3.4 stuff, populate SCT25 and UMLS codes in context group tables.

2015/08/0410

26 Add DSC work item number, resolve public comments (none), open issues, to do list.2015/09/011127 Generalize definitions and rename context group related to grafts, since not just cell28 suspension grafted (may be chunks of tissue); add position reference indicator for stereotactic29 coordinates.

2015/09/0112

30 Make animal feeding invocation 1-n to handle supplements and treats; extend feeding mode31 to include restricted diet and treats; add brain and tumor to administration site and intratumor32 to route of administration. Add outline of interconnected templates in lieu of separate figure33 (since it can be hyperlinked and conveys the same information). Update anesethesia example34 to split out different induction methods into separate examples. Checked IOD Value Type35 and Relationship Constraint tables against new templates as well as invoked standard36 templates. Extended list of procedure codes to cover brain. Populated exogenous substance37 context groups. Closed all open issues and removed section. Completed TODO items and38 removed section.

1 Closed Issues2 A Structured Report is used to encode the Acquisition Context, rather than encoding the information in the images in3 new Modules or the generic Acquisition Context Module.

1

4 Existing SR templates in PS3.16 make use of the concept of acquisition context, especially for cardiovascular waveform5 acquisitions, but TID 3530 "Hemodynamic Acquisition Context", for example, is too specific and lacks relevant content6 for reuse in this supplement.

2

7 The acquisition context SR templates are added to Annex A of PS3.16, not Annex C, despite the latter being entitled8 "Acquisition and Protocol Context Templates". Annex A is for Structured Report templates, which these are, as distinct9 from the templates for the Acquisition Context Module's Acquisition Context Sequence (0040,0555) Attribute that are10 defined in Annex C. Accordingly, Annex C is renamed to indicate that it applies only to Acquisition Context Module11 templates.

3

12 Depth is needed for bedding, since weight/volume are not useful unless cage dimensions are also known.413 Do need separate manufacturer for rack/cage bottom/lid. Need product name and code but not serial #. Do not need14 to factor out into common macro. Noted that manufacturers products and names may change over time.

5

15 Coding or defining quantity of enrichment material is currently beyond the state of the art. For now, text description,16 manufacturer and present or not is sufficient.

6

17 It is important to record the presence and type of devices to increase animal activity in cage because this may affect18 metabolism. Coding it is not necessary, and a text description and present or not is sufficient.

7

19 The full set of AQI route types does not need to be added to CID 11. The only specific route that needs to be added for20 small animals is retro-orbital.

8

21 Codes for local anesthetics have not been added, since systemic anesthetics are usually utilized.922 There is a need to model mixtures of anesthetic agents, so re-use the AQI MixtureMedications mechanism in the23 anesthesia record.

10

24 Specific Pathogen Free status monitoring information is not described, because it does not affect interpretation.1125 Animal positioning during acquisition is not included since it will be encoded in the images and there is no need to26 replicate it.

12

27 Feeding and watering are described separately from housing description.1328 WG6: Do not use "Procedure Reported", since we are not producing a "report" for the procedure; use Procedure Code29 instead, which can be inherited from TID 1005 Procedure Context via TID 1001 Observation Context.

14

30 We need not need to include number of passages of a cell line in exogenous substances.1531 A laterality modifier (left or right) is needed for the site of exogenous substance administration.1632 Stereotactic surgical coordinates are need for exogenous substances, e.g., that have been administered to the brain.1733 Species and strain identification is not described (replicated) in the observation context templates; rather it is encoded34 in DICOM Attributes in the top level data set (CP 1478).

18

35 A specific context group for imaging procedures for preclinical small animals is needed and has been populated, since36 the range of concepts needed is small and preclinical acquisition and management systems may not have access to37 an ordering/scheduling system to provide codes via a worklist. LOINC codes are used preferentially when available.

19

38 An IOD and SOP Class specific to the Acquisition Context root templates is required, since a generic SR SOP Class is39 thought not to be sufficient for the use case. The IOD and SOP Class are described as being specific to Acquisition40 Context in general, but not specifically Preclinical Small Animal Acquisition Context, in order to allow reuse for other for41 other purposes that might require acquisition context.

20

42 The SR Document Title (root container node) is specific to Preclinical Small Animal Acquisition Context, rather than43 being a generic Acquisition Context title, which qualifies the use of the generic IOD and SOP Class to be the more44 specific use case.

21

45 No cross-references are defined between the Acquisition Context SR instance and the acquired images, since they will46 likely be produced by different systems; commonality of the Study Instance UID should suffice (and that can be obtained47 via Modality Worklist (MWL)). If there is no MWL, then commonality of Study Date and Study Time may be the best that48 can be achieved (without merging/coercion at post-processing time).

1 Acquisition Context is not modeled as part of Procedure Context per se, since the latter is used mainly for identification,2 not description, of the procedure, and burying the acquisition context detail inside Procedure Context would be confusing.

23

3 The scope of what to encode is largely confined to significant factors that affect imaging outcome (e.g., metabolic effects),4 and which do not replicate imaging technique information that is (or should be) encoded in the images themselves (e.g.,5 reconstruction technique, image processing, timing of one image relative to another, subject positioning in the scanner,6 and information related to contrast and/or radiopharmaceutical injection. Other substances administered that are related7 to the anesthesia and/or other aspects of the procedure (e.g., cardiovascular agents) may be described in the Acquisition8 Context, as may exogenous substances (e.g., injected tumor cells) that are part of the experimental paradigm as opposed9 to being related to the imaging procedure per se.

24

10 With respect to acquisition timing, no special synchronization mechanisms are needed, beyond those already standardized11 in the Synchronization Module.

25

12 All of the subordinate templates that described the acquisition context are optional, since it is expected that different13 experiments will have different requirements for what needs to be populated, and different deployments will have different14 capabilities and priorities with respect to what can and needs to be recorded (by this mechanism). Also, there is no15 prohibition on creating multiple different instances for the same animal and acquisition, e.g., one to describe handling,16 one to describe anesthesia and another to describe exogenous substances (such as tumor cells), particularly since this17 information may be generated by different systems.

26

18 The human (AQI) anesthesia model has been followed fairly closely ... this is thought to be sufficient and appropriate19 for small animals as well as humans and has been reviewed by the working group (WG25). Additional concepts specific20 to routes of administration and other values sets that are used only in animals have been added where necessary.

27

21 The TID 9002 “Medication, Substance, Environmental Exposure” is used as the basis for a specialized TID ttt8222 “Exogenous Substance Administration” template, but not for anesthesia medications, for which the AQI model is used23 as a basis.

28

24 The relatively simple description of Circadian effects is thought to be sufficient without being overly complicated.2925 The short list of coded feed types is thought to be sufficient for now and may be extended later if necessary (CID fed0126 “Animal Feed Types”).

30

27 Limited types of physiological monitoring that were used, are described (ECG and respiration) and are thought to be28 sufficient (see TID ttt70 “Physiological Monitoring Performed During Procedure”)? If monitoring is recorded and included29 with the study in DICOM form, it is encoded elsewhere (e.g., in waveform instances).

31

30 As is common practice with other generic templates, medication-related units have not been more constrained and are31 left to the discretion of the implementer or user (e.g., as per AQI CommonUnit type, or specific to each invocation, e.g.,32 concentration, volume).

32

33 Covariates that require measurement (such as blood glucose measurements for PET) are not included since other34 mechanisms are available and intended to be reused per human imaging practice (e.g., the existing Acquisition Context35 templates intended for encoding in the images, such as TID 3471 “PET Covariates Acquisition Context”).

33

36 There is no need to record total number or rows and columns in cage rack, nor which housing units are populated versus37 empty?

34

38 There is no need to record explicitly that transport cage is the same as home cage (+/- different lid).3539 There is no need to describe more about the imaging carrier/sled/chamber, such as whether it is completely enclosed40 or not, whether there is a bite bar, presence or absence or number of stereotactic fixation points, presence or absence41 of limb ties, or the presence or absence of fiducial markers.

36

42 There is no need for a more sophisticated model of biological sex for animals and handlers than m/f/mixed, e.g., for43 transsexual handlers or animal experiments on such things.

37

44 The Classification code for exogenous substances is left unconstrained, and specific new content items have been45 added for tissue and taxonomic rank (species) of origin, since the single concept of Classification was not sufficient46 (indeed its intent in the original TID 9002 “Medication, Substance, Environmental Exposure” from which it is inherited47 remains mysterious).

38

48 A single value for housing unit air changes, average temperature, humidity and airflow is sufficient; based on feedback49 received, a range is not needed, since even though a range may be specified in procedures and requirements, for an50 actual instance, a nominal value (normally within the procedurally required range) can be recorded.

1 Scope and Field of Application2 This Supplement defines use-cases and templates for storage of information related to acquisition of small animal images during3 preclinical research.

4 Acquisition Context is a "description of the conditions present during data acquisition" (quoted from the text of PS3.3 Section C.7.6.145 Acquisition Context Module).

6 Traditionally, all attributes that describe an image acquisition are defined as specific Attributes (Data Elements) in a modality-specific7 IOD. This approach works extremely well when image objects have very specific applications and when the domain from which the8 descriptive concepts are drawn is well understood and clearly defined. When the visible light image object family was introduced, an9 alternative and more general mechanism was added to the standard, using lists of coded name-value pairs to specify acquisition10 content information, rather than depending on SOP-class-specific sets of traditional Attributes. The name-value pairs are encoded in11 the Acquisition Context Module. Using coded entries has benefits and disadvantages. One of the advantages is the ability to use12 coding schemes defined by other organizations to define names and value sets for concepts. The "acquisition context" idea was in-13 troduced to DICOM by Dean Bidgood and is well described in his article on the subject: Bidgood WD, Bray B, Brown N, Mori AR,14 Spackman KA, Golichowski A, et al. Image Acquisition Context: Procedure Description Attributes for Clinically Relevant Indexing and15 Selective Retrieval of Biomedical Images. Journal of the American Medical Informatics Association : JAMIA. 1998 Aug 17;6(1):61–75.16 available at http://www.ncbi.nlm.nih.gov/pubmed/9925229.

17 Both approaches assume that the Acquisition Context is known prior to encoding the image in DICOM, and that it is known to the18 device encoding the DICOM image (the modality). In some cases, information about the acquisition that is relevant to the interpretation19 of the imaging may not be known to the modality. This is particularly the case in small animal preclinical research, where a myriad of20 factors that affect quantitative analysis need to be recorded, which would be overwhelming if required to be captured at the modality21 console user interface. Though it would be possible to add this information by post-processing of the acquired images, WG 30 has22 concluded that it is preferable to encode the Acquisition Context in a separate standalone composite instance.

23 The type of information to be recorded needs to be in a structured form, and many of the concepts are expected to be available in24 external lexicons. This is particularly the case for descriptions of experimental conditions, animal physiology and animal handling.25 Accordingly, the DICOM Structured Report (SR) mechanism is a natural choice of encoding. The Acquisition Context Module in the26 images will remain unpopulated.

27 The Acquisition Context SR instances will usually be in the same Study as the acquired images.

28 This supplement defines a new DICOM SR root template and supporting templates, as well as an IOD and SOP Class specific to that29 template to facilitate interoperability.

19 TID ttt01 Preclinical Small Animal Image Acquisition Context20 This root template encodes a description of the conditions present during and related to data acquisition for a single imaging procedure.

21 Note

22 1. It is not expected that a single instance be used to describe the entire life of an animal, unless it is sacrificed after a23 single procedure. Rather, separate instances will be used for separate procedures, though there may be some duplication24 of common information, such as about the home cage environment.

25 2. It is expected that an SR instance encoded using this template will be contained in the same Study as other instances26 created during the procedure, e.g. with a common Study Instance UID. If this is not practical, e.g., due to recording on27 a separate device without use of a shared Modality Worklist, then commonality of other Study level attributes may be28 necessary to link procedures (and possibly coerce the Study Instance UID to a common value).

20 A single pre-coordinated code describing the general type of imaging procedure can be described using TID21 1005 Row 9 Procedure Code (included in TID 1001). For small animal (as opposed to human) imaging, this will22 likely describe a whole body acquisition in a modality-specific manner and the use of contrast and/or23 radiopharmaceutical. E.g., whole body FDG PET, or whole body DCE-MRI.

24 May be redundant with (or default to) the value present in the top level data set in Procedure Code Sequence25 (0008,1032) of the General Study Module.

26 Species and strain identification is not described in TID 1001; rather it is encoded in DICOM Attributes in the top27 level data set.

Row 3

28 The biosafety conditions are expected to be consistent across all phases of handling, so are not described29 separately per-phase.

Row 5

30 The period of time during which the phase is defined, i.e., during which the animal was managed in the specified31 conditions. This may be more important for interpretation for some phases (e.g., transport) than others (e.g., at32 rest in the home cage), and hence is optional.

Rows 8-9

33 The outline of subordinate templates follows the pattern of categories of Animal Housing, Care, and Physiologic34 Monitoring information described in [Stout et al 2013].

Rows 10-14

35 Animal feeding is 1-n to allow encoding of dietary supplements and treats in addition to the regular diet.Row 1136 A single anesthesia event is normally assumed for a single procedure, though the template included can include37 multiple pre-, intra- and post-procedure descriptions.

Row 15

38 Used to describe pharmaceuticals administered that are not described elsewhere, in particular, those that are39 not described as anesthesia medications, and those that are not described in the images (e.g., contrast,40 radiopharmaceuticals). This includes therapy (such as chemotherapy, immunotherapy) and similar interventions41 that may be the subject of the research.

42 The value set of $CodeValue is not defined, given the vast range of possible codes and coding schemes for43 drugs or medicaments that might be used. Nor are value sets for $Classification or $Site defined.

38 If this template is used, at least one description of anesthesia method is required. Note that the specific39 agents used are described separately, as intra-operative medications, per the AQI model.

Rows 1-3

40 These correspond to AQI elements that are named "Time" rather than "DateTime", though their value is a41 DateTime; the DICOM naming convention is used here.

Rows 6-7

42 Only inhalational methods of maintenance are included in this row. Absence of this row implies that the43 (non-inhalational) induction method is used for maintenance.

Row 9

44 The comment corresponds to AQI element "Anesthesia Notes".Row 1045 At least one description of airway management is required.

46 The airway management method also serves as the description of the method of inhalational anesthesia47 delivery, even if it does not involve "management" of the "airway" per se (e.g., delivery via nose cone).

1 In the AQI model, a single AQI MedicationsSet is used in the AQI IntraOp element to describe intra-operative2 medications.

3 This template allows a more general usage, with one or more Medications Set containers, each of which4 may be qualified by the phase of the procedure (pre-operative, intra-operative or post-operative). The purpose5 of the medication (e.g., general anesthetic) is described in the (111516, DCM, "Medication Type") of the6 included TID ttt31 “Medications and Mixture Medications”.

2 AQI Medication type and element correspond to (F-04460, SRT, "Medication given") (situation). (See TID 38063 Cath Procedure).

Row 1

4 AQI DoseStart and DoseEnd elements correspond to (122081, DCM, "Drug start") and (122082, DCM, "Drug5 end") respectively. (See CID 3409 Administration of Drugs/Contrast). If the medication is delivered as a bolus,6 the end time is omitted.

Rows 2-3

7 AQI MedicationRoute corresponds to (G-C340, SRT, "Route of administration"). The existing CID 11 “Route of8 Administration” contains a relevant subset of concepts for the enumerated values of AQI9 MedicationRouteCodeType.

Row 4

10 The AQI schema allows the Medication type not only to describe medications with a single component, but also11 to add MixtureMedications children, each of which is encoded following a similar pattern to the contents of12 Medication, though the start and end time and route of administration are shared. This had been modeled by13 allowing every medication to have one or more mixture children. For medications that are not a mixture, a single14 instance of this row defines the medication (even though the mixture container is still used).

Row 5

15 AQI MedicationName and MixtureMedicationName elements correspond to (122083, DCM, "Drug administered").16 (See TID 3806 Cath Procedure). The medication (e.g., anesthesia agent) can be described with a code or text,17 e.g., (F-61B0A, SRT, "Isoflurane") or "isoflurane".

Rows 6, 7

18 Both AQI MedDose (or MixtureMedDose) and DoseUnits (or MixtureDoseUnits) elements are combined in one19 content item. Units are required to be encoded as UCUM but are not otherwise constrained.

Row 9

20 Both AQI MedConcentration (or MixtureMedConcentration) and MedConcentrationUnit (or21 MixtureMedConcentrationUnit) elements are combined in one content item. Units are required to be encoded22 as UCUM but are not otherwise constrained.

17 No context group is provided for the value set, but it is recommended that values from a standard18 external coding scheme, such as SRT or NDC, be used. Classification is SCT:278201002,19 UMLS:C0008902, and is completely generic; i.e., it does not specifically refer to any particular20 type of classification.

Row 3 "Classification"

21 If both of these content items are instantiated, the concept names selected for each should match. For22 example, use "Relative dose amount" as the concept name for row 13 with "Relative dose frequency"23 as the concept name for row 14.

1 Note

2 (C0425422, UMLS, "Restricted diet") corresponds to the inactive SNOMED concept of "Dietary restriction NOS"; SNOMED3 currently does not seem to have an active generic concept of a restricted diet, as opposed to many specific types of restricted4 diet. In this context, the intent is to convey that the diet is controlled and restricted to finite quantities (e.g., as opposed to ad5 libitum) without requiring a detailed classification of what components are restricted.

16 • In this context group, SNOMED substance codes are used in preference to product codes, since there is no need to refer17 to specific products or preparations. SNOMED codes are used in preference to other potential sources of pharmaceutical18 related codes, such as from the National Drug Code (NDC) directory.

19 • For Lidocaine + Prilocaine, since it is a mixture of two substances, the code for the product concept is used. The code for20 a mixture of unspecified type is used, rather than a more specific code, e.g., for the so-called "Eutectic Mixture of Local21 Anesthetics (EMLA)", which consists of Lidocaine + Prilocaine. UMLS contains three distinct concepts, (C0059079, UMSL,22 "EMLA"), (C0617623, UMLS, "Lidocaine/Prilocaine") and (C0950567, UMLS, "Eutectic Lidocaine-Prilocaine").

The non-tumor specific concept (G-C1F9, SRT,11 "Graft material") may be used when the graft is not a tumor (though strictly speaking, it is a SNOMED attribute rather than12 substance; UMLS:C0181074 does not make such a distinction).

37 Note

38 1. The inconsistent pattern of modality and anatomy in the code meaning is present in the original coding scheme (e.g.,39 "Whole body CT" versus "PET whole body"), and not changed, except where necessary (e.g., LN:42175-0 is actually40 just "scan of whole body" in the original, which is insufficient, so "radionuclide" has been added).

41 2. The SNOMED codes for SPECT (+/- CT) of whole body are from the UK National Extension and are not present in the42 current SNOMED International release.

1 3. The UMLS codes that map to the SNOMED concepts, when present, are shown, in the cases when UMLS is lacking a2 mapping between the LOINC and SNOMED codes. E.g., (44138-6, LN, "Brain PET") maps directly to (C1715408, UMLS,3 "Multisection:Find:Pt:Brain:Doc:Radnuc.PET"), but (241434002, SCT, "PET Brain Study") maps to (C0412493, UMLS,4 "PET Brain Study"), which is used instead. In general, UMLS does not unify the mappings from LOINC and SNOMED,5 presumably due to the lexical dissimilarity of the terms (i.e., the LOINC mapping seems to be based on the fully-specified6 name rather than the long common name).

5 Amend DICOM PS3.17 - Explanatory Information to add new Annex XXX:

6 XXXX.1 7 This Annex describes the use of Preclinical Small Animal Imaging Acquisition Context.

8 XXXX.1.1 Example of housing and anesthesia for PET-CT

9 This Section contains examples for use cases involving imaging of a single animal in a hybrid PET-CT system.

10 The basic use case involves an animal, which:

11 • lives in an individually ventilated home cage with several other animals in the same cage

12 • is (briefly) transported (in its home cage) with its cage mates to the imaging facility, without heating, with an appropriate lid

13 • is removed from its home/transport cage for preparation for imaging, involving insertion of a tail vein cannula, performed on an14 electrically heated pad

15 • is induced by (a) placement in an induction chamber with more concentrated volatile anesthetic, or (b) intraperitoneal injection of16 Ketamine mixture

17 • is placed in a PET-CT compatible imaging sled/carrier/chamber for imaging (of one animal at a time), with anesthesia with Isoflurane18 and Oxygen as the carrier gas, and heated with an electric pad regulated by feedback from a rectal probe

19 • is removed for recovery in a separate cage

20 The content tree structure (when induction is by a volatile anesthetic) would resemble:
