Proceed southwesterly out of Myton, Utah along Highway 40 - 1.4 miles ± to the junctionof thishighway and UT State Hwy 53; proceed southwesterly - 6.6 miles ± to it's junctionwith anexisting road to the east; proceed in an easterly direction - 0.6 miles ± to it's junctionwith thebeginning of the proposed access road; continue in a northerly direction along the proposed accessroad - 500' ± to the proposed well location.

Newfield Production Company requests 500' of disturbed area be granted in Lease UTU-69060 toallow for construction of the planned access road. Refer to Topographic Map "B". Apermanent width of 30' and a running surface of 18' is proposed for the planned access road. Theconstruction phase of the planned access road will last approximately (5) days. The plannedaccess road will be an 18' crown road (9' either side of the centerline) with drainage ditches alongeither side of the proposed road whether it is deemed necessary in order to handle any run-off fromnormal meteorological conditions that are prevalent to this area. The maximum grade will be lessthan 8%. There will be no culverts required along this access road. There will be barrow ditchesand turnouts as needed along this road. There are no fences encountered along this proposed road.
There will be no new gates or cattle guards required. All construction material for this access road
will be borrowed material accumulated during construction of the access road.
Newfield Production Company requests 500' of disturbed area be granted in Lease UTU-69060 toallow for construction of the proposed surface gas lines. It is proposed that the disturbed area will
temporarily be 50' wide to allow for construction of a 10" or smaller gas gathering line, and a 3"poly fuel gas line, with a permanent width of 30' upon completion of the proposed gas lines. Theconstruction phase of the proposed gas lines will last approximately (5) days. Both lines will tie in
to the existing pipeline infrastructure. Refer to Topographic Map "C." For a ROW plan ofdevelopment, please refer to the Monument Butte Field SOP.

Water DisposalImmediately upon first production, all produced water will be confined to a steel storage tank. Ifthe production water meets quality guidelines, it is transported to the Ashley, Monument Butte,Jonah, and Beluga water injection facilities by company or contract trucks. Subsequently, theproduced water is injected into approved Class II wells to enhance Newfield's secondary recovery
project.
Water not meeting quality criteria, is disposed at Newfield's Pariette #4 disposal well (Sec. 7, T9SR19E), State of Utah approved surface disposal facilities, or Federally approved surface disposalfacilities.
Threatened, Endangered, And Other Sensitive SpeciesMountain Plover: If new construction or surface disturbing activities are scheduled to occurbetween May 15 and June 15, detailed surveys of the area within 0.5 mile of the proposed locationand within 300 feet of proposed access routes must be conducted to detect the presence of mountainplovers. All surveys must be conducted in accordance with the survey protocols outlined in the mostrecent USFWS Survey Protocol. Surveys must be completed prior to initiating new construction orsurface disturbing activities. No new construction or surface disturbing activities will be allowedbetween May 15 and June 15 within a 0.5 mile radius of any documentedmountain plover nest site.
Burrowing Owl: Due to the proximity of the location to active prairie dog towns, there is thepotential to encounter nesting burrowingowls between April 1 and August 15. If new construction or
surface disturbing activities are scheduled between April l and August 15, pre-construction surveyswill be conducted to detect the presence of nesting burrowing owls within 0.5 mile of any new
construction or surface disturbing activity (see Vernal BLM Field Office Protocol). No newconstruction or surface disturbing activities will be allowed between April 1 and August 15 within a0.5 mile radius of any active burrowingowl nest.
Reserve Pit LinerA 16 mil liner with felt is required. Please refer to the Monument Butte Field SOP.

We would like toemphasize thatthis is not an on-going issue and thatwe have drilled or plan to drill95% of theAPD's we've applied for in recent years. These were some areas and play typesthatwe wereassessing that we had to permit in advance due to long permit lag time -- thiscaused us to"over-permit" these areas/plays so that we would have options to drill if successful.

All lease and/or unit operations are to be conducted in such a manner that full compliance is made with theapplicable laws, regulations (43 CFR Part 3160), and this approved Application for Permit to Drill includingSurface and Downhole Conditions of Approval. The operator is considered fully responsible for the actionsof his subcontractors. A copy of the approved APD must be on location during construction, drilling, andcompletion operations. This permit is approved for a two (2) year period, or until lease expiration,whichever occurs first. An additional extension, up to two (2) years, may be applied for by sundrynotice prior to expiration.

SURFACE USE PROGRAMCONDITIONS OF APPROVAL (COAs)
• All new and replacement internal combustion gas field engines of less than or equal to 300 design-rated horsepower must not emit more than 2 gms of NOx per horsepower-hour. This requirementdoes not apply to gas field engines of less than or equal to 40 design-rated horsepower.
• All and replacement internal combustion gas field engines of greater than 300 design ratedhorsepower must not omitmore than 1.0 gms of NOx per horsepower-hour.
• If there is an active Gilsonite mining operation within 2 miles of the well location, operator shallnotify the Gilsonite operator at least 48 hours prior to any blasting during construction.
• If paleontological materials are uncovered during construction, the operator is to immediately stopwork and contact the Authorized Officer (AO). A determination will be made by the AO as to whatmitigation may be necessary for the discovered paleontologic material before construction cancontinue· RECEIVED

CONDITIONS OF APPROVAL: DIV.0FOlL,GAS&MINING
• Construction and drilling is not allowed from May 1 - June 15 to minimize impacts during mountain plovernesting.
• Construction and drilling is not allowed from March 1 - August 31 to minimize impacts during burrowingowl nesting.
• If it is anticipated that construction or drilling will occur during any of the given timing restrictions, a BLM orqualified biologist shall be notified so surveys can be conducted. Depending upon the results of the surveys,permission to proceed may or may not be granted by the BLM Authorized Officer.
Reclamation
Interim Reclamation:
Interim reclamation of the surface environment shall take place after drilling and completion and the well isplaced into production. At a minimum, this will include the stabilization of the soil surface during construction,re-contouring of the unused pad area, and the reserve pit to the original contour to the extent possible, and the re-
spreading of the top soil after re-contouring takes place, up to the rig anchor points and any area not needed forproduction. Buried pipelines will be re-contoured, the salvaged topsoil redistributed and reseeded. The outsideborrow of the access road will have the top soil salvaged and re-spread and reseeded. The area will be reseeded
using appropriate seeding methods, which includes, but is not limited to, the drill seeding method (see below).Any adjustments, due to site specific production concerns, to the reclamation COAs in this document can besubmitted on Sundry form 3160-5.
The reclamation objective is to establish a desired self-perpetuating diverse plant community. The objective is toattain 75% basal cover based on similar undisturbed adjacent native vegetative community, and comprised

desired species and/or seeded species within 5 years of initial reclamation action. However if after three (3)growing seasons there is less than 30% of the basal cover based on similar undisturbed native vegetativecommunity, then the Authorized Officer may require additional seeding efforts.
Topsoil will be segregated from the subsoil (without mixing them), stockpiled separately from other soilmaterials, and maintained for future use in rehabilitating the locations.
• Topsoil piles stored beyond one growing season will be stabilized and seeded to prevent erosion.
• Topsoil storage areas will be identified with appropriate signage, so it is easily identifiable where topsoil is onlocation.
• Noxious and invasive weeds will be inventoried and monitored. Noxious and invasive weeds will be controlledthrough integrated weed management including chemical, mechanical, and biological control methods. Anapproved Pesticide Use Proposal (PUP) is required for all herbicide applications.
• All trash and waste materials not needed for production will be removed immediately.
• Earth work for interim and final reclamation must be completed within 6 months following well completion orwell plugging (weather permitting).
• Salvaged topsoil will be re-distributed evenly, and to pre-disturbance depths, over the surfaces to be revegetated.
• The soil surface will be prepared to provide a seedbed for re-establishment of desirable vegetation. Sitepreparation may include gouging, scarifying, dozer track-walking, mulching, hydro mulching, contour
furrowing, water terracing, fertilizing, disking, soil additives or other approved methods. The seedbedpreparations will be determined to accomplish the reclamation objectives above.
• The seeding contractor will provide all seed tags to the appropriate SMA prior to seeding efforts.
• Seeding will occur after August 15 and prior to winter freezing of the soil.
• Drill seeding will be used except in areas where topography or substrate composition (rock) precludes the use ofthe drill. If drill seeding is not possible, broadcast seeding will be implemented. If the broadcast method is used(such as on slopes of 40 percent or greater), the seed rates established for drill seeding will be doubled and seedwill be immediately covered to prevent seed desiccation or predation by birds or rodents. The seeds can becovered in several ways including spreading and crimping straw over the seeded area, raking the area by hand, ordragging a chain or chain-linked fence over the seeded area.
• Any altered drainages,or water courses will be stabilized through rip rap, berming, seeding, matting, or otherapproved methods, and will be reconstructed to have the characteristics and features of nearby functionaldrainages. Any disturbance to "dashed line" drainages as depicted on USGS 1:24,000 topographic maps will becoordinated with the corps of engineers prior to disturbance.
Final Reclamation:
Final Reclamation will be similar to interim reclamation except, all disturbance associated with the well location,access road and pipeline will be reclaimed. The seed mix may include more shrubs and native species. The visualcomposition will be reestablished to blend with the natural surroundings through re-contouring and reseeding.

• After pipeline installation is complete, all disturbed areas will be reseeded. The seed mixtures to be used will besimilar to the vegetation of the surrounding areas and may consist of grasses, forbs, or shrubs. A recommendedseed mixture is provided in the table below.
Monitoring:
• The initial monitoring report will include but is not limited to: when the re-contouring and seeding wascompleted. The seed applied, the amounts, and the depth of application. Seedbed preparation such as disking ,
mulching, etc. Seed application such as drilling or broadcast seeding. Are there any soil amendments,fertilizers, watering, etc. The amounts of noxious and invasive weeds. Are any fencing requirements needed.
• Prior to any surface disturbance, vegetative monitoring locations and reference sites will be identified by theoperator and approved by the BLM Authorized Officer.
• Vegetation monitoring protocol will be developed by the operator and approved by the BLM Authorized Officerprior to implementation of re-vegetation techniques and will be designed to monitor % basal vegetative cover.
• Reclamation areas will be inspected annually and monitored to document location and extent of areas withsuccessful re-vegetation. Monitoring will continue until the site is considered a success by the authorized officerand the objectives established in the Green River District Guidelines. A reclamation report will be submittedelectronically to the Authorized Officer by March 31 of each year.
• On Federal lands, the reclamation objective will be a vegetation community that within 5 years is comprised ofdesired and/or seeded species, where the basal vegetative cover is 75 percent of a similar undisturbed adjacentnative vegetation community. If after 3 years basal cover is less than 30 percent, then additional seeding andreclamation efforts may be required.

DOWNHOLE PROGRAMCONDITIONS OF APPROVAL (COAs)
SITE SPECIFIC DOWNHOLE COAs:
. Newfield Production Co. shall adhere to all referenced requirements in the SOP (version: June 24,2008) ) along with all Oil and Gas rules and requirements listed in the Code of Federal Regulationsand all Federal Onshore Oil and Gas Orders.
All provisions outlined in Onshore Oil & Gas Order #2 Drilling Operations shall be strictly adheredto. The following items are emphasized:
DRILLING/COMPLETION/PRODUCING OPERATING STANDARDS
• The spud date and time shall be reported orally to Vernal Field Office within 24 hours of spudding.
• Notify Vernal Field Office Supervisory Petroleum Engineering Technician at least 24 hours inadvance of casing cementing operations and BOPE & casing pressure tests.
• All requirements listed in Onshore Order #2 III. E..Special Drilling Operations are applicable for airdrilling of surface hole.
• Blowout prevention equipment (BOPE) shall remain in use until the well is completed or abandoned.Closing unit controls shall remain unobstructed and readily accessible at all times. Choke manifoldsshall be located outside of the rig substructure.
• All BOPE components shall be inspected.daily and those inspections shall be recorded in the dailydrilling report. Components shall be operated and tested as required by Onshore Oil & Gas OrderNo. 2 to insure good mechanical working order. All BOPE pressure tests shall be performed by atest pump with a chart recorder and NOT by the rig pumps. Test shall be reported in the driller'slog.
• BOP drills shall be initially conducted by each drilling crew within 24 hours of drilling out fromunder the surface casing and weekly thereafter as specified in Onshore Oil & Gas Order No. 2.
• Casing pressure tests are required before drilling out from under all casing strings set and cementedin place.
• No aggressive/fresh hard-banded drill pipe shall be used within casing.
• Cement baskets shall not be run on surface casing.
• The operator must report all shows of water or water-bearing sands to the BLM. If flowing water isencountered it must be sampled, analyzed, and a copy of the analyses submitted to the BLM VernalField Office. RECEIVED

• The operator must report encounters of all non oil & gas mineral resources (such as Gilsonite, tarsands, oil shale, trona, etc.) to the Vernal Field Office, in writing, within 5 working days of eachencounter. Each report shall include the well name/number, well location, date and depth (from KBor GL) of encounter, vertical footage of the encounter and, the name of the person making the report(along with a telephone number) should the BLM need to obtain additional information.
• A complete set of angular deviation and directional surveys of a directional well will be submitted tothe Vernal BLM office engineer within 30 days of the completion of the well.
• While actively drilling, chronologic drilling progress reports shall be filed directly with the BLM,Vernal Field Office on a weekly basis in sundry, letter format or e-mail to the Petroleum Engineersuntil the well is completed.
• A cement bond log (CBL) will be run from the production casing shoe to the top of cement and shallbe utilized to determine the bond quality for the production casing. Submit a field copy of the CBLto this office.
• Please submit an electronic copy of all other logs run on this well in LAS format [email protected]. This submission will supersede the requirement for submittal ofpaper logs to the BLM.
• There shall be no deviation from the proposed drilling, completion, and/or workover program asapproved. Safe drilling and operating practices must be observed. Any changes in operation musthave prior approval from the BLM Vernal Field Office.

OPERATING REQUIREMENT REMINDERS:
• All wells, whether drilling, producing, suspended, or abandoned, shall be identified in accordancewith 43 CFR 3162.6. There shall be a sign or marker with the name of the operator, lease serialnumber, well number, and surveyed description of the well.
• In accordance with 43 CFR 3162.4-3, this well shall be reported on the "Monthly Report ofOperations" (Oil and Gas Operations Report ((OGOR)) starting with the month in which operationscommence and continue each month until the well is physically plugged and abandoned. This reportshall be filed in duplicate, directly with the Minerals Management Service, P.O. Box 17110, Denver,Colorado 80217-0110, or call 1-800-525-7922 (303) 231-3650 for reporting information.
• Should the well be successfully completed for production, the BLM Vernal Field office must benotified when it is placed in a producing status. Such notification will be by written communicationand must be received in this office by not later than the fifth business day following the date onwhich the well is placed on production. The notification shall provide, as a minimum, the followinginformational items:
o Operator name, address, and telephone number.
o Well name and number.
o Well location (¼¼,Sec., Twn, Rng, and P.M.).
o Date well was placed in a producing status (date of first production for which royalty will bepaid).
o The nature of the well's production, (i.e., crude oil, or crude oil and casing head gas, ornatural gas and entrained liquid hydrocarbons).
o The Federal or Indian lease prefix and number on which the well is located; otherwise thenon-Federal or non-Indian land category, i.e., State or private.
o Unit agreement and/or participating area name and number, if applicable.
o Communitization agreement number, if applicable.
• Any venting or flaring of gas shall be done in accordance with Notice to Lessees (NTL) 4A andneeds prior approval from the BLM Vernal Field Office.
• All undesirable events (fires, accidents, blowouts, spills, discharges) as specified in NTL 3A will bereported to the BLM, Vernal Field Office. Major events, as defined in NTL3A, shall be reportedverbally within 24 hours, followed by a written report within 15 days. "Other than Major Events"will be reported in writing within 15 days. "Minor Events" will be reported on the Monthly Reportof Operations and Production. RECElVED

• Whether the well is completed as a dry hole or as a producer, "Well Completion and RecompletionReport and Log" (BLM Form 3160-4) shall be submitted not later than 30 days after completion ofthe well or after completion of operations being performed, in accordance with 43 CFR 3162.4-1.
Two copies of all logs run, core descriptions, and all other surveys or data obtained and compiledduring the drilling, workover, and/or completion operations, shall be filed on BLM Form 3160-4.
Submit with the well completion report a geologic report including, at a minimum, formation tops,and a summary and conclusions. Also include deviation surveys, sample descriptions, strip logs,core data, drill stem test data, and results of production tests if performed. Samples (cuttings, fluid,and/or gas) shall be submitted only when requested by the BLM, Vernal Field Office.
• All off-lease storage, off-lease measurement, or commingling on-lease or off-lease, shall have priorwritten approval from the BLM Vernal Field Office.
• Oil and gas meters shall be calibrated in place prior to any deliveries. The BLM Vernal Field OfficePetroleum Engineers will be provided with a date and time for the initial meter calibration and allfuture meter proving schedules. A copy of the meter calibration reports shall be submitted to theBLM Vernal Field Office. All measurement facilities will conform to the API standards for liquidhydrocarbons and the AGA standards for natural gas measurement. All measurement points shall beidentified as the point of sale or allocation for royalty purposes.
• A schematic facilities diagram as required by Onshore Oil & Gas Order No. 3 shall be submitted tothe BLM Vernal Field Office within 30 days of installation or first production, whichever occursfirst. All site security regulations as specified in Onshore Oil & Gas Order No. 3 shall be adhered to.All product lines entering and leaving hydrocarbon storage tanks will be effectively sealed inaccordance with Onshore Oil & Gas Order No. 3.
• Any additional construction, reconstruction, or alterations of facilities, including roads, gatheringlines, batteries, etc., which will result in the disturbance of new ground, shall require the filing of asuitable plan and need prior approval of the BLM Vernal Field Office. Emergency approval may beobtained orally, but such approval does not waive the written report requirement.
• No location shall be constructed or moved, no well shall be plugged, and no drilling or workoverequipment shall be removed from a well to be placed in a suspended status without prior approval ofthe BLM Vernal Field Office. If operations are to be suspended for more than 30 days, priorapproval of the BLM Vernal Field Office shall be obtained and notification given before resumptionof operations.
• Pursuant to Onshore Oil & Gas Order No. 7, this is authorization for pit disposal of water producedfrom this well for a period of 90 days from the date of initial production. A permanent disposalmethod must be approved by this office and in operation prior to the end of this 90-day period. Inorder to meet this deadline, an application for the proposed permanent disposal method shall besubmitted along with any necessary water analyses, as soon as possible, but no later than 45 days -
after the date of first production. Any method of disposal which has not been approved prior to theend of the authorized 90-day period will be considered as an Incident of Noncompliance and will begrounds for issuing a shut-in order until an acceptable manner for disposing of said water is providedand approved by this

• Unless the plugging is to take place immediately upon receipt of oral approval, the Field OfficePetroleum Engineers must be notified at least 24 hours in advance of the plugging of the well, inorder that a representative may witness plugging operations. If a well is suspended or abandoned, allpits must be fenced immediately until they are backfilled. The "Subsequent Report ofAbandonment" (Form BLM 3160-5) must be submitted within 30 days after the actual plugging ofthe well bore, showing location of plugs, amount of cement in each, and amount of casing left inhole, and the current status of the surface restoration.

All lease and/or unit operations are to be conducted in such a manner that full compliance is made with theapplicable laws, regulations (43 CFR Part 3160), and this approved Application for Permit to Drill includingSurface and Downhole Conditions of Approval. SURFACE USE PROGRAMCONDITIONS OF APPROVAL (COAs)
• All new and replacement internal combustion gas field engines of less than or equal to 300 design-rated horsepower must not emit more than 2 gms of NOx per horsepower-hour. This requirementdoes not apply to gas field engines of less than or equal to 40 design-rated horsepower.
• All and replacement internal combustion gas field engines of greater than 300 design ratedhorsepower must not omitmore than 1.0 gms of NOx per horsepower-hour.
• If there is an active Gilsonite mining operation within 2 miles of the well location, operator shallnotify the Gilsonite operator at least 48 hours prior to any blasting during construction.
• If paleontological materials are uncovered during construction, the operator is to immediately stopwork and contact the Authorized Officer (AO). • Construction and drilling is not allowed from May 1 - June 15 to minimize impacts during mountain plovernesting.
• Construction and drilling is not allowed from March 1 - August 31 to minimize impacts during burrowingowl nesting.
• If it is anticipated that construction or drilling will occur during any of the given timing restrictions, a BLM orqualified biologist shall be notified so surveys can be conducted. Depending upon the results of the surveys,permission to proceed may or may not be granted by the BLM Authorized Officer.
Reclamation
Interim Reclamation:
Interim reclamation of the surface environment shall take place after drilling and completion and the well isplaced into production. At a minimum, this will include the stabilization of the soil surface during construction,re-contouring of the unused pad area, and the reserve pit to the original contour to the extent possible, and the re-
spreading of the top soil after re-contouring takes place, up to the rig anchor points and any area not needed forproduction. Buried pipelines will be re-contoured, the salvaged topsoil redistributed and reseeded. The outsideborrow of the access road will have the top soil salvaged and re-spread and reseeded. The area will be reseeded
using appropriate seeding methods, which includes, but is not limited to, the drill seeding method (see below).Any adjustments, due to site specific production concerns, to the reclamation COAs in this document can besubmitted on Sundry form 3160-5.
The reclamation objective is to establish a desired self-perpetuating diverse plant community. The objective is toattain 75% basal cover based on similar undisturbed adjacent native vegetative community, and comprised

desired species and/or seeded species within 5 years of initial reclamation action. However if after three (3)growing seasons there is less than 30% of the basal cover based on similar undisturbed native vegetativecommunity, then the Authorized Officer may require additional seeding efforts.
Topsoil will be segregated from the subsoil (without mixing them), stockpiled separately from other soilmaterials, and maintained for future use in rehabilitating the locations.
• Topsoil piles stored beyond one growing season will be stabilized and seeded to prevent erosion.
• Topsoil storage areas will be identified with appropriate signage, so it is easily identifiable where topsoil is onlocation.
• Noxious and invasive weeds will be inventoried and monitored. Noxious and invasive weeds will be controlledthrough integrated weed management including chemical, mechanical, and biological control methods. Anapproved Pesticide Use Proposal (PUP) is required for all herbicide applications.
• All trash and waste materials not needed for production will be removed immediately.
• Earth work for interim and final reclamation must be completed within 6 months following well completion orwell plugging (weather permitting).
• Salvaged topsoil will be re-distributed evenly, and to pre-disturbance depths, over the surfaces to be revegetated.
• The soil surface will be prepared to provide a seedbed for re-establishment of desirable vegetation. Sitepreparation may include gouging, scarifying, dozer track-walking, mulching, hydro mulching, contour
furrowing, water terracing, fertilizing, disking, soil additives or other approved methods. The seedbedpreparations will be determined to accomplish the reclamation objectives above.
• The seeding contractor will provide all seed tags to the appropriate SMA prior to seeding efforts.
• Seeding will occur after August 15 and prior to winter freezing of the soil.
• Drill seeding will be used except in areas where topography or substrate composition (rock) precludes the use ofthe drill. If drill seeding is not possible, broadcast seeding will be implemented. If the broadcast method is used(such as on slopes of 40 percent or greater), the seed rates established for drill seeding will be doubled and seedwill be immediately covered to prevent seed desiccation or predation by birds or rodents. The seeds can becovered in several ways including spreading and crimping straw over the seeded area, raking the area by hand, ordragging a chain or chain-linked fence over the seeded area.
• Any altered drainages,or water courses will be stabilized through rip rap, berming, seeding, matting, or otherapproved methods, and will be reconstructed to have the characteristics and features of nearby functionaldrainages. Any disturbance to "dashed line" drainages as depicted on USGS 1:24,000 topographic maps will becoordinated with the corps of engineers prior to disturbance.
Final Reclamation:
Final Reclamation will be similar to interim reclamation except, all disturbance associated with the well location,access road and pipeline will be reclaimed. The seed mix may include more shrubs and native species. The visualcomposition will be reestablished to blend with the natural surroundings through re-contouring and reseeding.

• After pipeline installation is complete, all disturbed areas will be reseeded. The seed mixtures to be used will besimilar to the vegetation of the surrounding areas and may consist of grasses, forbs, or shrubs. A recommendedseed mixture is provided in the table below.
Monitoring:
• The initial monitoring report will include but is not limited to: when the re-contouring and seeding wascompleted. The seed applied, the amounts, and the depth of application. Seedbed preparation such as disking ,
mulching, etc. Seed application such as drilling or broadcast seeding. Are there any soil amendments,fertilizers, watering, etc. The amounts of noxious and invasive weeds. Are any fencing requirements needed.
• Prior to any surface disturbance, vegetative monitoring locations and reference sites will be identified by theoperator and approved by the BLM Authorized Officer.
• Vegetation monitoring protocol will be developed by the operator and approved by the BLM Authorized Officerprior to implementation of re-vegetation techniques and will be designed to monitor % basal vegetative cover.
• Reclamation areas will be inspected annually and monitored to document location and extent of areas withsuccessful re-vegetation. Monitoring will continue until the site is considered a success by the authorized officerand the objectives established in the Green River District Guidelines. A reclamation report will be submittedelectronically to the Authorized Officer by March 31 of each year.
• On Federal lands, the reclamation objective will be a vegetation community that within 5 years is comprised ofdesired and/or seeded species, where the basal vegetative cover is 75 percent of a similar undisturbed adjacentnative vegetation community. If after 3 years basal cover is less than 30 percent, then additional seeding andreclamation efforts may be required.

DOWNHOLE PROGRAMCONDITIONS OF APPROVAL (COAs)
SITE SPECIFIC DOWNHOLE COAs:
. Newfield Production Co. shall adhere to all referenced requirements in the SOP (version: June 24,2008) ) along with all Oil and Gas rules and requirements listed in the Code of Federal Regulationsand all Federal Onshore Oil and Gas Orders.
All provisions outlined in Onshore Oil & Gas Order #2 Drilling Operations shall be strictly adheredto. The following items are emphasized:
DRILLING/COMPLETION/PRODUCING OPERATING STANDARDS
• The spud date and time shall be reported orally to Vernal Field Office within 24 hours of spudding.
• Notify Vernal Field Office Supervisory Petroleum Engineering Technician at least 24 hours inadvance of casing cementing operations and BOPE & casing pressure tests.
• All requirements listed in Onshore Order #2 III. E..Special Drilling Operations are applicable for airdrilling of surface hole.
• Blowout prevention equipment (BOPE) shall remain in use until the well is completed or abandoned.Closing unit controls shall remain unobstructed and readily accessible at all times. Choke manifoldsshall be located outside of the rig substructure.
• All BOPE components shall be inspected.daily and those inspections shall be recorded in the dailydrilling report. Components shall be operated and tested as required by Onshore Oil & Gas OrderNo. 2 to insure good mechanical working order. All BOPE pressure tests shall be performed by atest pump with a chart recorder and NOT by the rig pumps. Test shall be reported in the driller'slog.
• BOP drills shall be initially conducted by each drilling crew within 24 hours of drilling out fromunder the surface casing and weekly thereafter as specified in Onshore Oil & Gas Order No. 2.
• Casing pressure tests are required before drilling out from under all casing strings set and cementedin place.
• No aggressive/fresh hard-banded drill pipe shall be used within casing.
• Cement baskets shall not be run on surface casing.
• The operator must report all shows of water or water-bearing sands to the BLM. If flowing water isencountered it must be sampled, analyzed, and a copy of the analyses submitted to the BLM VernalField Office. RECEIVED

• The operator must report encounters of all non oil & gas mineral resources (such as Gilsonite, tarsands, oil shale, trona, etc.) to the Vernal Field Office, in writing, within 5 working days of eachencounter. Each report shall include the well name/number, well location, date and depth (from KBor GL) of encounter, vertical footage of the encounter and, the name of the person making the report(along with a telephone number) should the BLM need to obtain additional information.
• A complete set of angular deviation and directional surveys of a directional well will be submitted tothe Vernal BLM office engineer within 30 days of the completion of the well.
• While actively drilling, chronologic drilling progress reports shall be filed directly with the BLM,Vernal Field Office on a weekly basis in sundry, letter format or e-mail to the Petroleum Engineersuntil the well is completed.
• A cement bond log (CBL) will be run from the production casing shoe to the top of cement and shallbe utilized to determine the bond quality for the production casing. Submit a field copy of the CBLto this office.
• Please submit an electronic copy of all other logs run on this well in LAS format [email protected]. This submission will supersede the requirement for submittal ofpaper logs to the BLM.
• There shall be no deviation from the proposed drilling, completion, and/or workover program asapproved. Safe drilling and operating practices must be observed. Any changes in operation musthave prior approval from the BLM Vernal Field Office.

OPERATING REQUIREMENT REMINDERS:
• All wells, whether drilling, producing, suspended, or abandoned, shall be identified in accordancewith 43 CFR 3162.6. There shall be a sign or marker with the name of the operator, lease serialnumber, well number, and surveyed description of the well.
• In accordance with 43 CFR 3162.4-3, this well shall be reported on the "Monthly Report ofOperations" (Oil and Gas Operations Report ((OGOR)) starting with the month in which operationscommence and continue each month until the well is physically plugged and abandoned. This reportshall be filed in duplicate, directly with the Minerals Management Service, P.O. Box 17110, Denver,Colorado 80217-0110, or call 1-800-525-7922 (303) 231-3650 for reporting information.
• Should the well be successfully completed for production, the BLM Vernal Field office must benotified when it is placed in a producing status. Such notification will be by written communicationand must be received in this office by not later than the fifth business day following the date onwhich the well is placed on production. The notification shall provide, as a minimum, the followinginformational items:
o Operator name, address, and telephone number.
o Well name and number.
o Well location (¼¼,Sec., Twn, Rng, and P.M.).
o Date well was placed in a producing status (date of first production for which royalty will bepaid).
o The nature of the well's production, (i.e., crude oil, or crude oil and casing head gas, ornatural gas and entrained liquid hydrocarbons).
o The Federal or Indian lease prefix and number on which the well is located; otherwise thenon-Federal or non-Indian land category, i.e., State or private.
o Unit agreement and/or participating area name and number, if applicable.
o Communitization agreement number, if applicable.
• Any venting or flaring of gas shall be done in accordance with Notice to Lessees (NTL) 4A andneeds prior approval from the BLM Vernal Field Office.
• All undesirable events (fires, accidents, blowouts, spills, discharges) as specified in NTL 3A will bereported to the BLM, Vernal Field Office. Major events, as defined in NTL3A, shall be reportedverbally within 24 hours, followed by a written report within 15 days. "Other than Major Events"will be reported in writing within 15 days. "Minor Events" will be reported on the Monthly Reportof Operations and Production. RECElVED

• Whether the well is completed as a dry hole or as a producer, "Well Completion and RecompletionReport and Log" (BLM Form 3160-4) shall be submitted not later than 30 days after completion ofthe well or after completion of operations being performed, in accordance with 43 CFR 3162.4-1.
Two copies of all logs run, core descriptions, and all other surveys or data obtained and compiledduring the drilling, workover, and/or completion operations, shall be filed on BLM Form 3160-4.
Submit with the well completion report a geologic report including, at a minimum, formation tops,and a summary and conclusions. Also include deviation surveys, sample descriptions, strip logs,core data, drill stem test data, and results of production tests if performed. Samples (cuttings, fluid,and/or gas) shall be submitted only when requested by the BLM, Vernal Field Office.
• All off-lease storage, off-lease measurement, or commingling on-lease or off-lease, shall have priorwritten approval from the BLM Vernal Field Office.
• Oil and gas meters shall be calibrated in place prior to any deliveries. The BLM Vernal Field OfficePetroleum Engineers will be provided with a date and time for the initial meter calibration and allfuture meter proving schedules. A copy of the meter calibration reports shall be submitted to theBLM Vernal Field Office. All measurement facilities will conform to the API standards for liquidhydrocarbons and the AGA standards for natural gas measurement. All measurement points shall beidentified as the point of sale or allocation for royalty purposes.
• A schematic facilities diagram as required by Onshore Oil & Gas Order No. 3 shall be submitted tothe BLM Vernal Field Office within 30 days of installation or first production, whichever occursfirst. All site security regulations as specified in Onshore Oil & Gas Order No. 3 shall be adhered to.All product lines entering and leaving hydrocarbon storage tanks will be effectively sealed inaccordance with Onshore Oil & Gas Order No. 3.
• Any additional construction, reconstruction, or alterations of facilities, including roads, gatheringlines, batteries, etc., which will result in the disturbance of new ground, shall require the filing of asuitable plan and need prior approval of the BLM Vernal Field Office. Emergency approval may beobtained orally, but such approval does not waive the written report requirement.
• No location shall be constructed or moved, no well shall be plugged, and no drilling or workoverequipment shall be removed from a well to be placed in a suspended status without prior approval ofthe BLM Vernal Field Office. If operations are to be suspended for more than 30 days, priorapproval of the BLM Vernal Field Office shall be obtained and notification given before resumptionof operations.
• Pursuant to Onshore Oil & Gas Order No. 7, this is authorization for pit disposal of water producedfrom this well for a period of 90 days from the date of initial production. A permanent disposalmethod must be approved by this office and in operation prior to the end of this 90-day period. Inorder to meet this deadline, an application for the proposed permanent disposal method shall besubmitted along with any necessary water analyses, as soon as possible, but no later than 45 days -
after the date of first production. Any method of disposal which has not been approved prior to theend of the authorized 90-day period will be considered as an Incident of Noncompliance and will begrounds for issuing a shut-in order until an acceptable manner for disposing of said water is providedand approved by this

• Unless the plugging is to take place immediately upon receipt of oral approval, the Field OfficePetroleum Engineers must be notified at least 24 hours in advance of the plugging of the well, inorder that a representative may witness plugging operations. If a well is suspended or abandoned, allpits must be fenced immediately until they are backfilled. The "Subsequent Report ofAbandonment" (Form BLM 3160-5) must be submitted within 30 days after the actual plugging ofthe well bore, showing location of plugs, amount of cement in each, and amount of casing left inhole, and the current status of the surface restoration.
