Executive Summary This document presents the data dictionaries and third party interfaces elements of the Do CHANGE ecosystem. These are key components of the Do CHANGE architecture (Ref. [A]), which facilitate data sharing and input of data from third parties.

This document taken in combination with other Do CHANGE design documents defines the ecosystem architecture that will be implemented for use in the 2nd phase of Do CHANGE field trials.

1. About This Document

Do CHANGE aims to develop and provide a health infrastructure for integrated disease management of citizens with high blood pressure and patients with ischemic heart disease or heart failure. In this context, it aims to give them access to a range of personalised health services intended to encourage behaviour change and to allow their behaviour and clinical parameters to be monitored. The information collected will be shared amongst the health services in a manner that reflects the citizens wishes and aims to optimise their health and well-being. This document presents the data encoding to be employed for information exchange within the Do CHANGE ecosystem. It also describes the interface to allow patient related resources to be obtained from third-party systems in a generic manner and made available to services / care providers within the Do CHANGE ecosystem.

1.1. Deliverable context

Objectives This document is linked to the following WP4 objectives:

To Specify and develop the overall Do CHANGE Ecosystem Architecture, both the services & component infrastructure, the data infostructure and its semantics.

The Data collection, storage, management and communication will take place in a secure patient controlled environment and feedback on transactions will be fed back to the individual.

All incoming data will be semantically tagged such that background knowledge, context and precise meaning are stored with the data and will inform any use of the data and its analytics.

To define the service interfaces of all parties.

Exploitable results This part of the project is service design. It is anticipated that the services developed from this design would be further developed to commercially exploitable systems.

Work plan This deliverable is associated with Tasks 4.3 and 4.5 of WP4.

Milestones This deliverable is not linked to an overall project milestone but does mark the submission of D4.14 (D65)

Deliverables This document presents the data dictionaries and third-party interface components that are to be employed within the overall Do CHANGE ecosystem during the phase 2 clinical field trials.

Risks By having this document available, the risk of failure of the technology supporting the Do CHANGE phase 2 clinical trials due to inability with exchange data between systems and with external systems, should be reduced.

2. Data Exchange

2.1. Intention

The intention of this section is to detail the mechanism that shall be used to represent data that can be exchanged both within the Do CHANGE ecosystem and with external systems, such that its information content can be interpreted by consumers of the data. The scope of the data to be exchanged includes:

physiological measures;

details of clinical symptoms;

lifestyle measures (activity, sleep, food intake measures, fluid intake measures etc.);

lifestyle and life quality measures;

Do’s (messages intended to encourage behaviour change).

2.2. Use of HL7 FHIR Observations for Recorded Information

Various means by which recorded information could be exchanged were considered. It was notable that the physiological measures had well established approaches for their representation via Health Level Seven (HL7) (Ref. [B]). In contrast, while there are ways of encapsulating other types of data, these do not cover all types and tend to be specific to particular commercial vendors (for example Fitbit – see Ref. [C]) and / or are not widely adopted (e.g. HumanAPI – see [D]). Hence, the approach adopted builds on the functionality for data exchanged offered by HL7.

HL7 itself has various versions and forms. The variant to be employed as the basis encapsulating recorded information within Do CHANGE, is the Observation type specified by HL7 FHIR (Ref. [E]). The reason for selecting this is that it provides a relatively compact representation of data, compatible with more modern REST based interfaces, and that it allows the following key information items to be grouped:

timestamp;

the person (patient) to whom the encapsulated data relates;

the value of the data;

one or more codes that are associated with the data and / or the process for collecting it.

Furthermore, it provides a fall-back, plain-text / narration representation of the information for recipients that do not understand the coding used.

The structure of a HL7 FHIR compliant Observation is summarised as follows:

Text – this element contains the optional plain-text / narration element.

Status – this is a mandatory element that supports observations with different status values; in the context of Do CHANGE, it would have a fixed value equal to “final”.

Code – this is the optional representation of the encoding of the observation.

Subject – this identifies the patient associated with the data using a generic scheme that supports pseudonymised and non-pseudonymised identifiers.

Value – this is the value of the measurement, which will be the subtype Quantity for all supported Do CHANGE data items. It optionally includes one or more encodings for the value.

In the case of the physiological values, a combination of SNOMED (Ref. [G]), Loinc (Ref. [F]) and IEEE 11073 (Ref. [H]) codes are employed that uniquely identify the observation process and / or data type are used as appropriate. For other data items, an extendible Do CHANGE code system is used (see Annex A).

2.3. Use of HL7 FHIR Procedure Request for Do’s

Building on the use of HL7 FHIR Observations, a separate entity specified by HL7 that is relevant to the scheduling of Do’s is the Procedure Request (Ref. [E]). The structure of a HL7 FHIR compliant Procedure Request is summarised as follows:

Code – this mandatory item specifies a code, which explicitly defines the “procedure”. In this case, the “Procedure” is the delivery of a Do, and so a Do CHANGE specific coding scheme is employed, in which each Do has a unique code as summarised in Append A.

Subject – this mandatory item identifies the patient to whom the Do should be delivered.

Scheduled – within HL7, this is this is an optional item with three variants; where used by Do CHANGE to convey the scheduling of Do’s, it shall be mandatory and the scheduledTiming variant shall be used, with one entry that specifies the earliest time that the Do should be scheduled for presentation to the patient..

2.4. Examples

A range of samples of data of the type employed in Do CHANGE when encapsulated using HL7 FHIR are presented in this section, to provide models for the specific implementation used by the Do CHANGE ecosystem. Note that the “Subject” element in each of these examples employs a “DocoboAgreementNumber” reference. This is for illustration purposes only and in actual use, the scope of the Subject element will depend upon the consumer of the information.

3. Third-Party Interfaces

3.1. Introduction - Consume / Collect Service Pair

This chapter addresses the third-party integration with the Do CHANGE Ecosystem. As indicated in Figure 1, two services, Consume and Collect, facilitate the data acquisition and aggregation from external, third-party sources including Fitbit, Moves, Beddit, SAL, MySleeve, and Onmi Analytics.

The Consume service facilitates the authentication and authorisation of the participants granting Do CHANGE access to the data through a web browser view integrated in to the patient portal user interface (in this case the Do CHANGE Vire Smart Phone application is shown for illustration). The Collect service requests data from the external, third-party APIs through the Consume service, with Consume only authenticating requests made by the Collect service. The primary reason to separate these services, is to allow hosting on different servers, thereby ensuring security. The participant’s identity is pseudonymised by an identity provider service, which in this example is indicated by Auth0.

The Collect and Consume services have similar designs, being based on Node.JS (Ref. [I]), and employing the scripting-language ECMAScript (ES) (Ref. [J]). Both host machines run a NGINX http server (Ref. [K]) and employ a reverse proxy configuration to ‘re-route’ external access to the internal server process. The NGINX server is also configured to provide secure connections by only allowing https connections. Non-secure connections shall be automatically re-routed to their corresponding secure URL. In addition, the reverse proxy ‘cloaks’ the identity of the process that is running on the host. Both hosts have a MongoDB (Ref. #) database instance, with access to this restricted to the host. For the Consume service, this is used to store User Access Tokens and APIs details specific to each third-party interface. In the case of the Consume service, it is used to store User Data and inbound Requests. Both services are to run on a local cluster using Process Manager 2 (PM2 – see Ref. [L]) to ensure availability should a process crash and facilitate increasing capacity under normal operating conditions.

3.2. Concepts Employed

3.2.1. End-points

The interactions between subsystems is based upon the hypertext transfer protocol (http – see Ref. [M]) protocol. Reference is often made to end-points which are specified by a path, for example

/provider/subscription

This indicates that the system in question provides a http or https (secure version of http) interface and that the functionality in question is access via the URI (assuming https):

https://hostname/provider/subscription

Where ‘hostname’ is the name or IP address of the host providing the functionality.

3.2.2. JSON Web Tokens and OAuth2

The design makes extensive use of OAuth2 (Ref. [N]) and associated JSON Web Tokens (JWT – see Ref. [O]) to provide security in system-to-system interactions. The key ones of these are as summarised as follows:

JWT – JSON Web Tokens are items included in http headers that are an efficient way to combine attribute / value pairs with a digital signature that can be used to verify the authenticity of the attribute / value pair information. The signature may use either a shared, secret key or public / private keys, and the data itself is base64-encoded JavaScript Object Notation (JSON) format data.

OAuth2 is a standard that supports functionality related to the security and authentication of systems-to-system interactions. One of these is the following authorisation / authentication life cycle for a resource provider / resource consumer relationship.

1) The resource provider issues the consumer and ‘authorisation code’, to allow a specific type of access to specific resources; often this follows approval of such access by the owner of the resources through an interactive process.

2) The consumer exchanges the authorisation code for a pair of ‘tokens’, one for immediate use, which normally has a limited validity period, the other a ‘refresh token’ that can be used to obtain a new pair of tokens when the one for immediate use becomes invalid. Within the following text, these are referred to as ‘UserTokens’.

The tokens exchange using JWT.

A second form of interaction is when the provider issues a token to the consumer along with a shared secret. This token is then used by the consumer in interactions with the provider to verify it authenticity. In this case, the token is passed as a JWT with the signature being based on the shared secret.

3.3. Subsystems Interaction Message Sequence Charts

3.3.1. Introduction

In this section, interactions between subsystems are presented and explained for the following cases:

Consume service authorisation, that is the process by which the end-user (a patient) grants access to their resources held by a third-party provider through the Consume service;

Collect service invocation of third-party API – this is the process by which the Collect service calls a third-party API function (to access a resource) using the Consume service.

Consumption of pushed, third-party data – this is the process by which subscription data pushed out by a third-party is ‘consumed’ into the Collect service’s local database.

Notification of new or updated data - the third-party provider sends a notification that new or updated data is available, which is then consumed’ into the Collect service’s local database.

Collect service pulls data from third party, that is the process when the Collect service initiates the transfer of resources from a third-party;

String of resource data by the Collect service, that is, the processing of inbound resource data from third-party providers within the Collect service.

In many of these the third-party source Fitbit is used for illustration, although the approached used is generic.

3.3.2. Consume Service Authorisation

Figure 2 illustrates the message sequence chart for authenticating with the Do CHANGE ecosystem for third-party API integration.

a) The process starts with a Do CHANGE client, in this case indicated by ‘DoCHANGE App’, sending a POST request to the Consume API authorisation end-point for the third-party source. The general path to the end-point is ‘/provider/auth’; in the case of Fitbit, it would be ‘/fitbit/auth’. The POST request includes the query parameter ‘user’, which specifies identity provider ID for the entity making the request.

b) Upon receiving this POST request, the Consume API will trigger the authenticate function. This obtains API settings for the third-party provider (e.g. Fitbit) the API collection in the Consume service’s local database. Assuming these details are present, they are returned and loaded as a Passport Strategy. If no valid settings are present, the Do CHANGE client handles this appropriately as an unexpected error (in a normal use-case this can only be caused by a bug on the client-side).

c) The authenticate function redirects to the authentication URL associated with provider, which are contained in the API settings obtained from the Consume service’s local database. The third-party provider will request the interactive user associated with the Do CHANGE App to authenticate themselves (login using the account details they have with the provider). Upon successful authentication of the interactive user, the third-party provider requests that the interactive user authorises the Do CHANGE interface to have a specified level of access to resources within the third-party provider system that are associated with the interactive user. When the interactive user approves this access, the Consume API is called by the third-party provider an access authorisation code. The access authorisation code is exchanged by the backend for access and refresh tokens issued by the third-party provider.

d) Upon receiving the access and refresh tokens, the backend sends a GET request to the third-party for the profile information. Upon receiving the profile, containing the interactive user’s ID on the third-party system, the backend will search its local database for ‘UserTokens’ records with matching provider and user ID attributes. If existing documents are found, these are update with the new information provided by the third-party provider, otherwise, a new ‘UserTokens’ record is created identified by a composite consisting of the third-party name, third-party user ID for the interactive user, and the Do CHANGE identity provider used in the POST request that initiated the sequence.

e) Upon success or failure, the DO CHANGE client provides appropriate feedback to the interactive user. If the third-party supports subscriptions or notifications, the backend sends the third-party a further request to enable reception of updates.

3.3.3. Collect service invocation of third-party API

The message sequence chart for the Collect service accessing a resource on a third-party via that third-party’s API function, is presented in Figure 3.

a) The sequence begins when the Collect service sends the Consume service’s API a GET-request containing a JSON WebToken (JWT), that has the provider name as a parameter.

b) The Consume API then relays the URL, provider, and user ID to the Consume backend. Using this information, the Consume backend attempts to lookup the associated ‘UserTokens’ for the third-party in its local database. Assuming the ‘UserTokens’ exist (which is expected because Collect would not be notified otherwise), a GET-request is sent to the third-party based on the URL specified by Collect service and the ‘AccessTokens’. Since in accordance with the OAuth2 specification ‘AccessTokens’ have a limited lifespan, the third-party provider will respond with the requested data if the ‘AccessTokens’ are still valid. Otherwise, the response will indicate an ‘invalid access token’ error, in which case the Consume Backend will find the obtain a new ‘UserTokens’ using the refresh token previously issued, and the Consume Backend will update the ‘UserTokens’ and re-issue the initial request obtained from Collect.

c) The returned resources from the third-party are added to the body of a POST request, which is secured using a JWT and sent back to the Collect service. The Collect service will verify reception by issuing a http status code with value 200 (OK). It will also verify the JWT before processing the data in the POST; this is to prevent spoofing attacks.

3.3.4. Consumption of pushed, third-party data

Figure 4 presents the message sequence chart for the consumption of data pushed by a third-party into the Collect service.

a) The sequence commences when the third-party sends data to the end-point specified when the interface was authorised by the patient (see above). This is directed to an end-point in the Consume service of the form /provider/subscription, where ‘provider’ identifies the third-party and ‘subscription’ is a fixed string.

b) In accordance with the third-party provider’s protocol, the Consume API returns an appropriate response to indicate successful reception of the subscription information. Normally, this would be a response with a http status code equal to 200 (OK).

c) The handling of the subscription data is provider specific. For example, in the case of Fitbit, it contains an array of one or multiple updates, each with the Fitbit user id associated with the data and the Do CHANGE identity provider id associated with the original authorisation.

d) A POST request containing the update along with a JWT is created and send to the Collect end-point for subscriptions. Collect return http status code value 200 (OK) to acknowledge that it has received the information.

3.3.5. Notification of New or Updated Data

Figure 5 is the message sequence chart for notification by the third-party system of the availability of new or updated data, which triggers a transfer to / update of the Collect database.

a) The sequence commences when the third-party reports the update via the Consume Backend, which sends an array of updates signed by a JWT. The Collect service responds with http status value equal to 200 (OK), thereby acknowledging reception of the request.

b) The Collect service verifies the JWT and returns the decoded updates.

c) The Collect service queries its local database for requests. Requests are templates for URLs that contain placeholders for the parameters available in the updates. A function, utilising Regular Expressions will replace the placeholders with actual values, sign a JWT, and add this to the header of a POST request sent to the /request end-point of the Consume service’s API interface.

3.3.6. Collect Service Pulls Data from Third-Party

The message sequence chart for the Collect service requesting a patient’s resources that are stored on a third-party system is presented in Figure 6.

a) The sequence is triggered by an update received from Consume. The Collect service sends a request containing a JWT in the header and the request to be made of the third-party provider.

b) The Consume service verifies the JWT and decodes it. The Consume service API handler sends the third-party provider and user id details to the Consume backend.

c) The Consume Backend locates the ‘UserTokens’ in its local database and using these in the header, sends a request to the specified third-party URL.

d) The third-party provider returns the requested resource.

e) The Consume Backend packages the returned data from the third-party provider with a JWT. The returned data is sent as JSON in the body of the http packet,

f) The Consume Backend sends a POST request to the /response end-point of the Collect service with the JWT in the header and the data from the third-party encapsulated in JSON within the body; the data is placed in the body of the http request because it may be too large to fit in the header.

g) Collect will send http status code value 200 (OK) to acknowledge receiving the request and verify the JWT.

Note that the connection between Consume and Collect is encrypted by the TLS-tunnel (https) so the data cannot be sniffed by intruders.

3.3.7. Storing of Resources Data by the Collect Service

Figure 7 is the message sequence chart for the processing of inbound resource data from third-party providers within the Collect service.

a) The sequence commences after data is received from the third-party provider. The Consume service sends a POST request with a JWT in the header to the /response end-point of the Collect service.

b) The Collect service responds with http status value equal to 200 (OK) to acknowledge it has received the request.

c) After verifying the JWT, the Collect service checks for existing data in its local database based on the combination of the provider, user id, request URL and date; which are available in the JWT sent by the Consume service. If no documents are returned, the Collect service creates a new document with the data from the body. If a matching document exists, the Collect service overwrites the data element in the document and update the ‘updated-at’ timestamp in the document.

4. Conclusion / Future Work

Taken together, the two main parts of this document provide designs for two key elements of the overall Do CHANGE ecosystem. The next stage is to implement them and combine them with other key elements to form a working prototype for the Do CHANGE ecosystem for use in the phase 2 clinical field trials. This working prototype should then be tested and evaluated, making any necessary refinements before commencing the phase 2 clinical field trials.

Overview

This Appendix presidents the initial contents of the Do CHANGE specific data coding dictionary. Most the dictionary entries are related to questionnaires and / the presentation of prompt information and DOs. Hence, the following types are supported:

Prompt – textural information shown to the patient.

Yes/No Question – a question in which a prompt is presented to a patient and they are expected to respond with either ‘yes’ or a ‘no’.

Single Selection Multiple Choice Question - – a question in which a prompt is presented to a patient and they are expected to select one textural item from a list of options.

Multi Selection Multiple Choice Question - – a question in which a prompt is presented to a patient and they are allow to select one or more textural items from a list of options.

Scale Question - – a question in which a prompt is presented to a patient along with a slider scale that they can adjust. The scale may have numeric or textural labels at each end. The ends are associated with numeric values and the resolution with which the patient can adjust the scale is determined by a step-size parameter.

Grouped Questions – these are questions of the type that are grouped and may have a common prompt for the group.
