Bournville Netball Club: INTERS & SENIORS U18Photographic/Film Footage/Name Consent Form

Details:Bournville Netball Clubs (BNC) intends to maintain the clubs website and publicity material to communicate latest news and promote the clubs activities. This will involve taking photographs of players and adding these photos, along with players names for various purposes including ‘Player of the Match’ etc on Bournville Netball Club website (www.bournvillenetball.co.uk) and other various club publications. Please sign in section 1 below if you give permission for your child’s photo and name to appear on BNC publications including the website and return to BNC personnel. Please note the names of the players will appear next to photos of players but no other personal details will be given.

Also, due to strict checks and child protection, we ask that you do not bring camera’s or video recorders with you to games in order to take photographs of your daughter and her team or record the games. We understand that this will be difficult, but we have to look after all of the children’s best interests and provide them with the safety that they require.

I hereby grant/do not grant (delete as applicable) Bournville Netball Club the absolute right to use the images resulting from a photo/film shoot and print the name of my daughter. This includes any reproduction or adaptations of the images for all general publicity purposes. I also confirm that I will not use my camera or video recorder at training or matches, without the prior consent of Bournville Netball Club.

Copyright of the photographs belongs to Bournville Netball Club. Bournville Netball Club will not give permission for the use of these photographs/details to any third party who they believe will mis-represent them and the spirit in which they were taken. Please be assured, Bournville Netball Club intends to print the name of the players and will not print any other details for example date of birth/contact details etc.
