The Local Context

Dorset Population c. 400,000

Largest percentage of older people in the country – 29.3% aged 60+ in Dorset compared to 20.9% nationally

Christchurch - 36.1% of the population aged 60+

Dorset POPP Focus aged 50+

Aim of Dorset POPP To work with older people to develop supportive

neighbourhoods where older people can live and receive appropriate, timely help and support to stay within local communities for as long as they wish

Over a two year period POPP will support and develop positive interventions and preventative actions identified by local people. This will ensure cost savings on the provision of acute services, and on the take up of hospital bed spaces, which in turn will be clawed back into supporting a sustained POPP into years 3 and 4

Anticipated Gains for Older People An opportunity to ‘Challenge and Change’ Greater Confidence in working effectively in

partnership with providers of health & Social Care

Increasing awareness of services and support available within the community

Increased access to local information, resources, and transport

Social activities, befriending, shopping and practical support

Reduced isolation and increased sense of community safety

Anticipated Gains for Health Service POPP will enable greater choice for Practice

Based Commissioning – identifying local service need and provision

Addressing the issues of spending on acute services

Reduction in the uptake of hospital bed spaces by addressing the issues of delayed discharge

Community Development Programme to work with statutory organisations to support targets on:

Expert patient programme, long-term conditions, mental health promotion, and general health promotion

to help bring about change in each of the neighbourhoods

Infiltrate local meetings and partnerships such as Parish Council, Town Council, PCT PPI Forums, committees

Influence local service providers and develop and shape services based on the needs of older people

Use local knowledge and experience to identify existing facilities for establishment of community resource centres

Local Way-Finders Local Older People aged 50+ To provide advocacy, information and sign-

posting Supported by the local Leaders Raise awareness of services available Make information accessible to local older

people in their communities Make Links to preventative, health promotion,

and support services Identify support and help within communities

for older people being discharged from hospital and other institutional care providers

Local Evaluators

To measure change as it occurs during the life of the project

To use emerging evidence to influence the way services are provided by local councils and health

Working alongside Local Leaders and Way-Finders Within and across neighbourhood clusters Evaluate how services are working and what

impact they are having on the lives of older people Evaluate the success of the partnership, the Board

and the structure of Dorset POPP

Practical Changes Working with older people to develop ideas on

services that will help older people to live independently and will promote positive initiatives for good health and social activity:

Safer property – Handy van and staying put schemes

Physical environment – assistive technology, ‘sloppy slipper’ falls reduction

Intellectual and spiritual stimulation – Arts Reach, Book Clubs etc.

Companionship – Lunch Clubs, befriending, coffee mornings

Good Health – Exercise uptake, Healthy eating Older Peoples Services Mapping undertaken
