A medical problem is a disease state; that is a problem related to altered physiology resulting in clinical evidence of damage.

Drug therapy problem, however, is a patient problem that is either caused by or may be treated with a drug.

In practice, such distinctions are essential.The diagnose of medical problem is the responsibility of the

physician, whereas the scope of a pharmacist’s practice must be limited only to drug therapy problems.

Hypertension is a disease, and therefore is a medical problem. If the patient requires drug therapy for her hypertension and is not receiving it, she has a drug therapy problem.

The hypertension is not the drug therapy problem, but the need for drug therapy is.
