address on the bottom of the certifications
Application Review
Program Integrity Assurance (PIA), USAC reviews your Form(s) 471 to:
Check the eligibility of the schools and libraries and their discount levels
Verify that the services you requested are eligible for discounts
Receive a Receipt Acknowledgement Letter (RAL)
Make corrections to RAL if necessary (20 Days) Receipt Acknowledgement Letter (RAL)
Funding Commitments
Receive your Funding Commitment
Following application review, USAC issues a Funding Commitment Decision Letter (FCDL) to both the applicant and the service provider(s)
Carefully review the FCDL for details on approved or denied requests and NEXT STEPS
Complete Form 486o Must be filed within 120 days of the start of service
or Funding Commitment Letter- whichever is LATER o If deadline is missed: Funding will be reduced one
day for each day the Form is late!
