The hallways of Mountain Park Health Center are rich in our branded culture: one of patient-centered care, community engagement, and holistic wellness for those we serve. The pride we derive from this culture and our nearly 30 years of service throughout the Valley is tremendous and exceeded only by our pride in the style with which we lead.
The health industry is one which has borne witness to sweeping change over the past five years. Mountain Park has not simply weathered this change, but rather embraced—and in some cases, prompted—it. Our commitments to community, comfort, and reliability are aligned with health care policy stressing the same, and we are honored to serve as a model health center for the Valley.
Our newest clinic, scheduled to open in Fall of 2014, is another manifestation of our quality healthcare. The Gateway Clinic is Mountain Park’s response to the dire needs of the community; needs for a health facility large enough, a staff robust enough, and an organization compassionate enough to serve the thousands who will enter its doors. The Gateway clinic is — in its every fixture and amenity — just that.
The road ahead for Mountain Park is a familiar one. We’ve traveled it with confidence for nearly three decades. Our pursuit and dedication toward widespread, affordable healthcare is unrelenting; our decades-long bonds with the communities we serve are unbreakable; the quality of our care is incomparable.

At Mountain Park, our energies are wholly committed to creating
comfortable environments for our patients. For so many, the doctor’s office
is a daunting place—a cold, eerily quiet departure from the comforts of home.
The responsibility for combating these ideas falls not solely upon individual
physicians, but upon our entire Mountain Park team. Our patients experience
integrated visits with multiple care team members who work together to
ensure patient health. This cooperation is important to us. The continuity and
commitment we hope to earn from our patients is possible only if paired
with a patient-centered approach on our end. Therefore, our assurance
at Mountain Park is that patients will meet with the same care team from
visit-to-visit. This allows care team members including doctors, nurse practitioners,
clinical pharmacists, as well as patients to grow familiar and develop rapport

Patient-centered healthcare has long been the calling card for Mountain
Park, and while the nation trends toward this type of care, Mountain Park stands
confidently amid this fluid health industry. Our sights are now set on broadening
our patient’s power, control, and flexibility. Our newly-developed patient portal is one stride toward that end. Using the portal, patients will be able to
securely access medical records, check lab results, request an appointment,
and communicate with their care providers — all using mere clicks of a computer
mouse or swipes on a smartphone. Additionally, we’ve updated our website to
include a photo listing of all Mountain Park providers. Never has the patient-doctor relationship been so cooperative and personal.

The Mountain Park approach to wellness is a communal one. The success
of our care does not manifest itself merely in the wellness of a particular
patient, but also in the strength and vitality of the communities to
which they belong. Mountain Park offers free health classes for our patients,
equipping them with knowledge they may both internalize and disperse
to family and others in their communities. Our diabetes courses, for example,
inform patients how to monitor the chronic illness, giving them the knowledge
and control historically reserved for their doctors. Similarly, The All Kids Can program is committed to curbing childhood obesity. The course unites
children, parents and physicians around a curriculum designed to sponsor
health and wellness in 6- to 13-year-olds. These,
and our many other courses, educate our patients
and extend Mountain Park’s emphasis on health
and wellness beyond our walls.

raising it up2013 was a big fundraising year for our new Gateway clinic
Mountain Park continues to grow. In 2013, our tireless fundraising efforts
advanced the progress of our new Gateway Clinic; a clinic which will be
unlike any other in Phoenix. The Mountain Park Gateway Clinic will be a
community hub for wellness, designed by SmithGroup JJR. Our commitments
to preventive care and community education will remain, as the new clinic
will offer health education classes and have a demonstration kitchen. Additionally,
we’ve raised over $154,000 in funding for the development of a
playground at the Gateway Clinic. This investment is one particularly dear to
us. The playground will provide a space for children to be active while waiting
for appointments, as well as a play area for children after school facilities have
closed. We are excited to open the doors to our new Gateway Clinic at
3830 E. Van Buren St., in fall 2014. All are invited to follow our progress by

keeping our eyes on the ballThe Mountain Park Foundation is the extended arm which delivers tailored services to our unique client communities. Our tireless fundraising efforts are responsible for the intimacy, personality and specialty which characterize our clinics. Funds obtained by the Mountain Park Foundation have supported the construction of our new Gateway Clinic; installed handicap-accessible doors at our clinics; supplemented patients’ prescription costs; and provided transportation for our immobile cancer patients.
The Mountain Park Foundation will continue to operate with two feet evenly grounded: one inside our clinics and the other outside our walls gauging and responding to the communities we serve. Our strong ties to communities in the Valley grant us knowledge about the wants and needs of our patients, and our 501(c)3 status grants us the platform necessary to bring these wants and needs into fruition. The Mountain Park Foundation ensures that Mountain Park will forever operate in the best interests of our patients. At Mountain Park, our goal is to provide this quality care and broaden the scope of those who benefit from it.
The past year has been one of tremendous progress for Mountain Park; one in which we’ve accomplished the expansion of healthcare and the deepening of its services. We look forward to standing and building upon these successes in the coming year.
