NICOBAR, do hereby solemnly affirm & declare as follows: 1. That my father Late K SHANMUGAM got expired on 09.04.2020 at Garacharma, Port Blair, South Andaman, A & N Islands (Copy of Death Certificate annexed. He was working as “Permanent Industrial Mazdoor” in the “DEPARTMENT OF ENVIRONMENT AND FORESTS, A & N ADMINISTRATION, NICOBAR DIVISION, CAMPBELL BAY (Copy annexed).
2. That my actual name is ‘LOGESHWARI’, D/o LATE K. SHANMUGAM & my actual name is mentioned in my Aadhaar Card bearing No. 6693 3663 3653 (Copy annexed). 3. Inadvertently my name is mentioned as LOGESWARI in the afficial documents / Service Book of my father Late K. SHANMUGAM, in the DEPARTMENT OF ENVIRONMENT AND FORESTS, A & N ADMINISTRATION, NICOBAR DIVISION, CAMPBELL BAY instead of my actual name LOGESWARI (copy annexed). 4. That LOGESWARI & LOGESH MERI are the same name of mine & noboby else & are one & the same identical person i.e. myself. 5. That this affidavit is required for the purpose to declare my actual name as LOGESWARI for the purpose of rectification of my name in the official documents / service book of my father Late K SHANMUGAM, in the DEPARTMENT OF ENVIRONMENT AND FORESTS, A & N ADMINISTRATION, NICOBAR DIVISION, CAMPBELL BAY. I, the abovenamed deponent do hereby solemnly affirm and declare that the above statements are true & correct to the best of my knowledge and belief and nothing has being concealed thereon by me.
