BASIC CRITERIA FOR DEVELOPING AN EVACUATION PLAN

Section 404.1 of the fire prevention code states that public buildings are required to have evacuation plans. The following outline is the basic criteria and standards for developing a plan. If additional information is needed, please contact our office. NOTE: Public Buildings- An approved fire safety evacuation plan shall be prepared and maintained for the following occupancies and buildings. Group A = Assembly. Other than Group A, occupancies used exclusively for purposes of religious worship that have an occupant load of less than 2,000. Group E= Educational, Group H =High Hazard, Group I= Institutional, Group R-1=Boarding Houses, Hotels and Motels, Group R-4=Residential Care/Assisted living facilities, High Rise Buildings, Group M= Mercantile buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. Covered malls exceeding 50,000 square feet in aggregate floor area. Underground buildings and buildings with an atrium and having an occupancy in Group A, E or M.

1) DESCRIPTION AND PLAN REQUIREMENTS OF ALARM SYSTEM AND/OR AVAILABLE EMERGENCY EQUIPMENT.

i) Alarm System (a) Type - Simplex 4100. Monitored 24-7 by Mitec Monitoring and Atlanta Fire

Department.(b) Location - Events Office immediately off the theatre lobby.(c) Proper Usage – The system monitors all areas of the building for smoke and heat.

Should the system go into alarm it will sound/strobe in all common and industrial areas of the building. Those areas that do not have sound/strobe such as the auditorium and ballroom are properly monitored by Fox staff at all times while events are in progress. The system will automatically call the monitoring company and AFD should the fire alarm be activated.

(d) Routine Inspection & Testing Equipment – The entire system is tested annually by MitecNet. It also has internal test circuits that monitor and test smoke detectors, heat detectors, and batteries for troubles. Should a device experience trouble the system sounds an audible trouble alarm at the panel that has to be addressed before the alarm can be silenced.

ii) Other Available Emergency Equipment – The Fox is protected by a full service sprinkler system, fire extinguishers in all areas of the building, vent hood extinguisher in the ballroom kitchen, (2) heart defibrillators, first aid kits placed in specific locations, and an emergency generator that supplies emergency lighting throughout the building.

(a) Clear Identification & Designation of Emergency Exits - all emergency exits are clearly marked with lighted signs.

(b) Movement Routes Posted in Conspicuous Place(s) on Each Floor – The dressing room tower has routes posted.

(c) Elevators (In Event of Fire, Do Not Use Elevators ) – The Fox has six elevators. The four newer ones go to street level if an alarm sounds. The two original elevators located in the lobby are manually operated by Fox Staff during the day and Fox events staff during performances. Our staff has been instructed not to use elevators for egress during emergency situations such as fire, bomb threats, and severe weather.

(d) Emergency Exits – according to the City of Atlanta and AFD, The Fox has ample emergency exits that are adequate for any emergency evacuation.

(e) Elevator Keys – Due to the age and numerous styles of Fox elevators, keys are made available through the Fox maintenance department. Newer elevators have key boxes mounted at ground level.

(f) Other – N/A

2) ORGANIZATION OF BUILDING AND TENANT PERSONNEL

i) Duties, Responsibilities, Assignments Or Definitions Of Any Or All Of The Following As They Apply To Your Facility:

(a) Building Fire/Emergency Marshal – N/A(b) Emergency Equipment Operators – Numerous Fox Staff are certified by AFD to

operate fire extinguishers, CPR kits, and defibrillator units. The maintenance department operates the emergency generator.

(c) Floor Wardens – N/A(d) Stairway Monitors - Events Staff during performances.(e) Floor Guards -N/A

(f) Handicap Wardens – The Fox events staff and ballroom staff are trained to assist patron with special needs.

(g) Support Personnel: (i) Building Security – Security is monitored by all Fox staff during non event

hours. Both Fox staff and contract security are used to secure the building and provide patron safety during events.

(ii) Assistants, Fire – N/A(iii) Emergency Team – The Fox has an emergency team. They are made

up of members from each department and are trained in a full range of emergency services. This subject is addressed in the Fox Emergency Manual.

(iv)Maintenance – The Fox has a full time maintenance staff for both day and night operations.

(v) Etc. 1. Note: Designate Someone To Call The Fire Department. – During non-

event hours the Director of Operations or a member of the maintenance staff would call 911. If an alarm sounds during and event the manager on duty (MOD) would make the call.

ii) Diagram Of Building's Organization (a) Reporting System – Written – Refer to the Fox Emergency Manual under Chain

of Command.(b) Diagram Or Chart (Optional) – Refer to the Fox Emergency Manual under

Chain of Command Section Flow Chart.

iii) The Organization Should Be Structured To Provide For: (a) Easy Prompt And Comprehensive Dissemination(b) Any Change Or Amendments To All Building Occupants(c) Informative, Accurate And Sensitive Discussion of Plan(d) Familiarity with and willing Implementation of The Plan To Include Practice

Drills Or Test Of Plan Components

**The Fox is operating its Emergency Program under these guidelines**.

3) BUILDING EMERGENCY EVACUATION PROCEDURES

i) Evacuation When The General Alarm Sounds - General Alarm Evacuation – Fox (a) Refer to the Fox Emergency Manual under Evacuation.

(2) Evacuation Without General Alarm - Small Fire – Fox (a) Refer to the Fox Emergency Manual under Evacuation.

ii) Explain Delay in Fire Alarm If One Is Used – N/A (1) In case of fire - Two floors above and one floor below the fire floor shall be evacuated (2) When the building is designed to resist the spread of smoke and fire – N/A (3) Should there be a fire in the Fox during an event - a general evacuation will take place.

4) EMERGENCY PROCEDURES

i) Step-By-Step Instructions Of Action For Personnel And Tenants To Be Taken In The Event Of Emergency- Refer to the Fox Emergency Manual under the selected emergency.

(1) Fire (a) Within The Office (b) On The Floor (c) On Another Floor Of The Building And Away From Desk (d) Bomb Threats (e) Other Emergencies **All these subjects are covered in The Fox Theatre Emergency Manual.

5) PERSONS IN NEED OF ASSISTANCE i) Assign Personnel To Special Needs Persons On All Floors – Special needs patrons are

helped by the Fox Events Staff during performances. All Fox Staff are trained in assisting persons with special needs should that be necessary.

ii) If properly trained manpower is available, special needs personnel shall be escorted or carried all the way outside to safety areas

iii) Establish a list of all special needs occupants. Update as necessary and submit to The Atlanta Fire-Rescue Department. Maintain a copy in the building's Central Control Station.

iv) List Shall Contain The Following Information: (1) Building Owner/Manager's Name – Atlanta Landmarks, Inc./ Allen Vella(2) Address – 660 Peachtree Street NE, Atlanta, GA 30308(3) Emergency Telephone Number – 404 881 2047/404 625 9827(4) Handicapped Occupant's Name – N/A(5) Special Needs Occupant's Floor/Room Number – N/A

INTRODUCTION / STATEMENT OF PURPOSE

The Fox Theatre Emergency Procedures Plan is designed to provide basic guidelines, operational procedures and policies for Fox Theatre Personnel during emergency situations. The first priority is the safety of staff and patrons and protection of property.

This plan provides the basic administrative structures and procedures necessary to safely direct patrons during the following emergency situation:

. HOSTAGE SITUATION

. TERRORIST THREAT/ACTIVITY

. ELECTRICAL/POWER OUTAGE

. PLANE CRASH

. MEDICAL

. EMERGENCY EVACUATION

Note: Manager on Duty will be referred to as MOD through out this document.

The Emergency Procedures Plan is a tool which will be distributed to the various employees of The Fox Theater. This plan will be reviewed annually and, if warranted, updated accordingly by The Fox Theatre General Manager. Any recommended changes should be directed to the General Manager.

CHAIN OF COMMAND

The following chain of command will be utilized during all emergency situations at The Fox Theatre.

The highest ranking person on this list, present on-site, becomes the MOD (Manager-on-Duty).

1. General Manager2. Assistant General Manager3 MOD4. Director of Operations5. House Manager

Only the Manager-on-Duty, after gathering the necessary information and meeting with the appropriate officials, can order an evacuation. In an emergency, police and fire personnel may also order an emergency evacuation.

Immediately upon notification of a pending or ongoing emergency, the MOD will be notified and summoned, if possible, to the Events Office.

It is important that all parties involved in an emergency understand that the MOD is in command of all theatre emergency operations and all actions and decisions made need to be filtered through the MOD.

The MOD will continue to command until the emergency situation is entirely over or until the arrival of a higher ranking person, as listed in the chain of command.

Command Center

The 10-25 call sign will be used when it is necessary to go to a designated location.

In the event of an emergency the 10-25 emergency code will be used to summon the Emergency Management Team to one of the following locations:

Primary Command Center Events Office

Command Center Site #2 Bay 9

Command Center Site #3 Stage APM Office

The purpose of the Command Center is to establish a centralized location where the MOD along with the representatives of the APD/AFD and the appropriate department supervisors can meet during crises.

The person in charge will determine the command post location at the time of the emergency. The command center location is based on the type and location of emergency. If a location is not designated then go to Primary Command Center.

The following equipment will be made available at all three Command Center locations:

. Telephones (not requiring AC power)

. Two-way radios

. Emergency Procedures Plan

. Maps/diagrams of facility and grounds

. Note pads and writing instruments

Bullhorn Locations

To help with the communication process during an evacuation, bullhorns are located in the following areas:. Events Office. Bay 9. Production Office - Backstage . Egyptian Ballroom – Coordinators Office

The bullhorns in conjunction with the two-way radios (on channel 2) will act as the communication tools to announce an evacuation, as well as the ‘all call’ from the receptionist.

FIREPROCEDURES AND PREVENTION

One of the most important responsibilities of all employees is fire protection. Whether a person works in the office, in operations, as a guest services representative or in food and beverage, look for things that may be a fire hazard. Common sense should alert employees to dangers. A few of the more common hazards are:

. Frayed or unprotected electrical cords across aisles

. Large collections of trash

. Open trash bin covers

. Flammable materials

. Careless smokers

. Obscured or covered Fire Extinguisher and hoses

. Blocked exits which include fire doors, stairwells, hallways, etc…

It is the responsibility of ALL EMPLOYEES to immediately report all potential fire hazards discreetly to the MOD via the two-way radios or by dialing extension 2075/2047, or by sending a runner. Do not use the word “Fire” on the radios. Use code word: “Hot Coffee” if necessary

In addition to the above mentioned fire hazards, all fire suspicions should be reported immediately to the MOD. These fire suspicions could include the smell of smoke or smoke haze in the air, smell of gas, open containers which are marked toxic or flammable or reports by others that something is burning.

Prior to the beginning of their shift, all employees should know the location of the following items in the designated work area:

. The nearest fire extinguisher / fire hose

. The nearest telephone

. The nearest fire exits

. The nearest alarm-pull station (red box)

. The nearest first aid room

Staff Fire Procedures

Upon encountering a fire emergency, follow these guidelines:

If you discover fire or smoke. Warn and remove anyone in immediate danger.. If possible, confine the smoke or fire by closing doors (DO NOT lock doors).. If you have a two-way radio, inform MOD of the situation. Remain calm and state who you are,

where exactly you are located, and the nature of the emergency. Do not use the word Fire on the radio. Code word-“Hot Coffee”

Staff Fire Procedures (continued)

If you do not have a two-way radio, go to the nearest house or cell phone and call the Events Office at extension 2075/2047, or go to the nearest fire alarm pull station and activate the alarm. Remain calm and state who you are, where exactly you are located, and the nature of the emergency. Do not use the word “Fire” on the radio. Use code word-“Hot Coffee”.

. If a fire extinguisher is available and you have been trained in its proper use, take it to the location of the fire and if safe to do so, use it to extinguish the fire. Call MOD or 911.

. If you hear an alarm:

1. If time permits, lock down any sensitive or valuable items in your immediate area.2. Begin evacuation of yourself and people in your surrounding area by guiding them to the nearest

fire exits.3. As you are evacuating and helping others to evacuate, listen for further instructions over the

radio or public address system.

Departmental Fire Procedures

. MOD - obtain details of the incident and make necessary decisions. Prepare to activate the evacuation plan.

. Events Office: When determined by the MOD, evacuate all patrons as per evacuation procedures.

. Operations/Maintenance: assist in containing the fire or evacuating the patrons if possible

. Food and Beverage: If fire is located in a ballroom or concession stand, assist in containing the fire, if possible. Notify the MOD.

All other personnel: Listen for instructions. Remain alert to the danger, and be prepared to evacuate your area if necessary.

Fire Considerations

. Report the fire immediately using the proper method. If the fire is extinguished, no matter how small it was, it is still important that the MOD and Events Office are made aware of the incident.

. Close doors to contain the fire and smoke. Do not lock the doors.

. Stay low; heat and smoke rise. Hot air can scorch a person’s lungs and smoke may contain poisonous fumes that can cause death.

. Take short breathes and breathe through your nose. Cover your face with a piece of cloth to filter out smoke.

. Feel doors for signs of heat prior to opening. Do not open doors where heat is intense.

. See that the fire doors are closed, unless the door is being used for evacuation purposes.

. Always learn the location of the nearest fire extinguisher to your work area.

shortest safe route possible for evacuation

. Once outside move people away from the building and assign staff to take note of all present employees and locate any missing employees.

. Do not allow anyone to re-enter the building until clearance has been given by the Fire Department.

. Watch for falling materials or electrical wires.

. Do not use the elevators to evacuate.

SEVERE STORMS

TORNADOS/EARTHQUAKESOf all natural disasters that can affect The Fox Theatre, Tornados/Earthquakes occur suddenly, and may include widespread damage. It is during such a crisis that Fox personnel should present themselves with the proper demeanor and control.

Tornado WATCH Definition: Issued to alert the public that conditions are favorable for the development of tornadoes in and close to the watch area. These watches are issued with information concerning the watch area and the length of time they are in effect.

Tornado WARNING Definition: Issued by local NWS offices to warn the public that a tornado has been sighted by storm spotters, law enforcement or has been indicated by radar. These warnings are issued with information concerning where the tornado is presently located and which communities are in the anticipated path of the tornado.

GENERAL TORNADO SAFETY

The chances for survival are good if the public and staff remain calm. The following procedures should be implemented:

If Indoors - Stay indoors unless the order to evacuate has been given. Seek refuge under a desk or table or inside the auditorium or support areas of ballrooms away from windows. Stay away from glass windows, shelves and heavy equipment. Huddle against a solid interior wall with your arms covering your head. Adult bodies should shield infants and small children.

If Outdoors - Move quickly away from buildings, utility poles and other structures. Caution: Always avoid power or utility lines as they may be energized.

If in an Automobile – The Fox Theatre personnel using the company vehicle should stop as quickly as safety permits, but stay in the vehicle for shelter.

1. Stay clear of windows, tall cabinets, overhanging objects such as speakers, curtains or rigged items. Stay clear of areas with heavy false ceilings.

2. After the tornado has passed, evaluate the situation and if emergency help is necessary call MOD/Events Office at ext 2075/2047 or 911. Protect yourself at all times.

3. Encourage staff and guests not to run outdoors. They could be hit with falling objects or electrocuted by downed power lines.

4. Be prepared for additional unstable weather to follow.

5. Always go to Channel #2 and standby for instructions.

6. The evacuation procedure for tornados shall remain consistent with fire evacuation procedures.

7. In case of a tornado and there are guests entering the THEATRE, staff should direct people into the lobby and Spanish Room in an orderly manner and attempt to move as many guests and employees as possible into safe areas of the theatre

8. Do not use elevators in case of tornados/earthquakes.

SEVERE WINTER PROTOCOL

If winter conditions are not threatening but there is still the presence of snow, ice, or sleet in the surrounding area, the following precautions should be taken in order to avoid occurrences such as slip-and-falls in and around the building:

Clear the areas of the sidewalks immediately in front of the entrances to the building of any ice, snow, or sleet build-up.

Ensure that no snow, ice, or sleet is dragged into the building by placing mats down inside the building at entrance point.

Keep an eye out for puddles or any wet areas that may result from wet shoes and immediately dry them once notified. Also use adequate signage or cones to warn people who may traffic through any wet area.

In the case of an extreme snow storm, or in situations where severe winter conditions hinder Fox Theatre employees from getting to the actual venue on event and non-event days, certain assigned employees must be present regardless in order to take calls, assist any patrons who may arrive, and inform fellow employees of any threatening changes the weather may take.

Those employees who are able to get to the theatre will have the responsibility of contacting their respected colleagues and inform them of the situation at hand. Refer to the Chain of Command/Phone Call Tree under Section III of this manual for guidance on who to call in severe winter conditions.

Roles for Most Affected Departments

Ticketing Contact: Sunshine Tucker or Rachel Bomeli will contact the ticketing company, phone center, and

outlets regarding instructions to ticket buyers.

III. Food & Beverage Department Point Person- Jay Forrester Any F&B part-time staff that can safely arrive will be asked to do so. Generally, about the

same percentage of staff as patrons will arrive in rough weather. Staff will be appointed positions as they arrive.

alternate land line). If land line is the method of communication the staff member should update the supervisor every two hours until the situation is handled.

Responsibilities should include:

Equipment operation and monitoring of the building. Maintain sidewalks within reason and keep a clear path from the street to the stage loading

dock. Inspect cooling towers and roof.

VII. Housekeeping: Point person: Shirley Allen Two housekeeping staff members should be on site if they can work safely. Primary mode of communication should be via telephone. Staff members should stay in

constant communication with security or maintenance. Responsibilities should include:

March 2012 Manage any water that might accumulate throughout the building from melted snow or ice Routinely check on the bathroom’s tiled floors that are more susceptible to slippery

wet areas Rope off the Arcade down to a single lane with rain mat for safety of patrons. Assist maintenance with spreading of ice melt as required. Assist with monitoring the

conditions of the building.

VIII. Security: Point person – Tammy Folds. Mandatory staff - should be at least one person on site. Communication– cell phone or alternate land line.

Responsibilities should include: Assist with monitoring of sidewalks and entrances. Maintain building safety and security.   Maintain contact with security supervisor and coordinate observations with maintenance and

housekeeping.

Any person found to be lost (young or old) should be taken to the Fox Theatre Box Office and the Front of House Manager notified. Only if necessary, an announcement will be made to the audience and the entire building.

If we have a missing person:

1. Immediately obtain a description- Name - Sex- Age- Height- Hair Color- Clothes- Area last seen

2. All staff quickly sweeps entire buildings that we have access to: lobbies, backstage, dressing rooms, restrooms and offices

3. If necessary, coordinate with Stage Manager to stop the show and make an announcement

4. Call Public Safety Communication -- Dial 9-1-1 follow the 911 procedures

5. Continue assisting the client/ family/ and emergency response team and the part time staff will check the security feed.

BOMB THREAT/SUSPICIOUS OBJECT

The potential for bomb threats exists at all businesses and facilities. Even though very few bomb threats actually involve the placing of a bomb, all threats must be taken seriously.

Almost all bomb threats are received by telephone, generally through the switchboard. Included in this plan is a form to be completed when a bomb threat is received by phone. The form is located at the end of this section.

This form will assist in evaluating the threat and possibly the identity of the caller. The form should be immediately available and the telephone reception desk staff must become familiar with its format IN ADVANCE of a potential bomb threat emergency.

Staff Bomb Threat Procedures

1. REMAIN CALM!

2. If time allows, try to be friendly and congenial with the caller. DO NOT attempt to be insulting, angry, or argumentative.

3. Encourage conversation in order to keep the caller talking as long as possible and ascertain as much information as possible by following the bomb threat checklist.

4. If you are at a location that has a panic button, immediately press the button. Also, discretely summon the MOD by radio. DO NOT say the word “bomb” over the two-way radio. Calmly say that you need immediate assistance in your location. Continue to try and keep the caller on the line while collecting as much information as you can. If another person is with you, send them to advise the MOD.

Department Bomb Threat Procedures

Security: Notify the MOD of the bomb threat emergency by telephone or runner. Try to avoid using two way radios.

MOD: Immediately notify the APD Officer on duty, and House and Production Managers. Discreetly create a Command Center. Activate department bomb search teams and bomb search procedures as required. Begin evacuation assessment.

When necessary, employees should be made aware that a bomb search is in progress, if the search is in their designated work area. Employees are often the best qualified in detecting suspicious packages which may be present in their work area. Upon the discovery of a suspicious package or device, all employees and guests must be removed from the immediate area. The area must be kept clear pending the arrival of the Police Department. Prepare for Evacuation.

Events Office: If directed, assist in the search of public areas.

March 2012Operations Department: Search boiler room, main electrical room, etc. Assist in the search of restrooms, underneath seating areas, supply rooms and offices.

Stagehands: If directed, search backstage areas, cable runs, and catwalk.

Box Office/Administration: If directed, search offices.

Food and Beverage Supervisors: Search all food preparation areas, bars, and storage areas.

What to look for in a search:

Unfortunately, there are various types of containers which may conceal a bomb. One must look for unusual are seemingly out-of-place items (if the bomb has been described in size or type, this may reduce the need to be suspicious of every item). Look for items which do not seem to have any real purpose or things that were not there before and do not have any functional purpose.

In the theater, look for items such as briefcases or parcels, which can be hand-carried into the building without suspicion. Also look for abandoned travel/gym bags, boxes or other articles which are abandoned or left unattended.

DO NOT open or touch a suspicious package. DO NOT BROADCAST via your two-way radio units near a suspicious package. You may continue to receive messages, but do not attempt to broadcast. Use a nearby phone, if available.

If a suspicious package is found:

It is imperative that personnel involved in a search be instructed that their only mission is to search for and report suspicious objects. Under no circumstances should anyone move, jar or touch a suspicious object or anything attached to it. The removal or disarming of a bomb must be left to the professionals in explosive ordinance disposal. When a suspicious object is discovered, the following procedures should be followed:

Immediately notify the MOD and the security personnel, who will in turn notify the Police / Fire Departments.

It will be the responsibility of the MOD and the Police / Fire authorities to decide, if there is a need for an evacuation.

. Upon locating a suspicious package, since some bombs or devices are activated by radio signals, all two-way radio transmissions will be prohibited in the immediate area of the suspicious package/device. Notification must be made by word of mouth or telephone. All personnel must use discretion during this period if the FOX THEATRE is in public use to avoid panic. Specifiedmembers shall report to the command post for instructions to be given by the MOD.

If the decision to evacuate is made and announced:

1. Begin evacuation procedures as outlined in the Emergency Evacuation section of the plan.

March 20122. Report the location and an accurate description of the object to the command post by telephone or

runner. This information should be relayed immediately to the police and fire departments by the MOD. Officers should be met upon arrival and escorted to the scene.

3. Do not attempt to cover the object.

4. Check to see that all doors and windows are open to minimize primary damage from blast and secondary damage from fragmentation.

5. Do not permit re-entry into The Fox Theatre until the device has been removed or disarmed, and The Fox Theatre has been declared safe for re-entry.

“All Clear” Signal

After The Fox Theatre is determined to be secure of any problems, the MOD will broadcast an “all clear message” via radio channel 2. This message will reach any person equipped with a Fox two-way radio and on channel 2.

If Detonation Occurs

Detonation of a Bomb:

1. As soon as possible, emergency team members should report to the command post. The Atlanta City Police Department and The Atlanta City Fire Department should be notified immediately.

2. Depending on where the device is detonated, there may be a stampede by guests attempting to leave the area. Panic may rule for the moment and staff members should stay clear of the crowd flow. To avoid a pile up of people, staff should attempt to open and hold back as many doors as possible. Persons who have fallen should be removed out of the avenue of traffic flow.

3. Even though evacuation of the affected part of The Fox Theatre will probably be spontaneous, the Command Center will still make the emergency announcement of evacuation.

4. The command center should send an emergency team member to the damage areas as soon as possible to assist injured guests and staff members since it may be some time before support services arrive. Staff should be prepared to face a grim situation that may include dead bodies in states of dismemberment and severely injured guests and employees crying out in pain. Emergency team members should inform the command post of the status of the damaged area to aid support services. Do not key two way radios if the smell of gas is present.

5. If there appears to be danger of fire in the damaged area, emergency team members should begin evacuation of all injured persons. Since time may be of the essence and support services may still be in route, personnel should utilize all methods of transportation such as hand braces, blanket drags, and maintenance carts and motorized vehicles that may be driven to the damaged area.

6. Security personnel, to avoid public re-entry, should cordon off the damaged area; non-essential personnel should not be in the damaged area and should be sent out of The Fox Theatre. All personnel shall report to their designated staging area where area leaders will verify complete evacuation.

7. The MOD should confer with the police and fire department and consider transfer of authority.

March 20128. Recovery operations should include staff members once police and fire department have cleared the

area and released the facility back to the MOD.

Mail Bomb Threat

Mail bombs have been employed against individuals and organizations for the purposes of revenge, extortion and/or terrorism.

Remember, the physical appearance of a mail bomb is limited only by the imagination of the bomb maker. However, mail bombs have exhibited unique characteristics, which should be useful in identifying a suspect package. Some of those unique characteristics include but are not limited to the following:

1. Mail bombs have been contained in letters, books and parcels of varying sizes, shapes and colors.2. Letters feel rigid, appear uneven, lopsided or are bulkier than normal.3. Oil stains may be present on the wrappers.4. Use of excessive amount of postage stamps.5. Sender is not identified.6. No return address.7. Unusual restricted endorsement such as “personal” or “private”.8. Addressee normally does not receive personal mail at the office.9. Name and title of addresses/addressee are not accurate.10. Sender is prepared to ensure anonymity (i.e., homemade labels, cut and past lettering).11. Mail emits a peculiar odor.12. Mail appears to be disassembled or re-glued.13. Handwriting appears distorted or foreign.14. Protruding wires, foils or string are present.15. Pressure or resistance is noted when removing the contents.16. Outer container is irregular or asymmetric, has soft spots, or bulges.17. Wrapping will exhibit previous use, such as traces of glue, mailing labels, return addresses or tape.18. Several combinations of tape are used to secure the parcel.19. Unprofessionally wrapped parcel is endorsed “Fragile Handle with Care” or “Rush Do Not Delay”.20. Box emits any type of noise.21. Contents of parcel make a sloshing sound.

If Mail Is Believed To Be a Suspected Bomb Device:

1. Do not use two way radios to report message.2. Do not open the article.3. Isolate the letter or package and secure the immediate area.4. Do not put the item in water or a confined space, such as a desk drawer or filing cabinet.

A terrorist threat is a general threat against a person(s) or property primarily motivated by a desired political effect or a specific outcome.

. Wait for the arrival of the Atlanta City Police Deparment.

. Cooperate fully with the directions of the responding Atlanta City Police Department.

. It is possible that the terrorist may first seize the Events Office to cut off communications and use the area to dictate their terms. If this should happen, the command post will shift to another location.

March 2012. Any staff members who are in an area where gunfire takes place should lie down and encourage all

others in the area to do the same. Running away may cause you to become a target.

. Security should remove guest and VIP’s, if possible, to a safe area or out of the facility.

. Staff members who are taken hostage by a terrorist should cooperate, as the situation may only be temporary.

. In the case of an assassination or attempted assassination, personnel should give the perpetrators a wide berth as they begin trying to exit The Fox Theatre and take guests and staff members along as a shield. Once again, non-resistance may be the desired behavior.

. Perpetrators who are allowed to leave The Fox Theatre with hostages will probably make their demands known by telephone call or letter to The Fox Theatre or through the media.

. If and when an incident occurs before police support services arrive, the police department should be informed of the current status of the problem by telephone

ACTIVE SHOOTER

1. Remain calm. Call 911 immediately. Give detailed description of shooter and where in the building the shooting is happening.

2. Get as many people out of the area and in secure locations, preferably outside or areas that you can lock them in, i.e. dressing rooms, offices, etc….

3. If there is an event going on in the other theater, try to get over to it and secure them by locking them in and making everyone aware to stay put for their safety.

4. Try to be aware of what is happening and offer assistance to emergency personnel. You know the building better than they do so offer what you can.

5. Offer comfort to those who need it and contact a supervisor as soon as possible.

6. Note: You will be expected to recount every detail of what happened so when you get a chance, write down or record everything you remember.

**Be aware that media, family members and supporters will be arriving soon. Keep them out of the building as the theater will most likely need to be locked down for further investigation.

POWER OUTAGE

Electrical outages can occur, at any time, and for any number of reasons. When the electricity fails, the emergency back-up generator is scheduled to provide emergency power. It takes less than 10 seconds to activate the emergency back-up generators. They will provide enough light to allow patrons to leave safely, in case an event is in progress.

Departmental Power Outage Procedures

MOD: Ascertain the problem and possible impact in order to make decision regarding the continuation or cancellation of the event in progress.

Security: Stand by to make announcements based on the MOD’s decisions. Prepare to activate building evacuation if necessary. If the decision is made to evacuate the facility, follow the emergency evacuation procedures outlined in this plan.

If the problem cannot be located and repaired within a reasonable length of time and the decision is made to cancel the event, the announcement to evacuate the facility will be made over the PA system along with the information regarding ticket refunds, rain checks, etc.

Operation/Maintenance:

Notify the MOD and the Events Office of the cause of the power outage.

Continue to provide updates and progress reports to the MOD and the Events Office.If external, contact Georgia Power at (1-888-891-0938) to determine estimated length of the power outage. If internal, locate and initiate prompt repair. Additionally, the Director of Operations or Lead Engineer will be responsible for periodic testing of the back-up generator and the maintenance of test records, stored in the engine room.

Guest Services:

Attempt to keep guests in their seats for their own safety.

Operations/Box Office/Administration/Stagehands: Remain in your areas for further instructions.

Food and Beverage: Secure all equipment (i.e. refrigerators, freezers, cash registers, etc.) starting with cash registers in case of damage due to power surges. Await further instruction.

Power Outage Announcement Text

If the lights go out during an event, the production office, events office, and ballroom staff will make the following announcement to their respective areas:

“Ladies and Gentleman, for your own safety and the safety of those around you, please remain where you are. The emergency back-up generators will continue to provide power. The main lights will be restored as soon as possible.”

HAZARDOUS MATERIAL

Hazardous materials are those substances which may represent one of the following hazardous material classes:

. FLAMMABLE (gasoline, cleaning solvents, etc.)

. EXPLOSIVE (fireworks, T-N-T)

. RADIOACTIVE (medical, isotopes, radium, etc.)

. COMPRESSED GAS (oxygen, nitrogen, etc.)

. HIGHLY REACTIVE (chlorine, phosphorous, etc.)

. SEVERE HEALTH HAZARD (poisons)

. CORROSIVES (batteries, acids, etc.)

All hazardous materials stored or used at the FOX THEATRE will have an appropriate sign displayed indicating the substance, safety precautions, and appropriate emergency actions to be taken in the event of an accident. In addition, all chemicals used by the FOX THEATRE will have material safety data sheets (MSDS) on record in the Operations Office and all containers shall be labeled accordingly. These material safety data sheets provide first aid recommendations in case of injury.

Emergency Hazardous Material Procedures:

1. Immediately clear the area and close doors.

2. Notify the MOD.

3. Ensure that all persons are moved upwind of the spill. Operations shut off ventilation for effected space.

4. If possible, contain the spilled material through the use of such material as sand, sick sorb, or other suitable material (only if the material is non toxic).

5. DO NOT wash hazardous materials into storm or sewer lines.

6. Small spills may be cleaned by qualified persons. Large spills of toxic materials require special training and equipment possessed by commercial clean-up companies. The Manager-on-Duty will determine if outside help is required in the clean-up process. If outside help is needed the MOD will notify 911.

7. In the event that a chemical has splashed in the eyes of guests or employees notify the nearest EMT or Fireman for help.

CIVIL DISTURBANCE/RIOT

Disturbance

A civil disturbance might include guests engaged in a fight inside the theatre, ticket scalpers congregating outside the box office, labor strikes, political or religious demonstrations, or any other protest activities. Sometimes these actions are minor in nature, while others may require more forceful action, such as security intervention and/or police arrests.

Employees of The Fox Theatre may be required to enforce minor laws such as trespassing or warning others about unlawful activities. An example of this would be stopping a person from interfering or bothering other guests as they peacefully participate in scheduled events. If they continue, the problem will then become the concern of the events department. If it escalates, security should be immediately notified via the two-way radios or by dialing extension 2075/2047.

RiotThere are many factors that determine what action to take should a riot occur. These include whether the threat is inside or outside the building, the number of persons involved and whether it is before, during, or after an event. In such cases, the protection of guests, performers, VIP’s and employees are primary considerations.

Call APD first if able. Isolate the people and/or groups causing the disturbance by moving other guests away. If the lobbies are involved, keep guests moving. DO NOT let guests stand and watch. In a hostile mob situation, do not enter the mob area unless it is necessary to do so in order to save someone from injury by mob participants. Beware of thrown items and remember there is a possibility that weapons may be present.

If the disturbance escalates into a full scale riot while an event is taking place, the MOD will make the decision to turn up the lights and take action to move the entertainers or teams to the safety of their dressing rooms.

Allow any guests, who wish to do so, the opportunity to leave the building. If the decision is made by the MOD, in consultation with the entertainers or teams, to cancel the event, evacuation should be carried out in accordance with policies stated in this manual. Once the building has cleared, lock all doors.

If the disturbance occurs on the outside grounds of the theatre and the event is underway, call APD, secure all doors into the theater. This keeps the disturbance/riot participants outside. If guests are aware of the problem, attempt to calm them with an appropriate announcement on the PA system.

March 2012Departmental Disturbance/Riot Procedures

MOD: Determine the extent of the disturbance through information gathered and meetings with the appropriate police/fire authorities.

Security: Respond to the scene of the incident. Keep the MOD informed of the activities. Move the crowd away from the scene. If possible, form a line around the scene to keep it isolated. If APD has been called to assist, keep aisles clear to allow them to reach the scene promptly. Once APD personnel arrive, assist them as directed. Stand by to activate other relevant emergency response procedures.

Guest Services: Move guests away from the scene. Encourage them to return to their seats and assist them in heading in the right direction.

Operations/Maintenance: Stand by ready to assist MOD.

Administration: Staff: Should report to and/or remain in their offices until the situation is under control or otherwise directed by a supervisor, security or the APD.

PLANE CRASH

Although a plane crashing into The Fox Theatre is highly unlikely, we are in close proximity to flight paths and must be prepared for this type of emergency.

Staff Plane Crash Procedures

In the event of a plane crashing into the building, the two immediate problems will do to quell the panic and to treat the injured.

The public announcement system should be utilized to calm the crowd. The injured who are mobile should be assisted/directed the designated triage area. Those too seriously injured to be moved should be made as comfortable as possible until medical assistance arrives. Evacuation should be started as quickly as possible following the evacuation procedures established in this document.

Departmental Plane Crash Procedures

MOD: Instruct first aid teams to respond to the crisis. Start the evacuation of the building as soon as possible.

Security: Call 911. Initiate building evacuation procedures.

First Aid: Be available for triage, if conditions prohibit, at the designated meeting area located south of the FOX THEATRE on Ponce de Leon in the parking lot, or north of the FOX THEATRE in the AT&T parking lot.

Operation/Maintenance: Check building for damage. Keep the MOD informed.

Remaining Staff: Proceed with evacuation as established in this document, when ordered.

MEDICAL EMERGENCIES

The intent of the section is to provide basic guidelines for handling medical emergencies ranging from minor to serious injuries or death.

The Operations office should maintain a current list of those employees trained in first aid or CPR. Basic first aid and CPR information and guidelines will be posted in common-work areas.

Emergency Medical Procedures

Medical emergencies/injuries are classified into three categories:

1. Minor2. Serious/non-life threatening 3. Serious/life threatening

Minor injuries are those which can be handled by basic first aid procedures and do not require immediate additional medical treatment or transportation.

Serious/non-life threatening emergencies/injuries are those which require medical attention beyond the first aid which can be provided on site. These injuries do not require ambulance transportation.

Serious/life threatening emergencies/injuries are those which require immediate ambulance transportation to a medical facility. These include, but are not limited to, individuals with chest pains, breathing difficulties, severe bone breaks, back injuries, severe cuts with uncontrolled bleeding, unconscious persons, labor pains for pregnant guests, and severe head injuries.

Staff Medical Emergencies Procedures

. With minor injuries, notify your supervisor and the MOD. Take appropriate first aid measures.

. With serious/non-life threatening injuries take immediate action and appropriate first aid measure. Notify a supervisor and the MOD as soon as possible. Call 911 if necessary.

. In case serious/life threatening injuries, call 911 take immediate action by notifying the MOD. Make sure to state the exact location of the injured person and the severity of the injury. During all events, there will be an EMT on-site. If trained, provide immediate first aid and/or CPR measures as needed.

. A security officer or another employee will meet the incoming ambulance at the closet building entrance or location where the injured person is located.

. DO NOT ATTEMPT TO MOVE injured individual(s) unless his/her life is in danger at that location. Upon arrival of the ambulance, be prepared to provide any needed information to the Paramedics/EMT.

. Make sure to note which hospital/clinic the injured person is being transported to in order to assist with his/her family notification. No employee shall interfere with medical treatment provided by the ambulance or medical team.

. If an individual is discovered who is “obviously dead” ALL guests in the immediate area will be cleared from the area but asked not to leave until released by a law enforcement officer. The deceased individual will NOT be covered nor the surrounding area altered in any manner. The area should be closed off by security. If the deceased is in public view, the body may be covered at the direction of a law enforcement officer.

Additional considerations and medical emergencies:

1. Whenever possible, do not leave an individual requiring medical assistance alone.

2. If you are dealing with a conscious adult who refuses assistance, make annotation on the incident report.

3. If you are dealing with a minor, obtain their name, age, and the name and phone number of his/her parent(s) or legal guardian(s).

4. Make sure a written record of the location to which the injured person is transported is made to assist in family notification. This information is critical in the event of multiple casualties. Report this information to the Command Center.

Emergency Supplies

The Fox Theatre has 2 Automated External Defibrillators (AED) located in the Events Office and Stage Security area.

All defibrillators should be used only in case of emergency and by staff trained in its proper use.

EVACUATION PROCEDURES

Evacuation in an orderly manner is a necessity in order to assure that people vacate a dangerous area as quickly as possible. FOX THEATRE employees not only have to care for themselves, but must see that guests are guided from the Theatre to safety. FOX THEATRE employees should be the last to evacuate the facility.

It is the responsibility of all The Fox Theatre employees to understand and make themselves familiar with the Evacuation/Response plan. To assist each employee with evacuation routes, please refer to the already distributed floor plans kept at each department.

Emergency Evacuation Procedures

Command Center

The purpose of the command post is to establish a centralized location where the MOD along with representatives of the APD/AFD and the appropriate department supervisors can meet during crises. The command post should allow for an efficient communication and decision-making process.

Primary Command Center

The primary command center location during events is designated as the Events Office located on the event level, adjacent to the main theater lobby.

Secondary Command Center

If the Primary Command Center becomes disabled, damaged, or unsafe, the alternate locations in order of priority will be the following:

1. Bay 9 located on the Administrative Hall, at the Ponce de Leon Administrative exit.2. Assistant Production Manager’s office backstage, on Ponce de Leon near the stage door.

The following equipment will be needed at the Primary and Secondary Command Centers:

Re-Entry

The purpose of re-entry into the theater is to allow guests or employees who have been evacuated to return to their seats or offices for either the resumption of the event or work day, following the elimination of the emergency situation which precipitated the evacuation.

In some instances, if the emergency situation which necessitated the Evacuation/Response no longer exists, or was precautionary in nature, patrons/employees will be allowed to return to their seats/offices and the event/work day will resume.

Prior to any re-entry, the theatre must be declared safe by the MOD and/or the representatives of the Fire/Police Department. A thorough inspection of the facility must be made before any re-entry is allowed.

During events, all The Fox Theatre staff must be in position and at their posts, before the patrons are allowed re-entry.

Employee Evacuation

There are work areas in the FOX THEATRE such as administrative offices, ballrooms, Joe Pattons’ apartment, Food & Beverage, back stage, and the FOX THEATRE where employees work on a regular basis and are visited by vendors and/or contractors and patrons/guests. All employees should be familiar with the evacuation routes; for their respective areas. In addition to knowing the evacuation routes, department heads should ensure the following are in place:

Should an event occur which necessitates an evacuation; an immediate head count should be taken of everyone who successfully evacuated the theatre and is present outside. Those who may be absent from the head count must immediately be brought to the attention of the emergency response personnel at the scene.

The evacuation routes should be planned in advance. For those employees who are normally in a stationary work area, evacuation routes should be determined in advance, with a second or third route established in the event the quickest evacuation route is blocked or not accessible.

If possible, small offices or areas should evacuate together at the same time. A quick head count shall be made all persons should leave together helping those who may need help or may be physically challenged. Always be sure to check side rooms or restrooms for someone who may not be aware of the hazard, or that an evacuation is in progress. This should be done by either the department supervisor or assistant.

March 2012All evacuated persons shall gather at a predetermined place outside the building. The predetermined gather place shall be on the south side at the parking lot on Ponce de Leon and on the north side in the AT&T parking lot. A head count shall be taken and no one should leave this area until they have been accounted for and released by their supervisor.

DO NOT attempt to enter the building without permission. The MOD will notify employees when it is permissible to re-enter the building.

When the decision is made to evacuate the theatre, the following information will act as basic guidelines for directing employees to the nearest exit. Exits are identified on the map with #’s.

Evacuation Routes during Events

The MOD, in consultation with APD/AFD, decides that an evacuation is necessary, establish a command post and then communicate to the staff, through the use of two-way radios, the decision to evacuate. At this point all non-essential radio communications shall cease. Once the staff is notified, the public will be informed of the decision through the use of the Public Address system. Additionally, evacuation instructions shall be given over the PA system.

Departmental Emergency Evacuation Procedures

The following roles for employees are not meant to limit initiative, but to serve as a basic guideline. Knowledge of the facility and your work area is the key.

Security and Events Office: Direct the guests from the facility as quickly and efficiently as possible following the evacuation plans (see emergency evacuation plan diagrams). If any of the exit routes are blocked, you must re-route guests to the nearest alternate safe exit. Assist injured to the first aid area.

It is recommended that injured guests and guest with disabilities be directed to a location where they can be further assisted to safety by trained staff. Supervisors/Event Management: Communicate evacuation decision to personnel not within hearing range of public announcements. Assist security and guest services with crowd control; assist injured to first aid room. Make final sweep of theatre to assure that all guests and employees have exited; lock or close off areas as they are cleared.

Once the facility is clear of all non-essential personnel, supervisors should either lock or maintain watch at the exits to make sure unauthorized persons do not re-enter.

Operation/Maintenance: Report to events office or command center for evaluation of needs in areas of power, lighting, etc. Control other services such as gas, water, etc., for damage reduction and safety.

Check structure integrity, broken concrete, frayed or broken support cables, check emergency generators for proper operation and fueling, secure all unnecessary equipment if conditions warrant.

Monitor all piping and electrical systems, isolate and secure any sections that indicate damage. Check the elevators and escalators for operation and alignment. Make sure a set of building plans are available for reference by the Police and Fire Departments.

Box Office: Stop sales and close all windows. Secure money in safe, secure ticket stock, turn off computers and electrical equipment if it can be done without endangering staff.

Administration: If possible, secure computers so information will not be lost. Follow evacuation procedures as outlined in this document and report to assigned staging area.

Stagehand/Production: Secure any equipment with which you are currently working and report to the production office for instructions.

Sound Technician/PA announcer: Stay with sound system as long as it is working to inform guests. Announce the prepared evacuation statement as requested by the MOD. Everyone report to assigned staging area.

Food & Beverage: Secure cash and receipts. Secure products if it can be done quickly and without endangering staff. Everyone report to assigned staging area.

Housekeeping: Unplug equipment and assure it is not blocking exits. Secure harmful chemicals before leaving the area. Everyone report to your assigned staging area.

Ballroom staff: Assure that all exits area clear. If it can be done safely, power down lights and sound systems. Check restrooms, store rooms, and kitchen to assure that everyone is out. Everyone report to your assigned staging area.

General Evacuation Announcement Text:

“Ladies and Gentlemen, attention please”

“Your attention please, for this important announcement”

“We ask that you follow these instructions in a calm and orderly fashion”

“Due to an emergency in the theatre, we are required to ask you to evacuate the building”

“Please follow the instructions of the Fox Staff Members in your section as they will guide you to the nearest exit”

“Thank you for your cooperation”

Theatre Evacuation Routes during Events

When the decision is made to evacuate the theater, the following information will act as basic guidelines for directing patrons to the nearest and safest exit.

When directing guests in an evacuation, employees should keep in mind that there may be conditions (i.e. falling debris, blocked exits, etc.) that may prohibit them from following these guidelines. When this occurs, employees should use their best judgment in guiding guests to the nearest alternate safe exit.

For all events, guests seated in the upper concourse sections should be directed to the nearest and safest exit. If possible, direct guests to evacuate in the following manner:

Gallery & Second Dress: Exit right or left out the side exit doors (for fire). Loge & First Dress: Exit right or left out the side exit doors (for fire).

All Balcony areas: Exit down center portals areas when the call is given for interior movement

Orchestra Level

For all events, guests seated in the orchestra should exit to the nearest and safest exit.

Shelter in Place Announcement

During severe weather and tornado warnings, the following announcement will be made:

“Ladies and Gentlemen, your attention please”

“Your attention please for an important announcement”

“A tornado warning has been issued for our area. For your safety and the safety of others please do not

attempt to open any interior doors until an ALL CLEAR signal has been given”.

“Thank you for your cooperation”.

ATLANTA FIRE DEPARTMENT EMERGENCY ACCESS

In event of an emergency that requires the Atlanta Fire Departments response, the responding company would report to the fire location based on information from the MOD. The Fire Department personnel may also enter the theater via the loading dock.

In the event the emergency prohibits Fire Department personnel from entering the Fire Control Room or the loading dock, entrances located on Ponce de Leon and the AT&T parking plaza are equipped with entrance doors that would provide the Fire Department access directly into the theater. Important: The AT&T plaza will not support the weight of vehicles. All vehicles must stay on the paved parking area.

TWO-WAY RADIO GUIDELINES(During Emergencies Operate on Channel 2)

. During crises it is important to remain calm and to stop all non-essential radio communication. Do not shout into the radio as it will distort your voice and your message will not be understood.

. If the fire alarm sounds, change your radio to channel #2and listen for instruction.

. When transmitting a message via the two-way radio, hold the radio approximately 3 inches from your mouth. This will promote a clear transmission. Wait one second before talking.

. Radios assigned to managers should be programmed to receive Emergency channel #4.

. If you need security personnel to respond to your location, give your exact location and a brief explanation of your request so that the security personnel can prioritize their response. Note: under some circumstances i.e., bomb threat, simply request that security meet you at a designated location immediately.

. Please remember that the two-way radios are different from phones so try to be brief with your message. The best thing to do is think about what you are going to say and to word a message in your mind before you go on the air.

. If you have to change your channel to speak to someone in another frequency, don’t forget to go back to your own channel after you’re done.

. Check to assure that another conversation is not in progress before you start talking.

. When you first plug in an accessory to your radio, i.e. a hand mic or head set, always turn your radio off and then on so that the radio resets itself and recognizes the new accessory attachment.

It is the responsibility of all Fox Theatre employees to understand and make themselves familiar with the Evacuation/Response plan. To assist each employee with evacuation routes, please refer to the floor plans that are readily available for viewing at each department.
