Where does the process start?

Patients do not have to be on dialysis to qualify for kidney transplantation. We prefer to evaluate and educate patients early in the course of their disease, thus providing the opportunity for preemptive transplantation.
• Stage 4 (GFR <30 ml/min)
• BMI: We accept all referrals for BMIs <38. We provide BMI education for those who do not meet criteria.o BMI equal to/less than 38 prior to scheduling for evaluation.o BMI equal to/less than 35 prior to renal transplantation. o BMI equal to/less than 30 prior to combined Kidney and Pancreas transplant.
• Absence of a systemic illness or medical condition such as severe cardiovascular or peripheral vascular disease, dispersed malignancy, etc.
• Ability to be compliant with a complicated long term medical treatment regimen. Absence of active drug and alcohol abuse.
• Present strong social/family support system.
• Adequate financial plan to obtain the required long-term immunosuppressive medications needed for post-transplant.

Eligibility Criteria
Emory accepts referrals from dialysis centers, MD offices, and patients who want to self refer.
Required documents with referral:
Primary Insurance cards (front and back copy)
Secondary Insurance Cards (front and back copy)
Form 2728
Recent History and Physical (within 6 months)
• Hospital discharge summary
• Admission H&P
• Last office visit note
Recent Labs (within 3 months)

A fax confirmation is sent back to referral source.
Patient’s are contacted in 2 business days once the referral is completed.
The patient, dialysis center, and/or the referring MD can call the main line to get a status update on the patient.

T-REX: Transplant Referral Exchange
Purpose• To improve access to kidney transplant in the Southeastern United States by:
o Enhancing communication about patients shared across healthcare settingso Providing a platform to electronically manage kidney disease patients at all steps of the transplant process
Collaborators: Apex Health Innovations
Key Features• Real-time communication between staff across healthcare settings• Documenting the use of transplant education materials• Sending/receiving an electronic referral specific to a transplant center’s requirements• Tracking patients’ status in the transplant process (e.g., evaluation completion, waitlist status,
living-donor matching capability)
Emory Transplant Center was first in ESRD Network 6 to pilot test T-REX application.• Pilot testing time period: April 1, 2017 – April 1, 2018
• However, use of T-REX at ETC is STILL ONGOING to receive referrals from Emory Dialysis Clinics
Results of T-REX Pilot Testing at ETC: • High usability and adaptability among dialysis facility and transplant center staff
• T-REX could help streamline efficiency of the transplant process by:
o Enhancing communication between healthcare providers
o Collecting data at each step in the transplant process

Our team has an individual group solely dedicated to kidney transplant referrals. They are the ones who receive referrals, send confirmations, contact the patients for scheduling, etc.
Once the referral is received, you will receive a confirmation letter sent back to the fax source. If you do not receive this, please feel free to reach out to someone at the transplant center. You are more than welcome to utilize my contact information for questions like this. [email protected]
This is the point where the patient is now involved in this process. The referral team will call the patient to attempt to schedule the evaluation. The patient will need to have a voicemail set up in the event they miss the call. This allows the referral team to provide contact information for a call back.

Waitlist Process
After completing the evaluation process, the patient will be scheduled for selection conference. This conference occurs every Wednesday at 1:00 PM. This group involves a multidisciplinary team.

The patient will be made Not a Candidate (NAC) due to medical, financial, social support, psychosocial, etc.

All patients who are listed ACTIVE need to send in monthly kits.
Once a patient is listed active, the kits will be mailed a few weeks after.
Please feel free to print and post this flyer in locations that would benefit.
If your patient did not receive a kit and you believe they are active onthe transplant list, please send a message to the email below with the
patients name and DOB.
Transplant Options
I have a donor, now what?- We have an online questionnaire screening at emory.donorscreen.org
I have an approved donor, but not a match…- Emory participates in a organ donor swap called Paired Donor Exchange with National Kidney Registry. The organ travels, not the patient.
HepC+ to HepC-- Emory performs HepC+ organ transplants to patients that are HepC-. Once the transplant is completed, the patient will be treated for the HepC+.
HIV (HOPE Act)- Emory performs HIV to HIV transplants on the waitlist.
Monthly Patient Status Report Sent via fax to all dialysis centers on the last business day of the month.

Referral ConfirmationOnce a referral is faxed into the transplant center, a confirmation letter is faxed back to the referral source.
Selection ConferenceOnce a decision is made for a patient during selection conference, the patient is called the same day. A letter is also mailed to the patient.
A letter is mailed and faxed to the provider and dialysis center (if applicable) with the outcome of the conference.
Day to DayWhen the patient comes in for an evaluation, they are provided contact information to use if they have questions, to provide updates on personal information, and/or to speak with a coordinator regarding status. Most processes we have in place cover each of the concerns or updates that patients have.
