EMPOWERMENTIS A BRIDGE TO

COMMUNITY.SECOND CHANCES INCTransition Services for Independent Living

Services

M any people with disabilities who are fully capable of living independently are stuck in nursing homes, held

back from independent living by a combination of factors. Coming up with a first month’s rent and deposit is difficult if not impossible for people with limited resources. Even more daunting is not knowing where to turn for assistance and information about finding accessible, affordable housing. Transitioning to indepen-dence can seem like an impossible dream. Second Chances can help. We are a Transition Services Agency providing financial, educational, and life skill counseling services for people with disabilities who are mentally and physically able to live independently. We help people deal with all aspects of transitioning from nursing homes to independent living situations. Our counselors will visit clients in nursing homes and offer advice and training in:• How to budget money and pay bills on time• How to arrange for transportation to appointments• How to deal with CNA and nursing personnel in a positive

way to receive home health care With independence comes responsibility, and we make sure our clients are aware of all that will be expected of them. We work closely with nursing homes and other institu-tions, Home Health Agencies, Social Security, Medicaid, and all other agencies necessary to make the transition. We also follow up on each client’s progress so any problems can be dealt with in a timely manner. Second Chances will also provide the first month’s rent and deposit so the client can move in. We arrange a very liberal repayment schedule based on the client’s ability to repay.

Founders

Darryl Williams was 19 and living in Memphis, Tennessee, his whole life ahead of him, when a shooting incident left him with a spinal cord injury that changed his life forever. Darryl was cared for by his mother for ten years, and when the time came to consider moving to a nursing home, Darryl “hit the ground rolling.” Through the Memphis Center for Independent Living he learned about the Underground Railroad, a program helping people transition to independent living in Colorado. In Colorado, Darryl volunteered for disability rights organiza-tions, learning everything he possibly could about independent living. “I knew that if I put myself in the right position, I could put my life back together and live the way that I wanted to. I learned that being knowledgeable about what’s going on in the disabled community could lead me to my dreams. It’s time for us as a disabled community to share information.”

DeVoe Mack was living in a nursing home in Memphis, feeling like “the life was slowly draining out of my body.” Darryl Williams had come back to Memphis to give a motivational speech about independent living, and DeVoe couldn’t believe what he heard. Living independently in an affordable home or apartment and getting adequate home health care every day sounded like a fantasy. Darryl and Barry Rosenberg of PASCO (Personal Assistance Services of Colorado) convinced DeVoe that it was possible, and with help from the Memphis Center for Independent Living and some close friends, DeVoe moved to an apartment in Lakewood, Colorado. DeVoe has a BA in cultural anthropology from the University of Pennsylvania and a minor in business law. He serves on the Board of Directors of Center for Independent Living Development, manages two housing complexes for the disabled, and is a real estate broker for Keller/Williams Realty.

Darryl Williams and DeVoe Mack know first-hand the value of personal connections to build community. Inspired by those who reached out to them, Darryl and DeVoe are dedicated to helping other people with disabilities bridge the distance between feeling helpless and hopeless ... and building community through empowerment and independence.
