National consultation on strategic use of ARV (Aug 2012) [1]
In order to reach both the targets of the national strategy and the HLM targets, Thailand must reduce the number of new HIV infections significantly
Interventions targeting the general population and key populations will not significantly increase condom use from present levels
In order to reach the needed substantial decrease of new HIV infections, Thailand needs to design a new form of combination prevention which maximizes the prevention effect of early ARV treatment
National consultation on strategic use of ARV (Aug 2012) [2]
Capacity constraints exist to deliver expanded services like ARV treatment, HCT, and adherence support through the health system alone. Alternative systems will be needed.
In order to maximize the prevention effect of ARV, present interventions have to change from “delivering information” to “delivering of services”
Needle sharing behaviour of injecting drug users must be reduced, as there is no prevention effect of early treatment

ConclusionsConclusions• In order to ‘end AIDS’ in Thailand, present
levels of behaviour change interventions need to be combined with the possible prevention effect of early treatment
• Investment needed to treat everybody independent of their CD4 cell count, and to strengthen adherence support is relatively small (US$ 60 million over next 10 years), would prevent an additional 10,000 HIV infections, and has a marginal cost benefit ration of 1:3

(health system at the limit of capacity) and can be combined with behaviour change interventions

• Quality assurance, quality control and a new form of monitoring systems must be adapted to account for community data and alternative service delivery
LimitationsLimitations
• AEM only until 2025• Several variables need to be adjusted and
discussed (e.g. % refusing treatment, progression, behaviour variables, etc)
• Full cost-benefit very complex and difficult (productivity, loss of human capital, out of pocket spending, etc)
• Alternative service delivery models (recruitment, CCT, ARV, adherence) do not exist yet and costing is hypothetical
