Enrollment Agreement
Thank you for choosing Faith Heritage School to partner with your family in the education of
your child(ren). It is an honor and privilege for us to be able to come alongside you in this
endeavor. Our goal is to provide not only the academic excellence expected for your student(s)
but also the spiritual setting where your child(ren) can flourish and grow in their relationship
with our Lord and Savior. In order to accomplish this, outlined below are the expectations
concerning our joint partnership relating to the financial responsibilities involved to accomplish
these goals.
1. Acknowledgement – Parent/Guardian acknowledges that the policies, procedures and
practices of Faith Heritage School (FHS), including, but not limited to, those set forth in the
Parent/Student Handbook and the Family and School Covenant are incorporated in this
agreement by reference. Parent/Guardian agrees to abide by all such policies and procedures and
acknowledges that FHS may make periodic modification to such. FHS will provide adequate
notification of such modifications.
Parent/Guardian attests that the information provided in the Student Application is accurate and
will notify FHS immediately of any changes. Enrollment at FHS is a privilege, not a right, and
suspension or termination of enrollment is at the sole discretion of the FHS administration.
During the term of this agreement, the parent/guardian does hereby promise and agree to fully
and timely pay FHS (in accordance with the payment plan elected) all tuition, charges, and fees
associated with the student’s admission, enrollment and/or participation at FHS. The tuition and
fees for any subsequent school years are set by the FHS Board of Directors by the spring of the
current year.
The Term of this agreement begins at the signing of this document and extends through the
graduation of the student(s) from FHS or as written termination of this agreement is provided.
This term shall, therefore, renew automatically for each successive academic year (July 1 – June
30) until graduation from FHS unless terminated by written notice from the parent/guardian.
2. Enrollment/Reenrollment – Families will remain enrolled year after year unless written
notice is provided. After the school year (beginning July 1 and ending June 30) has commenced,
any student not returning to FHS would be considered a withdrawal (*see #4).
3. Payment – Parent/Guardian understands that tuition payments are due based on the plan
arranged as offered through our invoice and billing service, FACTS Management:
a. Annual Payment – full tuition due on August 1st (Discount 0.5% off base tuition)
b. Semi-Annual Payment – ½ due August 1st and ½ due February 1st (No discount)
c. Monthly Payment* – ACH payable on agreed date within the month
d. Monthly Payment – Invoice agreement due the 1st of each month
* The Monthly ACH agreement plan is the preferred payment method of Faith Heritage School *
4. Withdrawals / Dismissals – Parent/Guardian understands that the overhead expenses of
the school do not diminish with the withdrawal of students. By accepting this contract with FHS,
they accept the obligation to pay on a timely basis the tuition due during attendance including the
current month and one additional full month of tuition after the withdrawal date.
5. Overdue Tuition Policy – Parent/Guardian understands that report cards, transcripts and
other such student records will be withheld should their account become delinquent. Should a
family account be in arrears at the end of a semester, the student(s) will not be allowed to attend
classes until the account is made current and student will be considered withdrawn if account is
not promptly settled.
I understand the Enrollment Agreement and agree to the terms as set forth above for the
Workbooks, Field Trips, Retreats, Graduation Cap & Gown, Planners, Specialty Classroom Supplies &
Books, Technology Services & Fees, Specific Physical Education Supplies, 1 Yearbook per High School
Family, Specialty Classes (Science Labs, Art Classes, Band, Choir).

Financial Assistance Information 2017 - 2018
There are several programs the school has implemented to help families reduce the cost of a Christian Education at Faith Heritage School. This summary is intended to acquaint you with the various programs. More detailed information and appropriate forms are available from the school office.
1. Need Based Financial Aid:
Faith Heritage School reserves a fund every year to provide assistance to families seeking aChristian education for their children who may not have the means to pay full tuition. Families canapply during one of three separate application periods prior to the start of school as follows:
Application Deadline – May 19th Award Date – May 31st
Application Deadline – June 30th Award Date – July 14th
Application Deadline – July 31st Award Date – August 18th
Due to the limited amount of financial aid available, families with AGI (adjusted gross income) in the following amounts will not be eligible for a financial aid award:
The maximum award possible will not exceed 35% of base tuition, the minimum award is $200
Applications must be made annually through FACTS Grant and Aid Assessment with a non-refundable cost of $30. The method of application is via the online website at

New families must apply to Faith Heritage School prior to being considered for an award.
2. Faith Heritage School Tuition Credit Plan:
For every family you introduce to Faith Heritage that enrolls for the 2017-2018 school year, yourfamily will receive a one-time $500 tuition credit. At the time of enrollment, the introduced familymust sign a Tuition Credit form indicating which family encouraged their enrollment.
3. Pastors and Missionaries Discount:
Families of full time ordained Pastors, Associate Pastors, and Youth Pastors of establishedchurches and Missionaries serving with established mission agencies are eligible for a 40%discount on base tuition. The primary source of income for the family must be from theministry position. This discount must be applied for annually.
FHS utilizes FACTS Management as our tuition & billing service., FACTS Grant & Aid is used as our Financial Aid data system.
1. Tuition Account Setup
There are several ways to make the tuition payments at Faith Heritage School.
Full Payment – This payment is due August 1st. With this payment, there is a discount of0.5% off base tuition.
Semi-Annual Payment – This payment is due August 1st (1/2) and February 1st (1/2).
Monthly Plans – These plans require the family to set up an online agreement with
FACTS Management at https://online.factsmgt.com/signin/3M7N3.
There is a choice of two plans lengths available.
12 Month Plan – July through June. 10 Month Plan – September through June
There is a choice of two plan types available:
Invoice Agreement – FACTS Management will mail or email an invoice aroundthe 12th of the prior month of which payment is due. Payment will then be due onthe 1st of the month. Any payment that is received after three business days ofthe 1st will be assessed a $25 late fee.
ACH Agreement – Payment will be deducted directly from your checking orsavings account on a specified day (typically the 5th or 20th of each month;however, a different day may be requested).
