illness, people dealing with addiction,

people who are homeless and

newcomers to Canada.

Eligibility Most services and programs are offered

to Centretown, the Glebe and Old

Ottawa South residents. See p. 2 for a map

of our catchment.

Some programs are open to everyone in

Ottawa and surroundings (indicated in

After-Hours Care

When the clinic is closed, we have a physician on-call to help our registered medical clients with urgent matters. 613-233-4697 Medical emergencies: Go to a hospital emergency department or call 9-1-1.

Our Centre’s medical clinic serves

registered medical clients only. A few

times a year, we open our waiting list.

When we do, it’s posted on our website.

Registered medical clients can walk-in

to see a nurse practitioner or a family

physician. They have access to all

clients’ medical records. First come,

Glebe Teen Health Clinic Are you a student at the Glebe Collegiate Institute? Our nurse practitioner offers confidential health and wellness services. Drop in or call student services to book an appointment.

Type 2 Diabetes Education

If you have ever been diagnosed with Type 2 Diabetes you may want or need some support. Come to one of our group programs or meet one-to-one with a Diabetes Educator. Our program serves people who are starting on or already on insulin as well.

Prediabetes Education

If your Health Care provider has said that your “sugar is a bit high” or that you have Prediabetes you can attend our group education program or meet with a diabetes educator. To attend a group education session or meet with a Diabetes Educator, please register with Diabetes Central Ottawa at 613-238-3722 or online www.diabetesottawa.ca. You can refer yourself or be referred by your Health Care Provider. Some of our services (for example, if you need insulin) need to be referred by a doctor.

Community Diabetes Education Program of Ottawa – serving the entire City of Ottawa Registered Diabetes Nurse and Dietitian Educators provide information and support to adults with Type 2 Diabetes, and Prediabetes. We offer group sessions and individual appointments with a Nurse and/or Dietitian. We have programs and services during the day and evenings at locations throughout Ottawa.

Group sessions will give you an overview of Prediabetes and Type 2 Diabetes and how it can affect the body as well as help for you to manage your condition yourself. We offer many workshops on key topics to help you manage diabetes and prediabetes including workshops about nutrition, being active, understanding medications, and monitoring your condition to promote wellness and stay healthy.
