Aldosterone is a steroid hormone produced by the zona glomerulosa of the adrenal cortex in the adrenal gland It increases the reabsorption of sodium and water and the release of potassium in the kidneys. This increases blood volume and therefore, increases blood pressure.

Eplerenone

Mechanism of action

Eplerenone binds to the mineralocorticoid receptor and blocks the binding of aldosterone, a component of the renin-angiotensin-aldosteronesystem (RAAS).

IndicationsEplerenone is indicated for Congestive Heart Failure Post-Myocardial Infarction Hypertension

Dosage and administration

Congestive Heart Failure Post-Myocardial Infarction

25 mg once daily Titrate to 50 mg once daily, within 4 weeks

ContraindicationsEplerenone is contraindicated in all patients with the following: Hypersensitivity to Eplerenone or any component of this medication Patients with clinically significant Hyperkalemia; Severe hepatic impairment Serum potassium >5.0 mmol/L at initiation; Moderate to severe renal impairment [creatinine clearance 221umol/L; Concomitant use with potassium-sparing diuretics, potassium supplements or strong CYP3A4 inhibitors such as ketoconazole, itraconazole, nefazodone, telithromycin, clarithromycin,ritonavir, and nelfinavir
