Terms & Conditions: Licensed builders, contractors, or installers purchasing LP SmartSide products will receive a rebate from LP in the following amounts subject to all terms, conditions and rules in this rebate form:

Builder cannot have submitted for a rebate request to LP for SmartSide products in the past two years.
One rebate per builder/contractor/installer - $1,000 maximum This rebate may not be combined with any other rebate or discount on LP SmartSide products. Rebate applies to LP SmartSide products purchased between January 1, 2016 and December 31, 2016 and is limited
to the first 80 complete submissions received by LP for the Midwest Region.
All completed rebate forms and supporting documents must be received by LP prior to January 31, 2016.

This rebate offer is made available to licensed builders, contractors or installers for their first time use of LP SmartSide only. Each rebate offer shall be subject to review and approval by LP and shall be subject to the terms and conditions of the rebate. This rebate applies only to product purchased by December 31, 2016. Limit one rebate per builder/contractor. This rebate is NOT available to homeowners, owner-builders, electricians, plumbers, heating & A/C, roofers or sub-contractors. Rebate checks will be issued to eligible builders, contractors or installers based upon the verifiable information provided on the registration form. Invoice must show the date of purchase, product description, builder’s name and address, and the purchase price. Failure to provide this information may result in the delay or denial of the rebate. All forms and invoices must be postmarked by January 31, 2017 to be eligible for the rebate. Please allow 6-8 weeks for your rebate to be processed. LP reserves the right to modify this program or terminate it at any time.
