The heart of CMC is to provide health, healing, and hope to patients and their families in a manner that offers innovative high quality services, compassion, and respect for the human dignity of every individual who seeks or needs our care. We accomplish this mission by focusing on five important pillars: People, Service, Quality, Finance and Growth. These pillars serve as the foundation of everything we do.

Throughout this Annual Report you will find information about how the work we do is made possible because of our People. The board members, physicians, healthcare providers, staff and volunteers compassionately make decisions about our patients’ experiences and care, hoping to provide them with the information and support they need to make important medical decisions.

Catholic Medical Center is passionate about providing quality care and the finest experience to all patients regardless of their ability to pay. This is a vital component of our Service pillar. As part of this pillar we focus on the community health services that are offered through CMC. These programs provide prenatal care for low income women, primary care for refugees, oral healthcare for low income or uninsured children and adults, healthcare for the homeless of our city, and medication assistance for those who are unable to afford lifesaving medications. CMC was proud to invest over $72 million dollars in services to support our community.

The Quality pillar is represented through the caliber of care we provide to our patients. Our physicians and staff work tirelessly to ensure that our patients are cared for in accordance to federal guidelines and that the hospital exceeds national expectations of safety and infection control. In fiscal year 2015, CMC was given many awards in recognition of the quality of the care delivered to our patients. We are proud to list them on the back cover of this report.

I see our Finance pillar as an important element that supports the care of our patients. Thanks to engaged donor support and sound financial management, CMC is able to invest in lifesaving technologies, cardiac research, top notch care providers and the expansion of vital community health programs. The strength of our finance pillar allows CMC to invest in keeping quality, compassionate healthcare close to you and your family.

The pillar for Growth focuses on looking at strategies to support the future of Catholic Medical Center’s programs and services for the future. We have some exciting plans for the future that involve the expansion of new programs and facilities. These expansions will help us better meet the needs of our patients and their families.

History

Catholic Medical Center was established in 1974 with the merger of Notre Dame Hospital and Sacred Heart Hospital, both Manchester-based Catholic Institutions founded in the 1890s. CMC is a not-for-profit, tax-exempt licensed 330-bed full-service acute care hospital with an affiliated medical staff of over 500 physicians serving the residents of the greater Manchester area and throughout New Hampshire. CMC has one of the highest case mix indexes in the state, which means that we serve some of the most critically ill patients in the state of New Hampshire.

Along with sophisticated clinical and medical quality, our compassionate care extends to vital programs and services to meet the health needs of our community, including the most vulnerable and medically underserved populations.

Between July 1, 2013-June 30, 2014 (FY2014), CMC provided $72.9 million to care for our community. CMC was ranked the highest in NH (as percentage of revenue) at about 19% and well above the average of 7.5% for US hospitals. Subsidized Health Services CMC underwrites these services and seeks donor and grant support to increase access to care for those in need, including:

Compassionate Excellence

Community Health Improvements Based on needs identified in the 2013 Greater Manchester Community Health Needs Assess-ment, CMC regularly provides community-based health education, prevention and screening programs, including:

• Community health education and physical fitness classes

• Health screenings at little or no cost to community members, including breast, cervical and skin cancer screenings

• Fruit and Vegetable Assistance Program• Parish Nurse Program• Patient transportation services• Fertility Health Education Services• Wellness programs for the homeless

CMC Named 2015 “Health Care Business of the Year” by Business NH Magazine

We are so proud of this award. It acknowledges the contributions we make to our community, patients, and employees. Our providers, nurses and allied health professionals provide outstanding care in a wide range of settings. Managers, administration and support staff are committed to making daily operations more patient-focused and efficient. Many of our heroes are located off campus or behind the scenes. It recognizes all of the work that goes into each of our pillars: people, service, quality, finance, and growth.

On Top of the World: Conquering Cardiac Rehab and Mount Fuji

Since his days as a US Navy pilot, Howie Wheeler wanted to scale Mount Fuji in Japan. But Mount Fuji faded to a far-away dream as Howie and his wife raised their three daughters and lived full lives.

A near-death experience brought the dream back into focus. “I thought I had no limitations, because I had always been reasonably fit and followed a good diet,” says Wheeler. In June of 2013, Howie was mowing his lawn and nearly passed out. “Next thing you know, my doctor tells me I needed coronary artery bypass graft surgery because of serious plaque build-up in my heart,” says Wheeler. He was immediately admitted to CMC, where doctors performed surgery. “I was out of intensive care in a day, in

recovery for a week and then discharged,” explains Wheeler.

Every day he felt noticeably better. Howie was pain free and quickly gaining strength in CMC’s cardiac rehab center, where he performed light exercises for eight weeks under the watchful eye of nurses and trained exercise specialists.

But Mount Fuji beckoned. “My nurses urged me to join the CMC Wellness Center, which is known as phase three of cardiac recovery,” says Wheeler.

After nine months of training, and a practice hike up the Mount Washington Auto Road, Howie was ready for the next step: Japan. A little more than a year after his surgery, Howie arrived at the base station of Mount Fuji. He was the oldest member of his hiking party. After hiking to 11,000 feet, “I watched the sun rise above the clouds and it was breathtaking. It was like an upside-down sunset,” says Wheeler. Later that day, Howie reached the summit of Mount Fuji, elevation 12,388 feet. A 30-year dream was realized when he safely made it back down.

Around that time, the phone rang at CMC’s cardiac wellness center. “Howie called me from Japan,” recalls Mary Kay Smith, a Cardiac Rehab Nurse Specialist. “That is how much he cares about our program. It was like he was calling home.” Says Wheeler, “I see myself as a result of my care from the nurses and Mary Kay, and their guidance through the phases of recovery. I will do that for the rest of my life.”

What’s next on Howie’s bucket list? “I plan to go to Scotland and climb the highlands area,” says Howie. “I’m getting ready and will train at the wellness center.”

In the meantime, Howie volunteers for CMC’s Caring Hearts Program where he shares his story with patients in the hospital as they are recovering from open heart surgery.

Our Board and Senior Leaders have a responsibility to keep our finances stable, despite an unstable healthcare market of ever-changing insurance demands and ever-increasing technological advances. We are pleased to report that Standard & Poor’s (S&P) increased CMC’s long-term bond rating to A- with a stable outlook, and Moody’s upgraded CMC’s outlook from stable to positive.

These represent independent endorsements of our hard work, and make Catholic Medical Center stand out over the majority of other hospitals.

FY15 Improvements • Expanded access to care through: - The New England Heart Institute

becomes The New England Heart & Vascular Institute (NEHVI). Launch of vascular and wound services at Huggins Hospital in Wolfeboro and vascular services at Overlook Medical Park in Derry
