Global Convenience

The U.S. Bank Executive Platinum Card contains both EMV Chip technology and a traditional magnetic stripe. Widely used outside the United States, EMV Chip cards are embedded with a microchip and use either a signature or a Personal Identification Number (PIN) for transaction verification. Combining both technologies on your card ensures that your purchases can be conveniently processed around the world.

USing yoUr EMV Chip CArd When using a chip-reading terminal, follow this process:

• Insert your card into the terminal• Leave your card in the terminal for the duration of

the transaction• Follow the prompts on the terminal as a guide, and

either sign your name or enter your PIN• Remove the card once the transaction is complete

STATEMEnT CrEdiT progrAM*The U.S. Bank Executive Platinum Card lowers the cost of airline amenities and services through its statement credit program. With this benefit, your account will be credited up to $100 per month for domestic airline ancillary service charges that are $50 or less. These service charges are typically incurred for airline airport lounge access, seat upgrades, in-flight meals, and checked bags. The statement credit benefit helps U.S. Bank Executive Platinum cardholders enjoy greater choice for less. Simply use your U.S. Bank Executive Platinum Card to pay for the fee, and your account will be credited for that amount within 10 days.

CorporATE rEwArdS MEMBErShipU.S. Bank Corporate Rewards is a turnkey program that offers a complete selection of Airline Rewards, Merchandise Rewards and Gift Certificates. Members earn one point per dollar charged to their card and are provided with convenient 24/7 online access to view and manage the points they earn for a wide range of redemption options.

There are no air travel black out dates, seating restrictions or Saturday night stays required. A 21-day advance reservation is the only requirement.

STATEMENT CREDIT PROGRAM*The U.S. Bank Executive Platinum Card lowers the cost of travel amenities and services through its statement credit program. With this benefit, your account will be credited up to $1,200 per year for ancillary travel service charges. These service charges are typically incurred for airport lounge access, Global Entry application fees, seat upgrades, in-flight meals and internet, and checked bags. The statement credit benefit helps U.S. Bank Executive Platinum cardholders enjoy greater choice for less. You may use the entire $1,200 statement credit in one month or throughout the enrollment year. This allows you to choose how to use your travel benefit — whether on annual or a la carte airport lounge access or other expenses.

Simply use your U.S. Bank Executive Platinum Card to pay for the fee, and your account will be credited for that amount within 6-8 weeks.

FLEXPERKS CORPORATE REWARDS MEMBERSHIP

FlexPerks Corporate Rewards is a turnkey program that offers a complete selection of Airline Rewards, Merchandise Rewards and Gift Cards. FlexPerks Corporate Rewards shares the rich and flexible redemption features of its consumer counterpart, the award-winning FlexPerks Travel Rewards Visa Signature® card.

ConCiErgE QUESTionS & AnSwErSHow do I get these services?Have your account number ready and call the toll free Visa Concierge Service phone number at 800-895-8518.

How does the concierge know I am eligible for this service?The concierge will identify you by your 16-digit Visa account number.

How long will my request take?Depending on the request, the concierge may be able to assist you on the phone. If the request requires additional research, then the concierge will e-mail or place a follow-up call to you. Most requests are completed within 24 hours. Your concierge will notify you of the anticipated time for completion.

How do I pay for what the concierge has purchased?The concierge will advise you of the costs. The items or services purchased will be billed to your Executive Platinum Card. There is no cost for the concierge’s time or assistance.

When is this service available? The service is available 24-hours a day, seven days a week.

Is there a limit on the number of times I can use this service?No, you may use the service as often as needed.

What types of concierge services are available?Concierge services are available for entertainment, planning, business services, travel information and assistance, gift arrangements, country or city information, pre-trip assistance and specialty services.

Complimentary Upgrades from Golden Tickets, Inc.U.S. Bank Executive Platinum cardholders can order tickets for any sporting or entertainment event through Golden Tickets and receive an automatic upgrade to the next best available seating.

Golden Tickets provides ticket brokerage services for events across the country. Reservations can be made by calling the toll-free number at 800-288-2461 at least three weeks in advance. By specifying that the transaction will be placed on a U.S. Bank Executive Platinum Card, executives receive:

• Automatic upgrade to the next best ticket category• Best quality seating available charged for the price of

the next less expensive level

10% Discount on Carey Worldwide Chauffeured Limousine ServicesThe U.S. Bank Executive Platinum Card entitles Executive Platinum cardholders to a 10% automatic discount on any limousine service rental with Carey Limousine. Carey provides complete chauffeur driven limousines, sedans, vans and minibuses in 550 cities and 60 countries.

Additional services include “as-directed” hourly services, airport transfers and group programs. English-speaking chauffeurs and chauffeur guides are available throughout the world.

As a Preferred Service member, executives enjoy speed renting services at more than 250 locations across North America and hundreds more around the world.

• Reservations can be made by calling the exclusive Avis Preferred Service line at 800-331-7423

• U.S. Bank Executive Platinum cardholders receive a free upgrade on their first rental at most Avis Preferred service locations

How do I receive my Executive Platinum Card benefits? • Statement Credit Program* — Use your U.S. Bank

Executive Platinum Card to pay for any domestic airline ancillary service charges of $50 or less, and your account will be credited for that amount within 10 days, up to a maximum of $100 in credits per month.

• U.S. Bank Corporate Rewards — ExecutivePlatinum cardholders are automatically enrolled in U.S. Bank Corporate Rewards. Simply visit www.usbcorporaterewards.com to create an online account, view reward account balance and redemption information. Members can also contact the Rewards Center 888-229-8864 for more information.

• Concierge Service — Refer to Concierge Services Section of this brochure.

* Executive Platinum cardholders receive dollar-for-dollar credits for domestic ancillary airline purchases (“Airline Purchases”) up to $50.00 each, not to exceed $100.00 in credits each calendar month. Such credits will appear on Executive Platinum cardholder’s monthly account statement no later than 10 days after the Airline Purchase posts to the account. Airline Purchases of $50.01 or greater do not qualify for the credit. Credits do not carry over from one calendar month to the following calendar month, nor is it possible to earn credits in advance of Airline Purchases. Partial credits may be earned. For example, if an Executive Platinum cardholder earns an $80.00 credit in one calendar month and has an additional Airline Purchase in the amount of $35.00 in that same calendar month, a credit of $20.00 rather than a credit of $35.00 will be earned for that Airline Purchase. That’s because a credit $35.00 added to the existing $80 credit would exceed the allowable $100 maximum by $15.00. Domestic Airline Purchases may include but are not limited to airline lounge visits, seat upgrades, baggage charges and in-flight meals and vary depending on the airline.

• Auto Rental Collision Damage Waiver — Provides coverage for damage due to collision or theft of most vehicles. Coverage is primary when used for business purposes for eligible rentals up to 31 consecutive days. Certain restrictions, limitations and exclusions apply. Refer to the Your Visa Card Guide to Benefits brochure for details.

• Lost Luggage Reimbursement — When paying for the cost of a common carrier ticket with the U.S. Bank Executive Platinum Card, Executive Platinum cardholders are eligible to receive excess reimbursement for the cost of replacement luggage and its contents up to the amount paid to the cardholder by the common carrier, or $5,000 — provided the luggage was checked with the common carrier and lost due to theft or misdirection by the carrier. Refer to the Your Visa Card Guide Benefits brochure for details.
