OBSERVATION AND NATO “ODCR” FORMAT

Title

• Observation Description: “what happened?”

• Discussion: “why did it happen?”

• Conclusion: “what can we learn from this?”

• Recommendation: ”who can do what about it?”

An Observation - this is a comment based on something someone has

heard, seen or noticed that has been identified and documented as an

issue for improvement or a potential best practice.

“ODCR” FORMAT EXAMPLE

Observation Description: During Exercise Cooperative Archer it was observed that there is a

lack of medical equipment, individual protective equipment, training/qualification on such

equipment and training in aeromedical evacuation procedures. (What happened?)

Discussion: During the exercise it became apparent that there was no individual protective

equipment for personnel working on the flight line or on board aircraft, despite the fact that even

ERO (Engine Running Offload) operations were being conducted. No doctors were qualified as

flight surgeons, and other medical personnel had no specific training in the areas of flight

medicine. There was a lack of electro medical equipment for use on the aircraft. Ambulance

drivers were unfamiliar with flight line procedures such as the approach to aircraft. Overall there

was insufficient familiarity with, and provision of, acceptable levels of aeromedical evacuation

equipment during the exercise. (Why did it happen?)

Conclusion: There was insufficient equipment and training for personnel involved in the

aspects of aeromedical evacuation during Exercise Cooperative Archer. (What can we learn

from this?)

Recommendation: Investment should be made in individual protective equipment and training

for personnel involved in flight-line and in-flight procedures. Key medical personnel require

training in flight medicine, aeromedical evacuation and flight physiology. Electro medical

equipment (subject to airframe certification and airworthiness) should be obtained for use on

board aircraft, and personnel should be trained in its use. (What can we do about it?)

What should we avoid when drafting Observations?

• Avoid using Observations to air complaints;

• Avoid to prepare observations about some issues which can be quickly

resolved within departments;

• Make sure the issue is applicable.

• Make sure that the Description includes all the key facts.

• Avoid introducing new data in Conclusion.

• Ensure that your analysis allows you to describe the root causes, not the

symptoms. Remedial Action which solves the symptoms only, will not solve the

issue.

• Example: If you have a terrible headache from dehydration,

Aspirin ® will only fix your pain: it will not address

your dehydration.
