• EXPLAIN GEORGIA’S AGING AND DISABILITY RESOURCE CONNECTION (ADRC) NETWORK STRUCTURE AND THE WAY INDIVIDUALS ARE CONNECTED TO RESOURCES AND SERVICES THROUGH THEIR LOCAL AREA AGENCY ON AGING.
• 2. DISCUSS HOW MAJOR SERVICES AND PROGRAMS FOR OLDER ADULTS THAT SUPPORT HEALTHY, INDEPENDENT AND ENRICHED LIVING WHICH CAN HAVE AN IMPACT ON THE REDUCTION OF HOSPITAL READMISSION.
• 3. CONNECT OLDER ADULTS AND/OR THEIR CAREGIVERS TO THE TOOLS AND APPROPRIATE RESOURCES.
• 4. DESCRIBE HOW THE ENHANCED SERVICES PROGRAM (ESP) IS A PROFESSIONAL TOOL USED IN GEORGIA BY THE AGING AND DBHDD NETWORK TO LOCATE SPECIFIC SERVICES AND PROGRAMS.
• 5. DISCUSS THE IMPORTANCE OF HOW LICENSED HOME CARE PROVIDERS AND PERSONAL CARE HOMES SELF REPORT IN THE ESP DATABASE IF THEY CAN SERVE PERSONS HAVING A MENTAL HEALTH DIAGNOSIS.
