Authority) is a fully owned Government of Kerala company set up in 2007

for providing services to the various health care institutions under the

Department of Family Welfare and Health. One of the key objectives of the

KMSCL is to act as the central procurement agency for all essential drugs

and equipments for all health care institutions (hereinafter referred to as user

institutions) under the department. The corporation has also been entrusted

with the setting up and running of all kinds of modern Medical and

Paramedical or medical based ancillary facilities such as hospitals,

pathological labs, diagnostic centers,

x-ray/scanning facilities.

1.2.1.2.1.2.1.2. The Government has entrusted KMSCL as the management agency for

‘108’ emergency ambulance service which is called as “Kerala Emergency

Medical services Project” (KEMP). The KEMP project was initially launched

as a pilot study in the district of Thiruvananthapuram on 19th May 2009 with

25 Advanced Life Support Ambulances (ALS), which has so far attended

more than 50,000 emergencies. On successful pilot study the project was

expanded to Alappuzha district on 20th April 2012 with 18 ambulances. The

project is soon to be expanded along the national highways in the district of

Kollam so that the national highway from the stretch of Parassala to Aroor is

completely covered by KEMP services. Now the State was sanctioned with

283 Patient Transport ambulances under Janani Shishu Suraksha

Karyakram (JSSK) and 218 Basic Life Support Ambulances. Hence it was

decided to invite tender for providing KEMP services to the entire State of

Kerala.

1.3.1.3.1.3.1.3. In this tender,,,, Expression of interest is invited from the firms who are willing

to operate 24x7 pre-hospital emergency ambulance service in the entire

State of Kerala and also to operate referral transport for pregnant women

under JSSK. The patient transport ambulances will be used for inter

hospital patient transfer when not used for JSSK service. The present control

room and available 50 ALS ambulances will be transferred to the successful

bidder. It is proposed to set up one more control at a different location

which shall function on a disaster recovery site/redundancy. The

procurement of ambulances has to be done by the bidder and the capital

expense will be reimbursed by Tender Inviting Authority.

1.4.1.4.1.4.1.4. It is also essential while dealing with public money that utmost transparency

has to be maintained in the procurements of the corporation. All decisions

will be published from time to time on our website www.kmscl.kerala.gov.in.

The corporation will not wait for the mandatory 30 days period to provide

any information under Right to Information Act and will provide the

information within the minimum possible time. Bidders could prefer appeal

to the government against all decisions of the corporation.

2.1.1. The bidder will be required to operate 24x7 pre-hospital emergency

medical service under KEMP and also transport pregnant women during

their routine visits to hospitals at various stages of pregnancy and during

emergencies under JSSK and assist in any disaster situations for the entire

state of Kerala with centralized control room having one single toll free

number ‘108’.

2.1.2. This Expression of Interest (EOI) is invited for operating 551 ambulances

service for a period of 5 years from the date of signing the agreement which

can be extended further to a period of 2 years with mutual consent.

2.1.3 The successful bidder has to scale up the existing control room, establish a

new control room at another location in Kerala, procure ambulances, establish training centre, recruit human resources and effectively operate the ambulance service with minimum 99.5% uptime. At any given instance only a maximum 2% of the vehicles should be withdrawn from the field for routine maintenance etc.

2.1.4 Bidders are expected to submit EOI documents and based on the proposal

submitted eligible firms will be short listed. Only short listed firms will be

provided Request for Proposal (RFP) Document and shall be requested to

submit final proposal. The request for proposal document will have the

detailed terms of reference. The specific criteria for evaluation will be

mentioned in the RFP document.

4.1.1 Overall Objective of this EoI is to engage agencies having sufficient and

proven expertise and experience of operating a network of emergency

medical ambulance services to provide 24x7 pre hospital emergency

medical ambulance service for the entire State of Kerala and to operate

patient transfer ambulances under one toll free number.

4.2.1 The successful bidder has to provide 24x7 pre-hospital emergency

ambulance services in the entire State of Kerala and also operate referral

transport for pregnant women under Janani-Sishu Suraksha Karyakram

(JSSK). The ambulances for JSSK shall be used for patient transfer

during the rest of the time. All these ambulance services shall be

operated through calls obtained in one single toll free number ‘108’.

4.2.2 The successful bidder has to operate 283 patient transfer ambulances,

218 BLS ambulances and 50 old ALS ambulances. Out of the 283

patient transfer ambulances 10 ambulances shall be 4 wheel drives

suitable for providing service in hilly areas.

4.2.3 At present KEMP ‘108’ ambulance service is operated in the district of

Thiruvananthapuram with 25 ALS ambulances and Alappuzha with 18

ALS ambulances and 4 ALS ambulances to be deployed at NH – 47

passing through Kollam district and 2 ALS ambulances has to be

replaced by insurance company under Public Private Partnership.

4.2.4 The existing 50 ALS ambulances will be transferred to the successful

tenderer and have to be redeployed across the State, which at this stage

will be on the high accident prone areas of National Highway and Main

Central (MC) road (SH-1). The maintenance of the 50 old ALS

ambulances has to be taken care of by the successful tenderer for the

entire period of the contract. The detailed technical specifications and

other facilities of the existing ALS ambulances are provided in Annexure

V & VI.

4.2.5 The existing control room located at Technopark Thiruvananthapuram

will be transferred to the successful tenderer. This control room shall be

expanded to provide services to 7 districts and one more control room

shall be established at a different location preferably Kozhikode in the

State of Kerala for providing service to another 7 districts. In case of

disaster like fire, etc at one control room the entire operation has to be

switched over to the available second control room without any major

downtime, the other control room should be able to provide

uninterrupted service to the entire State.

4.2.6 The present control room at Thiruvananthapuram is located at

Technopark. The rental charges should be paid by the successful

tenderer on behalf of the Government. The ideal location for the new

control room has to be identified by the successful tenderer.

4.2.7 Capital expense (CAPEX): The expenses of the following capital cost

shall be paid upfront to the successful bidder with 90% down payment

and remaining 10% on the basis Bank Guarantee. The Tender Inviting

Authority shall pay the following expense on a lump sum amount.

4.2.7.1 Scaling up of the present control room for 7 districts.

4.2.7.2 Establishing a new control room for the rest of 7 districts in another

mutually agreed location. This includes rental, designing, refurbishing,

IT infrastructure (hardware and software), office furniture and fittings,

communication hardware and accessories.

4.2.7.3 Procuring ambulances, fabricating and equipping, obtaining registration

and related formalities, insurance, GPS device and deployment.

4.2.7.4 Setting up training centre (Land to be provided by Government/TIA.

4.2.7.5 Setting up of Network Operation Centre (NOC) at KMSCL Head

quarters

4.2.8 Operating expense:(OPEX)

4.2.9 The ambulances with fabrication and equipments for the operation shall

be purchased by the successful bidder in the name of Director of Health

services and the specification will be provided by the Tender Inviting

Authority. All related formalities for the procurement and registration of

the ambulances as per the prevailing Kerala State rules has to be done

by the successful bidder. The capital expense of the ambulance

procurement will be paid on completion of job.

4.2.10 10% of the entire capital expense will be withheld or released on

submission of equivalent amount of bank guarantee for the entire

period of contract.

4.2.11 It is proposed to collect user charges from the patients who are using the

service for going to private hospitals.

4.2.12 A state of the art training center has to be established by the successful

bidder in the premises provided by the Tender Inviting Authority.

Periodical training has to be provided to doctors and paramedical staff

of the Government hospitals engaged in the service.

4.2.13 The successful bidder should have the assistance of doctors 24x7 for

monitoring and guiding EMTs during the emergency service.

4.2.14 The successful bidder should provide on-line access to the location

specified by the Tender Inviting Authority for all MIS like GPS tracking,

Voice recording, report dashboards etc. Also a network operating center

(NOC) for the tender inviting Authority to monitor the

deployment/movement of vehicles, CCTV from the two control centre

GPS/GPRS logs and thean MIS for managing the project should be

established at a place suggested by the tender inviting authority.

4.2.15 It is proposed to have one pilot (driver) and one Emergency Medical

Technician (EMT) for all KEMP ambulances. The patient transport

ambulance shall have only one driver.

4.2.16 It is also proposed to operate boat ambulances in some parts of Kerala

during the period of contract. The terms and the conditions shall be

finalized with the successful bidder on mutual agreement.

4.2.17 It is proposed to invite financial proposal for operational expense with

1500 km per ambulance per month and for additional kilometer will be

reimbursed at the rate of Rs. 15/- per kilometer for KEMP ambulance

and Rs.8/- for JSSK ambulance. Additional kilometers payment will be

reimbursed only if the average kilometer travelled by all the ambulances

deployed in the State, crosses the 1500 kilometer mark. This shall be

calculated separately for KEMP and JSSK ambulances.

4.2.18 Retaining the existing staff of 108 services in Thiruvananthapuram and

Alappuzha district shall be finalized by the successful bidder with the

present operating agency namely M/s. Ziqitza Health Care Ltd..

4.3.1 The emergency ambulance service and the system shall be implemented

in coordination with other agencies/ department like; Police, Fire,

medical, road transport, etc and to provide a smooth efficient service

without legal and operational complications.

4.3.2 Provide technological, leadership, administrative and managerial

support as the Partner in an open and transparent manner to produce

mutually agreed outcomes.

4.3.3 Develop a suitable integrated solution including Computer Technology

Integration, Voice logger system, Geographic Information systems (GIS),

Geographical positioning systems (GPS), Automatic vehicle Location &

Tracking (AVLT) Computer Aided Dispatch (CAD) and mobile

communication systems.

4.3.4 Operate the Emergency Response Center for 24 hours through the

dedicated toll free telephone number “108”. Shall provide sufficient

number of parallel lines with hunting facilities and enhance upon the

increase in number of calls receiving during the contract period.

Emergency Response Center may receive emergency call of the nature

of medical, police and fire fighting service.

4.3.5 Provide the Application Software for the project and the hardware

components. The successful bidder shall also update the software

periodically as per requirement. Any hardware which requires

replacement/upgradability with respect to the latest technological

advancement, which in turn enables to make the Emergency response

service faster and more efficient. An analysis in the most accident prone

areas/Zones shall be periodically handed over to the tender inviting

authority so as to liaise with the police and other departments to make

engineering modification to reduce accidents at such locations.

4.3.6 Serve as a vital emergency management information and assistance

resource and raise societal awareness of, and capability in, Emergency

Management and Response mechanisms and thus save lives and reduce

the economic impact to the citizens, firms and the government through

appropriate awareness, education and capacity building programs.

4.3.7 Operate the ambulances and ensure that ambulance services are

available on 24 hours per day and 365 days a year basis to the people

of Kerala without any charges being levied from the public opting

Government hospitals / healthcare institutions. The ambulances shall

reach the required spot within 20 minutes of alert.

4.3.8 Provide redundant systems for all the important components including

voice logger, call taking, vehicle dispatch, etc to provide hassle free 24

hours per day and 365 days a year basis emergency medical services.

4.3.9 Provide Un-interrupted power supply facility to the Emergency Response

Center to provide un-interrupted services.

4.3.10 All the ambulances should have GPS/GPRS device on it and two active

mobile phone connections of different service providers with maximum

local coverage. The Emergency Response Center shall communicate

with all the ambulances by means of voice as well data. The data

includes the destination name, landmark and other relevant information

shall flash on the GPRS device monitor kept on each of the ambulance.

The map shall be displayed on this monitor with current position of

ambulance, destination and all the nearest hospitals. In the later stage it

should be possible to highlight the shortest distance to the destination as

well as the hospital on the map displayed on the monitor. The device

should also have facilities to log the status of emergency calls attended

by the ambulances to the control room for the purpose of monitoring

and evaluation.

4.3.11 On receiving call of medical emergency, Emergency Response Center

shall communicate with the nearest ambulance and the caller, and

arrange to take the patient to the nearest hospital depending upon the

severity of the patient’s condition. On best effort basis, the concerned

hospital should be informed in advance. Any call requiring assistance

from police or fire fighting authorities shall be transferred accordingly.

4.3.12 In the event of multiple causality or major accident more than one

ambulance shall be deployed depending upon the condition within a

short time span.

4.3.13 It is necessary to introduce a foolproof system for collecting user fee from

patients and crediting it to an account and maintain a separate

accounting process.

4.3.14 Recruit, train and position the required man power, including Pilots

(drivers), and Emergency Medical Technicians [EMT] who will be

present in the ambulance while shifting an emergency case to a hospital

as per the Appendix 10. The recruitment and training has to be

completed prior to the launch of the project in every district without

creating any additional expense to Tender Inviting Authority.

4.3.15 Ensure that in every ambulance operated under KEMP scheme, shall

have at least one Pilot and one EMT present at any given point of time

to provide patient-stabilization, first-aid and other pre-hospital care. An

illustrative list of total number of personnel involved in pre hospital

emergency management and shall be appointed/hired by the successful

bidder is attached as per (Appendix 10). All ALS ambulance should

have EMT-advanced at any given time.

4.3.16 All the break-down and maintenance problems of the Ambulance

vehicles are to be done by bidder at its cost. Any warranty claims of the

vehicle shall be coordinated and executed by bidder at Tender Inviting

Authority’s satisfaction.

4.3.17 Bidder shall replace at its cost all the missing tools or equipments from

the ambulance with tools and equipments of same specification from the

supplier of the ambulance equipments and tools.

4.3.18 Bidder shall meet all expenses towards insurance, Annual Fitness

certification, Maintenance, Minor & Major repairs, replacement of tyres,

batteries etc.

4.3.19 Bidder shall maintain separate log books for vehicle and patient, stock

register and status register in all ambulance with relevant details in

consultation with Tender Inviting Authority. Bidder shall also implement

any system which Tender Inviting Authority intends to introduce.

4.3.20 Provide daily (operational), monthly (administrative and financial)

reports and quarterly (fund utilization) statements to the Tender Inviting

Authority through the MIS integrated with the call center and data logger

systems of the vehicle on a real time basis.

4.3.21 Attend periodical review meetings held by the Government or Tender

Inviting Authority (physically or virtually) for the assessment of the

operationalization of the scheme.

4.3.22 Maintain separate financial accounts and records of its operations in

Kerala. These accounts shall be duly audited by a Chartered Accountant

firm as approved by Tender Inviting Authority and furnished to Tender

Inviting Authority by the end of the first quarter of the succeeding year.

A trust may be formed by Tender Inviting Authority consisting of both

parties for monitoring and evaluation of the project.

4.3.23 Make all attempts to attend every emergency call that is received at the

Emergency Response Center.

4.3.24 Bring in technology and service excellence and work towards improving

delivery of emergency response of global standards over a period of

time.

4.3.25 Liaise with various departments and agencies of the Government of

4.3.26 Establish standards and procedures for accrediting various service

providers such as hospitals and ambulances so that quality standards are

met.

4.3.27 Conduct periodic training programs for paramedics, nurses, doctors and

others engaged in Emergency Response Service in the Government

healthcare institutions and also other academic activities

(workshops/seminars) as required for government doctors, nurses,

paramedics and others. Periodic training programs has also to be

conducted for the operational staff including Driver and Emergency

Medical Technician. Training should be given for at least one doctor,

two paramedics from all the hospitals where ambulances are parked and

few more doctors, staff whom SHFWS nominates.

4.3.28 Conduct periodic training programs to policy makers, government

personnel, school and college students and others.

4.3.29 Strive for continuous improvement in emergency management through

strategic partnerships, innovative programs, and collaborative policies.

4.3.30 Undertake applied research assignments in implementing Emergency

Response Services in the field.

4.3.31 Provide awareness building about the KEMP and JSSK across the state.

4.3.32 Prepare the standard operating procedure framework (SOPF) for

criteria mentioned above.

4.6.6 Bidders who submit all the necessary documents as prescribed for

inclusion in the technical bid under cl.4.8 without any ambiguity, errors

etc.

4.6.7 Bidders who have been blacklisted/ debarred by Tender Inviting

Authority or blacklisted / debarred by any State Government or Central

Government department/Organization should not participate in the EOI

during the period of blacklisting.

4.6.8 Firm / company who have withdrawn after participating in any of the

previous tenders of KMSCL are not eligible to participate in this EOI.

4.6.9 In case of consortium the lead member of the consortium shall meet all

5.5.2. There will not be any individual communication in respect

documents to prove the technical competency and capability of the

Bidders to the next level.

the Corporation shall assume no responsibility for the offer’s

5.8.1 EOIs must be received in the office of the Corporation at the above address

at Thiruvananthapuram not later than the time and date specified in the EOI

working day.

5.8.3 If the EOI is sent by Registered post or by Courier, it shall reach the above

Corporation shall not be held liable for the delay in transit where the EOI is

a. Patient’s cabin ergonomically designed and fabricated with FRP ( Fiber Reinforced Plastic ) and having a 3 layered section consisting of FRP, PUF, thermocol to have a noise free atmosphere and maintain the internal temperature. The panels are fire retardant, fully washable, have highest tensile stress to weight ratio. Made of medical grade which withstand corrosion caused by fumigation chemicals.

b. Designed to provide adequate comfort, space and safety for patient and patient’s attendant. Seat belts and restrains provided for every seat.

c. Overhead racks provided for storage of stocks with hydraulic doors which ensure that during fast movement, the doors of the cabinet do not open out.

d. The ambulance interiors designed with care to avoid injuries by fall of equipment or cylinder on persons inside the ambulance in case of turmoil due to bad road conditions.

e. Adequate space provided for storing medicines necessary for one week consumption in racks made of plywood coated with cold rolled non corrosive chemical resistant steel finish for easy cleaning and elegant look in addition to maintaining sterile condition.

f. Waste disposal done with the help of 2 steel dustbin demarcated into bio hazardous and non bio hazardous.

g. There is provision of wash basin with foot operated control for hygienic washing of hands without touching of tap.

h. Direct and diffused lighting for optimal patient’s comfort and provided with spot and focused lighting and special examination lights for the clinician to undertake “procedures” during transport.

i. The A/c power supply from the inverter which is fed by the high powered alternator of 90 amps which runs on engine power or solar power. An additional battery is provided for a back up of 4-5 hours which is continuously charged by this inverter while the vehicle is in motion and when not in motion the battery is powered by solar panel. Adequate power supply for medical equipment and other electronic equipments is provided with both 12 Volts DC and 220 Volts A/c. Anti Static flooring should be provided for safety.

j. Provision of hooks for hanging IV bottles on the roof. k. Syringe destroyer for disposal of Sharp wastes and needles. All equipments placed

appropriately to help Emergency Medical Technician to provide better care without much inconvenience & hooked tightly to the wall.

l. Air conditioning with 1.5 ton condenser with twin roof mounted blower fans placed at both ends of patient compartment in addition to adequate insulation of the patient’s cabin which help to provide immediate cooling for patient comfort.

b. Scoop Stretcher – Useful in shifting patients with multiple injuries and spine injuries and helps in shifting the patient keeping the spine intact. a. Patient’s cabin shall be ergonomically designed and fabricated with Fire retardant FRP (Fiber Reinforced Plastic) and having a 2 layered section consisting of FRP, PUF/ thermocol to have a noise free atmosphere and maintain the internal temperature. The panels should be fire retardant, fully washable, have highest tensile stress to weight ratio. Should be of medical grade and should withstand corrosion caused by fumigation chemicals.

b. Should be design to provide adequate comfort, space and safety for patient and patient’s attendant. Seat belts and restrains shall be provided for every seat.

c. Overhead racks shall be provided for storage of stocks with lockable sliding toughened Glass in Aluminum frame with rounded corners and guide rails, which shall ensure that during fast movement, the glass of the cabinet do not open out.

d. The ambulance interiors should be designed with care to avoid injuries by fall of equipment or cylinder on persons inside the ambulance in case of turmoil due to bad road conditions.

e. Adequate space shall be provided for storing medicines necessary for one week consumption in racks made of plywood coated with cold rolled non corrosive chemical resistant steel finish for easy cleaning and elegant look in addition to maintaining sterile condition.

f. Waste disposal shall be done with the help of 2 steel dustbin demarcated into bio hazardous and non bio hazardous.

g. There should be provision of stainless steel wash basin with foot operated control for hygienic washing of hands without touching of tap. Should have soap dispenser.

h. Should have direct and diffused lighting for optimal patient’s comfort and should be provided with spot and focused lighting and special examination lights for the clinician to undertake “procedures” during transport.

i. Adequate power supply for medical equipment and other electronic equipment is provided with both 12 Volts DC and 220 Volts A/c.

j. There should be a provision of hooks for hanging IV bottles with secured straps on the roof side.

k. Syringe destroyer for disposal of Sharp wastes and needles. All equipment should be placed appropriately to help Emergency Medical Technician to provide better care without much inconvenience & hooked tightly to the wall.

n. Oxygen Manifold System – High pressure tubing regulator, low pressure tubing, Oxygen 2+1 Common rail nipple with needle valve for ventilator in Advanced Life Saving (ALS) ambulance with flow meter and tubing supplying oxygen to victim using Face mask/ nasal prong. Two Oxygen cylinders shall be housed under the medical drug racks with easy push pull type arrangement for easy loading and unloading of heavy Oxygen Cylinders.

o. External charge port shall be placed at the rear end of vehicle for charging vehicle in case battery gets discharged due to medical equipment.

p. Should have LED flash lights one red and orange pair on each side and both red on the rear indicating emergency when ambulance is moving on the road

q. Should have one flood light on both side and one on rear side of the ambulance which shall provide lighting when operating in dark conditions.

r. Should have unique high intensity Siren and Public addressing systems consisting of 100 watt with LED flashers and lights indicating emergency while transferring a serious patient. Siren should have five different tones that can be interchanged in order to draw attention of public and take way in high density traffic.

s. Fire extinguisher – ABC type should be provided to extinguish fire for rescuing victims in fire accidents.
