Mr. Garofolo nominated Mr. Kubik to be the Chairperson. All were in favor and Mr. Kubik was appointed chairperson of the Facilities Committee.

1. Bungay School a. Oil Spill-Root Cause and Corrective Action

Mr. Falzone reviewed recent minor oil spill. The oil spill may have been caused by a defective vent pipe, which will be addressed when the weather warms up enough to inspect the oil tank and pipe. The contracted oil company has p~id all expenses to remediate the effects of the spill and subsequent deliveries have been monitored by a Seymour School employee. Mr. Falzone indicated that the oil tank would also be inspected and sludge removal would be done during the summer. Bungay School is the oldest building in district at this time and is need of major repairs. Major areas of concern are floor tiles, roof leaks, paving issues and heating issues including the need for a new boiler. Mr. Belden indicated that we are going to need to address these issues. 4. Seymour High School a. Boilers

The two boilers had eight sections replaced at a cost of $35,000 during the summer of 2017. The water returning to the boilers was too cold resulting in cracking. It was decided to call in an outside company to see if there are any solutions to this issue (i.e. insulating pipes, mixing value, and a system to monitor the return water temperature.)

b. Pool The pool has a dedicated boiler which runs 24/7 and is not energy efficient due to the length of the pipe run. Mr. Falzone reco·mmended the possibility of adding an auxiliary pump.

C. OCR Status Update Mr. Belden distributed a copy of our Voluntary Corrective Action Plan with the areas that we are continuing to address. Mr. Belden indicated we are chipping away at the open issues and Dr. Wood (State Consultant) is satisfied with the work that is being done. An annual report is prepared for the State OCR department with the status of the remediation efforts. Mr. Belden noted that there is significant work to be done in the locker room/gym areas if a student/staff member requires ADA accessibility.

B. Planned/Required Maintenance and Updates 1. Bungay School

4. Seymour High School Parking lot striping, locker repairs, recoat gym floor, replace state curtain, OCR Repairs, replace flooring The items above are the areas proposed to be repaired and maintained. These items were identified in the supporting detail for the 2018-19 budget.

C. Needed Equipment and Supplies The following equipment is budgeted for this 2018-19: 2 riding floor machines (one at CLS and one at SMS). Mr. Belden indicated that funding has been maintained in our annual operating budgets to replace custodial equipment in a planned manner.

D. Reviewof2017 Fire Marshall School Inspections Mr. Falzone provided a copy of the Fire Marshall's inspection report for 2017 (attached), along with the work performed to address the cited items. There are no outstanding issues.

State of Connecticut. This inspection is required every three years and there were no issues cited that require any remediation in the report.

Sections 1·2: Management Planner (MP) shall complete and submit form electronically to the LEA Designated Person (DP). If MP recommends a response action, other than operations and maintenance (O&MJ and/or Preventive Measures, submit Attachment A, detailing the specific recommendations for each school.

Section 3: Local education agency (LEA) shall complete and submit to the DPH within thirty-tao) days of the date al the relnspection. The DP must 11st the course name, dates and hours of training received to carry out the LEA's duties. If the DP satisfied training requirement by reading the DP's Self Study Guide, specify the dates and number of hours It was read. Description of Violation

Extension cords shall not be used as a substitute for permanent wiring.

A zip electrical cord is feeding a dehumidifier and pencil sharpener. The extension cord

must be removed and a proper way of feeding these devices must be found.

Live electricaJ equipment shall be guarded against accidental contact by approved

enclosures.

The wall electrical box where the dock used to be is not covered exposing live wires.

This box needs to be properly covered.

CONNECTICUT STATE FIRE SAFETY CODE ABATEMENT ORDER OF FIRE/LIFE SAFETY HAZARDS

On 8116/2017, at approximately 0900 hours, an inspection was conducted of the premises located at 2 Botsford Road, Seymour, Connecticut for the purpose of compliance with Connecticut Fire Safety Code and the applicable referenced applicable standards, adopted pursuant to the Connecticut General Statutes §29-292 and §29-293. The Code and said standards are available for your inspection at this office.

The violation of the code found on the date of inspection are listed on the attached inspection report.

You are hereby ordered to take the proper corrective action to remove or remedy all the listed violations within thirty (30) days from the day that this notice is received, unless otherwise noted on the attached mspection report.

If you believe that compliance with the code will impose an unreasonable hardship, and that alternative methods of achieving an equivalent level of safety could be attained, you may reques~ in writing, a modification of the requirements of said Code as outlined in the Connecticut General Statues § 29-296. If you believe compliance will take more than that specified, you may request an extension of time prior to the expiration of the thirty (30) day period, unless otherwise noted on the attached inspection report. Sample forms for application for modification and extension of time for compliance are available from this office. In addition you have the right to appeal this order pursuant to Connecticut General Statues §29-306 for a period of not more than thirty (30) days from receipt.

Plans/specifications for work to be done shall be submitted to this office prior to the commencement of any construction. This review of all plans/specifications would avoid unnecessary expense that could result from non-complying changes. Please note that the correction of certain violations may require proper permits and approval from the building official and other local agencies prior to any construction.

This is the only order you will receive. This office will conduct a re-inspection of the premises to determine compliance with this order after the expiration of the thirty-day (30) or the time period noted on the inspection report. Your failure to comply with this order within the period specified as determined by said re-inspection constitutes your failure to comply with the Connecticut State Fire Safety Code which may subject you to criminal prosecution as prescribed by Connecticut General Statues §29-306 with penalties of a fine not less than two hundred dollars ($200.) nor more than one thousand dollars ($1000.00) or imprisonment ofup to six months or both, as prescribed in Connecticut General Statues §29-295. Non-compliance may result in a civil proceeding against you as authorized in Connecticut General Statues§ 29-306

On 8/14/2017, at approximately 1030 hours, an inspection was conducted of the premises located at 211 Mountain Road, Seymour, Connecticut for the purpose of compliance with Connecticut Fire Safety Code and the applicable referenced applicable standards, adopted pursuant to the Connecticut General Statutes §29-292 and §29-293. The Code and said standards are available for your inspection at this office.

The violation of the code found on the date of inspection are listed on the attached inspection report.

You are hereby ordered to take the proper corrective action to remove or remedy all the listed violations within thirty (30) days from the day that this notice is received, unless otherwise noted on the attached inspection report.

If you believe that compliance with the code will impose an unreasonable hardship, and that alternative methods of achieving an equivalent level of safety could be attained, you may request, in writing, a modification of the requirements of said Code as outlined in the Connecticut General Statues§ 29-296. If you believe compliance will take more than that specified, you may request an extension of time prior to the expiration of the thirty (30) day.period, unless otherwise noted on the attached inspection report. Doors that open into exit access shall be self closing and self latching.

These doors did not properly latch.

A door leaf normally required to be kept closed shall not be secured in the open position

at any time and shall be self-closing or automatic closing in accordance with 7 .2.1.8.2,

unless otherwise pennitted by 7.2.l.8.3

This room is being utilized as storage and the door is to remain in the closed position.

Working space required by electrical panels must not be utilized for storage.

All storage must be cleared a minimum of 36 inches from the service panels.
